Maybe Toshiro didn't speak up to Laios before, because telling him "please shut up, we've been talking for 3 hours and I'm tired" was too rude and he is the never be rude guy.
Like He could have done it but this could have spelled the end of there friendship and Laios was the first friend he ever had so he didn't want that.
Like the poor guy was sooo lonely all his life and Laios was the first person to talk to him ever outside of his family and retainers.

Like they are clearly friends before the story start but the friendship is flawed, and Toshiro was kinda bearing all the burden from it before the fistfight.
And Laios didn't realize he was being a burden because he only ever was like that with his sister Falin before, and Falin too is supportive to her own expense so Laios couldn't realize what was wrong with his behavior.
The fist fight is soooooo important because it's the first time for the both of them they deal with their emotions toward someone else like responsible adult. And by responsible i mean, they fight like kids, but they also talk and both them and their friendship come out of it stronger and more mature.
i can't stop thinking about transfem shuro while working on my analysis.. . it's like. kind of funny. i'll be writing something like "i think subconsciously part of his admiration for falin is how she represents a part of himself he felt like he couldn't have, where he can approach from a safe distance" and you add all the stuff about how at the whims of conformity and rules he is, and how held back he is in expressing himself at all, and how he's a sensitive person deep down but is something that's covered up under a guise of stoic seriousness, and how he explicitly likes falin for being an Unconventional Girl who is Herself where he feels he couldn't be (and where they also have a lot in common) it's like "Hey. Honestly though, thank you anon for making me remember that that whole confrontation happened when Shuro was literally running on fumes. This man had not taken any breaks except the absolute bare minimum required to survive; he was hungry, sleep deprived, and probably suffering from a major adrenaline crash the moment he realized that he wouldn't be able to save Falin and that he had no goal to keep moving towards.
Like everyone who thinks that Shuro genuinely hates Laios, instead of just being frustrated with him, needs to remember how they feel about their loved ones after skipping a meal or two lmao
