From my fic "A Burning Helm" (also I'm sorry I know you probably haven't read this or even watched the actual play this is based off of but you didn't specify a fic and I really wanted to talk about this fic right now)

Distantly, Ari felt someone tugging on his arm, but Thora was across the hallway. She shook her head.

“It’s not that I want to die, but…” Thora inhaled sharply as the flames grew larger and brighter. “Ari, you need to run.”

Right as Ari was about to ask for clarification, the tugging on his arm turned to a sharp yank and –

Shade was pulling Ari away from Capt– no, Thora’s unwilling attempt to slice through his throat.

Ari let himself be dragged into a run (except hadn't they stopped running?), but he called back to Thora over his shoulder.

So, during the entire scene with Ari in Thora's head, Ari had been subconsciously casting a form of Hold Person on Thora. The others recognized this, and they remembered that when Ari's casting Hold Person and similar spells, hurting the person he has held in place can hurt him as well. Thus, the Stryke Force is trying to run away from Captain Blackwheel while she's paralyzed by the spell, since that might actually give them an advantage over her. Unfortunately, in order to enter Thora's head, Ari basically paralyzed himself as well. The tugging that Ari feels is Shade tugging on him, trying to get him to move away from their enemy.

The flame is meant for three reasons: a symbol of how Thora is under Blackwheel's control, to make my fave be in pain, and also to remind her of her greatest shame of having recruited people into Blackwheel. (Originally, this fic would've been part of a 5-fic series, and I would've treated the reveal of her recruiting people with more weight and drama than canon did, but tbh I liked the lack of drama in canon.) When the flame got larger and brighter, it was to show that Blackwheel was exerting its control by basically forcing a successful wisdom saving throw. The sharp inhalation is both because I imagine this would've been painful (both in the mindscape and IRL) and because she's realized that she's being forced to hunt and kill the Stryke Force again.
