Heart Skips a Beat
Pairing: Peter Parker x Reader
Summary: You work at a coffee shop on campus and develop a crush on the cute astrophysics major who studies there everyday.
Word Count: 2.2k
Warnings: Pure fluff. One curse word.
A/N: There WILL be a sequel to Even If It’s a Lie! Until then, here’s a little something to hold you guys over :-)
“I saw you on a Sunday in a café And all you did was look my way And my heart started to race And my hands started to shake” -Nervous, Shawn Mendes
3 p.m. had quickly become your favorite time of the day. Sure, you had already been working for half of your shift by then, but it was worth it. Because despite how tired you were from a full day of lectures, like clockwork, the cutest guy you had ever seen walked into the café to order a caramel latte and do his homework.
You were a hopeless romantic. A homeschooled bookworm whose only experiences with romance were through the many YA novels you had read growing up. College was honestly your first experience with independence and having a social life outside of your mom, your cat, and your elderly neighbors, Brenda and Pat.
The first day he had come was at the beginning of the semester. You had just started working there to help cut the costs of tuition that your scholarships couldn’t cover. Even though you barely knew how to use the machines at that point, and it took you almost 20 minutes and one phone call to your shift manager to give him his order, he was still really nice to you about it.
By the end of the first week that he started hanging out here, you had memorized his order. For a stranger, there was a lot to like about him. He was super sweet and always dropped his spare change in the tip jar, a rare occurrence amongst struggling college students. He always greeted you by your name, which he only knew because of the tag on your uniform, but you didn’t care. You liked hearing him say it either way.
The more you learned about him, the bigger your crush grew. His name was Peter. Peter Parker, which matched him perfectly. He was an astrophysics major, and he was always busy. So what was he doing hanging out in the café where you worked? Sure, homework, but you had always preferred to go somewhere more quiet, like the library. Not some noisy little coffee shop in the middle of campus where everybody hung out after class.
In between making coffee orders for other customers, you would oftentimes steal glances at him. You thought about how soft his messy brown hair would feel between your fingers, or what it would be like to smell his fabric softener from closer than the width of the countertop that always separated the two of you.
But it was just a silly crush. When you thought about it, the two of you were virtually strangers. He had an entire life outside of the walls of the coffee shop, and he probably didn’t think about you other than when you were taking his order every day. Still, just when you thought about how weird it was to think about somebody you barely knew this much, Peter found ways to make you like him even more than you already did.
Every now and then, Peter wouldn’t show up, and you worried that maybe he had found another place to get coffee or came at another time because he didn’t want to see you anymore. You knew you were overthinking. There was no way that Peter was planning his entire schedule around you. You were just the coffee girl to him and nothing more.
Little did you know that on your days off, Peter was disappointed to find that someone else would be making his coffee that day. Sure, he started coming here to study because it was conveniently located across the street from the laboratory that he interned at, but it was more than that. He came to see you. To see your smile and the way your eyes lit up whenever he walked in. The way you still managed to look cute in the unflattering café uniform and how you didn’t even have to ask what his order was.
He had the biggest crush on you. The five minutes that he spent talking to you each day made him feel all warm and fuzzy inside, and he took comfort in the fact that maybe, just maybe, you felt the same way about him. But he didn't know what to say to you past small talk. He told Ned and MJ that he had been planning to ask you out for a while now, but every time he walked up to the register, he chickened out. You made him too nervous, and he knew that if he tried too hard, he’d start rambling about string theory or something and scare you away.
Peter thought about what kind of person you were outside of these four walls. Were you dating anybody? What was your favorite subject? Did you like pineapple on your pizza? He wondered if you even noticed that he was gone every time his “internship” at Stark Industries dragged him away for days at a time. Probably not. He thought about what it would be like to walk you to wherever you went after your shift ended, to make sure you got there safely, but he always had to rush off to somewhere else before you had finished working.
You had started experimenting with leaving little designs on top of his coffee instead of just haphazardly pouring the milk in. At first, they were just sad little blobs, but then you graduated to vaguely leaf-like blobs, and you could now make something that was shaped like a turnip on top of his lattes. You had been working up the courage to leave little hearts, but every time you did, you swirled them away and served it to him without any design at all.
Your friends would lovingly describe you as a klutz. You were constantly tripping over nothing or knocking things over. There was even that one time you almost broke your leg trying to climb one of the supermarket shelves to reach your favorite brand of maple syrup. But out of all of your clumsy mishaps, none of them lived up to what had just happened.
You had sworn you had looked both ways before crossing the street. But when the barrage of car horns and tires screeching interrupted your favorite song, you saw your life flash before your eyes. That was, until, you were flying through the air. Looking at your savior, you were both starstruck and shocked to see Spiderman holding you tightly as he swung from building to building.
It felt like forever before your feet touched the ground and he let go of your waist, but when he did, you were a little disappointed. How did he even know to drop you off at this specific coffee shop? It was insane, but you felt like you knew him. You just couldn’t place why or how before he saluted you and swung away without a word. “Thank you!” you screamed into the sky, knowing he definitely couldn’t hear you over the hustle and bustle of the city.
Peter’s heart was about to burst out of his chest. That was way too close of a call. What if you found out that it was him? What would you think? But he knew that if he hadn’t saved you, he would never forgive himself. Sure, you might get free tuition like that rumor people always talked about in middle school, but you’d also have a ton of hospital bills to pay because Spiderman was a selfish idiot and let you get hit by a taxi.
Your head was still spinning from what had just happened. You quickly changed into your uniform and started your shift, giddy with excitement to tell Peter who you had met today. As 3 p.m. came and went and there were no signs of Peter, you started to accept that the only person you’d get to tell about today was your mom. Go figure, you thought. You finally had something interesting to say to him and he doesn’t show up.
Eventually, after many disappointing coffee orders later, it was time for you to go home and curl up to a cup of tea and watch the latest episode of the Great British Bake-Off. It was ironic, but your favorite contestant this season was Peter, and every week was more and more tense as you worried whether or not he’d make it to the next episode. Sure, he was really good at technicals, but sometimes the flavors of his signature bakes were off. And when was stupid Laura going to get eliminated?
You were so caught up in worrying about who would be going to the finals that you didn’t even notice the guy walking through the doors just as you were leaving the café, knocking you onto the ground.
“OW!” you squeaked as you hit the tiled floor that your co-worker had just finished mopping.
“Shit, Y/N, I’m so sorry,” a familiar voice apologized from above you. You squinted up to see Peter’s brown eyes full of worry, and at that point, you couldn’t even tell that you were hurt anymore.
He held out his hand for you to take and pulled you up as if you weighed nothing. Still a bit dizzy, you stumbled, and he wrapped his hand around your waist to keep you from falling again. That same familiar feeling you felt earlier that day was rushing back to you.
“Hey, don’t worry. I’ve got you, Y/N,” he assured you, walking you over to a table to sit down.
You laid your head down on the table, both embarrassed and woozy from having your crush knock you flat on your face because you were too busy thinking about a televised baking competition to watch where you were going.
“You know we’re closed, right,” you groaned, lifting your head to see him staring attentively at you.
“Yeah, I know,” he answered, laughing nervously and running his hands through his hair. You wished that you were the one doing that instead.
“Oh. Well, you can if you really want to,” he said, leaving you confused as to what on earth he was talking about. It wasn’t until a few moments later that you were ready to curl up into a ball and hide forever. Did you say that you wanted to touch his hair OUT LOUD? TO HIS FACE?
Your cheeks turned crimson and you buried your face in your arms again. “Peter, I’m okay. You can go now.” If he didn’t leave now, you’d probably die of embarrassment right in front of him.
“Y/N,” he started, running his thumb across your arm. “I don’t want to go. I… I came here to see you.”
Your head shot up and you stared at him in disbelief. “You know I’m not the only one here who knows how to make a caramel latte, right?”
“Mhmm,” he hummed, nodding his head with a soft smile. Even in the dim lighting of the café at night, his eyes sparkled. “I hope this is okay for me to say, but… Y/N. I like you. I like you a lot, and I don’t even like caramel lattes but by the time I realized that I would never get used to the taste, you were already making them without me even having to ask you to.”
“Oh,” you replied, instantly feeling good as new. “I, uh...I like you too.”
You watched as the smile that never failed to brighten your day returned to Peter’s face. “So... Do you—would you, maybe, like to go out with me sometime?”
“Yes!” you replied enthusiastically. Checking your phone, you realized just how long you two had been sitting down. “But we should probably go. The doors were supposed to be locked thirty minutes ago.”
You grabbed his hand, pulling him out the front doors of the coffee shop before scrambling to lock them. When you turned around, you were standing face to face with the guy you’d been crushing on for months.
Right before Peter could lean in to kiss you, you remembered what you had been dying to tell him about all day. “I almost forgot. I met Spiderman today!”
Peter’s eyes widened. He had hoped you wouldn’t bring it up, but then again, he probably shouldn’t have come here in the hopes of seeing you tonight. Play it cool, Parker, he thought as the two of you walked towards the subway station.
“Oh. That’s really cool,” he said in a tone that was suspiciously underwhelmed. “How’d that happen?”
“Well, I was on the way to work and I guess I was listening to my music a little bit too loud,” you started. “And suddenly I hear all these cars honking at me and WHOOSH! I’m in Spiderman’s arms and he drops me off right in front of the coffee shop. I mean, how did he even know that that’s where I was supposed to be? It was incredible!”
Peter really liked listening to you talk. He liked the softness of your voice and how excited you sounded whenever you spoke to him. “Huh. Are you sure you have a crush on me and not on our friendly neighborhood Spiderboy?”
“Spiderman,” you corrected, intertwining your fingers with his. “And no. I like you and only you.”
Peter laughed, thinking about how mad you’d be when he finally let you in on his little secret. But for now, there was no Spiderman. It was just you and him, and that’s how he liked it.

O'Hare started as an airfield serving a Douglas manufacturing plant for C-54 military transports during World War II, and was intended to be the successor to Chicago's Midway International Airport, dubbed the "busiest square mile in the world." E was the inspiration for the moniker. It was popularly named after Edward "Butch" O'Hare, the first Medal of Honor recipient for the United States Navy during the war. Why Business Class?
There are many categories a businessman may choose from when flying to a destination, such as the UAE. But preferring luxury is a good thing, primarily because of the distance that is covered in terms of time. The flight can take anywhere from 17 hours to 25 hours, with one or two stops in the middle for refuelling, changing crew and mechanical checking.
Facilities:Usually, all the business class flights from New York to Abu Dhabi get some really nice benefits. Such as chauffer driven pick-up and drop by the airlines, to th3e airport. Access to the luxury airport lounge, quick check-in and check-out with the assistance of a team of an entourage, luxurious separate cabin area while travelling, inflight entertainment, exclusive beverages of choice from the in-flight menu and variety of cuisine. And above everything else, being connected to the business over the internet while flying across the continents.
Comfort: Indeed, with the above facilities, the comfort of travelling in Business Class Flights to Abu Dhabi is excellent as expected. Also, it gives the true value for the money, and that is unbeatable because not every airline give a complete package to its luxury travellers. Only a few airlines who have A380 or Boeing 777 planes in their hanger can deliver it all. Flying to the other side of the globe without losing sleep is one of the major concerns of many business class travellers. With such cabin space and a reclining chair that could quickly turn into a flat-bed, sleep is part of the comfort delivered to business class travellers.
Tumblr media

Close to the end of the year, and also when the semester ended, I looked forward in selling my books after finals so that I would get money back. I’d get a friend together to take me to Tower Records in Huntington. In a matter of one hour, that money would be gone. Right now I’d be very excited about what I would find in those bins. But due to the pandemic it hasn’t been the case and the euphoria of being in a store, any store at midnight, has been absent. During the mid-00′s, Tower Records went under and currently co-opting the former Huntington location is a third-rate clothing store.
One day as I was driving to work, people stood on the sidewalks of Route 110 holding picket signs like it was the end of the world. First advertised on those signs were 10%-30% percent off all merchandise at Tower. When passers-by and sidewalk shoppers took a closer look, they ended up in shock as they realized that it was an end of an era of some sort. A few days later, the stakes went up. 40%-60% discounts finally garnered Tower’s final audience and rush of shoppers. The final swan song came when all merchandise was down to a whopping 70%-90% percent off. That was when they had their biggest ever turnout because no one wanted to pay full price for music. (Not then when downloading broke, and still not now.) Music collectors and fans were reluctant to buy any new release or any title that was full price. Now they all jumped at the opportunity to clean out Tower’s shelves with a steal. Buyers hovered and tracked entire sections looking for that one shining crown jewel that made their lifetime.
As a music fan for years, I couldn’t begin to tell you how much Tower shaped my taste in music and style. Unlike FYE and Sam Goody, Tower was the one commercial music chain that had a lot of artists and titles that no other commercial store had. Think of what was the Port Jefferson Music Den (closed in 2002) where they were the one underground record store that had everything other stores didn’t even come close to carry. I remember summertimes visiting the Massapequa location picking up rare CD singles and imports as well as other rare hard-to-find and ahead-of-release-date CD’s and artists I picked up such as Autechre’s LP5 (1999) and EC8OR’s World Beaters (1998).
And being a fan of print media and graphic design, I also bought stacks of magazines every week. The magazine selection in Huntington was unrivaled, measuring at least several rows of 60-70 feet of anything and everything you could think of: Industrial Nation, Punk Planet, The Wire, Disruptiv, Mass Appeal, Mean, Vice, YRB, and art design magazines like LoDown, as well as hardcore zines Short Fast Loud!, Maximum Rock And Roll, Under The Volcano and other cult magazines I happened to be lucky to pick up (Lisa Carver’s Rollerderby, anyone?). Every month went at least $60.00-$70.00 total on magazines alone.
Yet towards the end of Tower’s presence I didn’t pick up on music as much because just like other buyers, I had to stand back at the higher-than-usual prices for releases. Compared to discount chains like Best Buy and other record stores, it wasn’t unusual to find a new release with no-frills to be priced at $19.99 or even $21.99. DVDs I noticed were priced at times to be five dollars higher than their competitors. Maybe some shoppers felt the same sentiment as I did.
Not only that, the forces of internet piracy and MP3 downloading of recent years proved to be too dominant and powerful to be stopped, and is currently but begrudgingly co-existing with other existing record stores this day and age. Consumers then re-routed the system right to their own bedrooms with no price to pay for their music. These factors, plus incurring debt that led the chain to bankruptcy proved to be too much for them.
Without Tower Records, it was less convenient for me to pick up whatever artist or movie title I wanted right away right after work. When Tower closed down it took a bite out of some of the physical record collecting I had. Yet, only they could have given me these experiences: no more magazines letting me know what the latest art direction was or who was in the spotlight. No more frantically walking around with a huge stack of everything and more in my hands still looking for one more CD. No more back room full of posters, sheet music, jazz, classical, hip-hop, techno, and soul. No more silly Hollywood memorabilia and comic-book fantasy merchandise. No more video games, no more characters hanging out in the store wearing goth, back-pack indie, or hardcore.
No more attitudes from the employees who treated customers like nothing because they felt like it. That was really the only problem I had with Tower Records, mostly in Huntington. The cashiers pointed people out with a huff and puff because they were given more than enough change or money, or how they rang up a transaction without even making an emotion or saying a word. Some stuck female cashiers had that flat out know-it-all act for no apparent reason. Sometimes I even renounced the shop because of its sometimes poor service, but that was not the case in the end as the attitude did improve, thank you.
And no more of where I had my first date with my Peruvian then-girlfriend. I would never forget wandering in that Huntington store looking for the next purchase when she walked in, greeting each other with open arms. We left when a thunderstorm knocked the power out but we returned after dinner and had a fun time, the first of many for months to come. This was also where I met a pretty pale Irish ginger who I later met again into at community college the following year.
What is now left of the record store scene on Long Island? A few of them which existed when Tower crumbled are still around (Cheapo’s, Looney Tunes, Infinity Records) and even new ones popped up (High Fidelity and Record Reserve to naming two). Some opened and either re-located (Record Stop) or said good-bye by the time you read this (Permanent Records and Vinyl Paradise). The majority of shops in total are still around because internet opinion made their case about the lack of quality, esthetics, art, and ritual of having the physical thing that you don’t get from digital or streaming; and the classic ritual and synesthesia of experiencing a record store and handling the format makes the case of cherishing these shops. On the online circuit, some titles are now being released in very limited quantity, mainly in the low thousands or even in the mere hundreds. It’s a huge drop-off from what the millions in print runs used to be. Naming your own price for digital downloads, streaming, or even buying from the label or artist directly is the way to go nowadays.
With my radio show, Omega WUSB, I do most of my music testing at home. My habits have been ruined by MP3s and streams as acquiring music is of very low cost and extremely ubiquitous. After all that, it hasn’t stopped me from going to record stores or celebrating Record Store Day to find breaks, samples, or dollar music to win it all. Yes, I still very much prefer to go out of town to buy music with my money which I support the artists and shops I like. Even to this day, I’ll be very happy spending $200.00-$250.00 on music just to have the artwork, colors, lyrics, pictures, ink, liner notes, credits, barcodes, and the entire release itself, because later on I will turn a profit in style points as time goes by, just like when people are hoarding and sharing their vinyl and cassette collections now.
That November was my last visit to Tower Records ever, a few months after getting a substantial check from a near-fatal car accident. Acquired would be some titles that made my pre-Omega shows and would be part of some personal seasonal mixtapes. Prices were 40% off on music and that was when I decided to go and treat myself. There comes a time when you just can’t wait and risk a good sale on music before they’re gone forever. The total spent on my last ever visit there was $200.00 on music and the Andy Warhol book at $75.00, originally $125.00 (have you ever carried a book so massive and so heavy?) I took my bags worth of music and magazines with me to the trunk of my car, never to return to the Tower Records experience again.
On another note, I’m still friends with M-Ro to this day, a former manager there. He moved on to become a lead of Hicksville’s Utopia further west before becoming a ticket broker at an independent movie house. He once had a show on WUSB and was a major figurehead and darling of the Long Island punk scene. You might also know his brother J-Ro, a master archivist who still has his own show with us and is known as a major record collector by practically everyone on the island.

At Urgent Care, He Got 5 Stitches and a Big Surprise: A Plastic Surgeon’s Bill for $1,040
It was a Sunday morning in late November when Bryan Keller hopped on a bike for a routine ride to pick up his groceries, cruising with ease in a relatively empty New York City.
The surprises came fast and hard: a fall that sent his head into the pavement and left him bleeding profusely and in shock, a trip to an urgent care clinic for five stitches and then a $1,039.50 bill.
Keller’s health insurance covered much of the cost of his visit to the CityMD clinic on Manhattan’s Lower East Side. But it didn’t cover the physician who arrived to stitch his forehead ― an out-of-network plastic surgeon with a Park Avenue office.
“The people at CityMD just said [this] sort of thing is covered as part of an emergency procedure,” said Keller, a regular cyclist who’s lived in New York City for three decades. Even in post-accident “delirium,” he said, he asked several times whether the stitches would be covered by his health insurance because it struck him as unusual that a plastic surgeon would do them.
“It really irked me that, it’s this classic thing you hear in this country all the time,” Keller said. “When you do all the right things, ask all the right questions and you’re still hit with a large bill because of some weird technicality that there’s absolutely no way for you to understand when you’re in the moment.”
Under a law Congress passed last year, many surprise medical bills will be banned starting in January. Patients with private insurance will be protected against unexpected charges for emergency out-of-network care, for treatment by out-of-network providers at in-network facilities and for transport in an air ambulance. But one gray area: visits to urgent care clinics, which have proliferated in recent years as patients seek speed and convenience over waiting hours at an emergency room or weeks to get a regular doctor’s appointment. There are roughly 10,500 urgent care centers in the U.S., according to the Urgent Care Association, which lobbies on their behalf.
Urgent care clinics were not explicitly addressed in the No Surprises Act, but Keller’s experience underscores patients’ predicament ― insurers often try to steer patients to urgent care and away from costly emergency rooms, but individuals could still get hit with large bills in the process. The Biden administration has expressed an interest in prohibiting surprise bills in those clinics, which may treat serious conditions but not life-threatening injuries and illnesses.
In July, several federal agencies issued interim regulations that largely would not protect patients from surprise urgent care bills. Regulation varies significantly across states, and data is scarce on how common surprise bills are in those facilities. Before the surprise billing rules are finalized, the Department of Health and Human Services and three other federal agencies have asked for information on issues such as the frequency of such bills at urgent care facilities and how health insurers contract with the clinics.
The current regulatory gap, if left untouched before the new law takes effect in January, is one that health care experts say could leave patients at risk.
“There’s a real interesting question about whether it should apply to the extent that people perceive these as places to go for an emergency,” said Jack Hoadley, research professor emeritus for Georgetown University’s McCourt School of Public Policy.
CityMD, which was founded by doctors in 2010 and merged with the large medical practice Summit Medical Group in 2019, operates a massive chain of urgent care clinics in New York and New Jersey. Most of its physicians are emergency doctors. The combined enterprise created Summit Health, which is backed by private equity with investments from well-known firms Warburg Pincus ― which acquired CityMD in 2017 ― and Consonance Capital Partners.
Matt Gove, chief marketing officer of Summit Health, confirmed that the plastic surgeon who treated Keller ― Dr. Michael Wolfeld ― has an agreement with the company that allows him to see patients at certain CityMD clinics. Though he was unable to comment on the specifics of Keller’s situation, he said, CityMD’s “normal procedure” is to “make the patient aware that this is available to them and that they can then make the choice as to whether or not it’s important to them to be seen by a plastic surgeon.”
“This is a patient choice,” Gove said. “We certainly don’t require that a patient be seen by Dr. Wolfeld or any other provider.”
But Keller said it was never put to him as an option. “It was framed to me as This is how we do things,’” he said. “In order to have a preference I would have to know that there is an alternative.” Wolfeld did not respond to a request for comment.

Last month the Biden administration proposed prohibiting surprise bills at urgent care centers licensed to perform emergency procedures, essentially treating them as free-standing emergency rooms. Some states, like Arizona, allow urgent care centers to provide emergency services, but they then are considered free-standing ERs, a spokesperson for the state Department of Health Services said. But urgent care centers aren’t licensed as health care facilities in most states, let alone encouraged to provide emergency services, according to health care advocates that have tracked the issue and have pushed for greater government oversight of the industry.
New York, where Keller lives, doesn’t consistently regulate urgent care providers, requiring licenses for some companies but not for CityMD clinics.
Regardless of what’s prescribed in state regulations, what’s considered an “emergency” versus “urgent” can vary by patient. That potentially creates confusion about whether patients would be protected from certain kinds of out-of-network bills if they show up at an urgent care facility for an acute illness or injury.
KHN also found that the urgent care clinic where Keller was treated describes several of its services as emergency care even though many are not meant to treat emergency conditions as envisioned in federal law. For example, the clinic characterizes physical exams, flu shots and vaccinations as emergency medical services. Under federal law, an emergency medical condition is defined as one where the absence of immediate medical attention could seriously jeopardize a patient’s health.
Summit Health spokesperson Gove said the use of the term “emergency” is meant to be “patient-facing and patient-centric, and not having to do with miscategorizing or misrepresenting the nature of the services we provide.”
The provider is “just making it clear to people that when you have something you need done quickly, which you might call personally an emergency, we’re here to do that.” CityMD has never marketed itself as an emergency room designed to treat all emergency conditions, Gove said.
Lou Ellen Horwitz, CEO of the Urgent Care Association, said urgent care clinics are akin to private doctor practices rather than an emergency room or hospital facility that would be subject to broad bans on surprising billing. She said that, even as urgent care clinics grow more common, there’s “no data” to suggest consumer confusion about what they treat.
The association would oppose any federal push to classify these clinics as something akin to independent emergency departments, Horwitz said. Indeed, she said, such a move “contradicts” their very purpose: to treat non-life-threatening injuries and illnesses.
“The standard practice of the industry as well is that we don’t hold ourselves out to be emergency departments,” she said. “The likelihood of this being misunderstood is very low.”
Nationwide, under the Biden administration’s interim regulations, patients needing care for nonemergencies will not be protected if treated by an out-of-network provider at an in-network urgent care facility, according to health care experts. “You don’t have protections if it turns out the doctor or the physician assistant was out of network,” Hoadley said.
A March report from Community Catalyst, a Boston-based health care advocacy organization focused on consumer issues, and the National Health Law Program, a civil rights advocacy group, found that fewer than 10 states issue facility licenses for urgent care clinics. Those licenses give state officials greater leeway to set standards for care, staffing levels, inspections or price transparency, but could also make care more expensive by increasing providers’ expenses.
Without being licensed as a health care facility ― something that exists for hospitals, ambulatory surgery centers and critical access hospitals ― urgent care clinics are generally treated as private physician practices subject to less regulation. “They’re really flying under the radar now in many cases,” said Lois Uttley, director of the Women’s Health Program at Community Catalyst.
Horwitz, however, said the clinics should not be lumped in with those providers because their operations are fundamentally different.
Unlike hospitals and other practices that include facility fees in their charges to patients, “we don’t charge or receive payments as a facility,” she said.
In the midst of an injury, however, making such distinctions can be difficult. Keller said his motivation in going to urgent care was to get his wounds treated quickly instead of waiting hours in an ER, amid a spike in covid-19 cases that would presage the country’s deadly winter. He had also been to that particular CityMD clinic for a covid test, so he knew it accepted his insurance.
Keller hadn’t been wearing a helmet the day of his accident, caused by trying to prevent a bag of groceries from falling off his bike. With a bleeding forehead and banged-up knees and wrists ― Keller brushed a parked car and went off the bike himself ― he was given a tetanus shot and had elevated blood pressure from the shock of the accident. Still, in that moment, he thought it was odd that a plastic surgeon was being called in to give him a handful of stitches, he said.
“It sounds expensive and it sounds like something optional,” he said. “I said, ‘OK, is this going to be covered?’ And they said, ‘Oh, yeah, they should be covered. He does this, he comes here all the time.’”
In New York, CityMD is not subject to facility licensure requirements because it’s considered a private physician practice, said Jeffrey Hammond, a spokesperson for the New York State Department of Health. As a result, rather than more sweeping regulations that would govern the practices of urgent care clinics, state health officials oversee individual practitioners and investigate complaints related to misconduct.
On its website for the location Keller visited, CityMD advertised many of the services it provides as “emergency medical services.” They include physical exams, vaccinations, pediatric care, lab tests, X-rays, and treatment for sore throats and ear infections.
“Just stop by the CityMD walk-in clinic located on 138 Delancey St. between Norfolk and Suffolk St, where quick, reliable, emergency care service is available 365 days a year,” the website reads.
About six weeks after receiving his stitches, Keller said, he went to the same plastic surgeon to get them removed. His health insurer, Aetna, has denied an appeal to fully cover the cost.
“It’s so clear that getting stitches for a wound, for an open bleeding wound, is an emergency procedure to the normal world,” Keller said.
As for his forehead, eight months later, Keller still has a visible scar.
