Introduction to: Delusional Misidentification Syndrome
What is Delusional Misidentification Syndrome?
Delusional misidentification syndrome (DMS) is an umbrella term for a collection of delusions that involve misidentifying a person, object, place, body part, or the delusional person themselves.
DMSs are associated with neurological trauma, dissociation, and schizophrenic disorders.
Types of DMSs
DMSs typically involve misidentifying one of three things: the self, the other, and/or the place.
The self: These delusions involve the misidentification of the person themselves, or a part of their body.
The other: These delusions involve the misidentification of other people, pets, or objects.
The place: These delusions involve the misidentification of places or locations.
The Self
Mirrored self misidentification is the delusion that the person's reflection is someone else.
Syndrome of subjective doubles is the delusion that the person has a double (doppelgänger / clone) of themselves acting independently.
Cotard('s) delusion / syndrome is the delusion that the person, or parts of their body or organs, are dead, dying, or don't exist. Most people with this delusion have severe depression.
Clinical lycanthropy is the delusion that the person has turned, or is turning, into an animal. It can be considered a type of reverse intermetamorphosis.
The Place
Reduplicative paramnesia is the belief that a place or location (or, rarely, an object, person, or part of the body) as having been copied, existing in two places at the same time, or moved to a different location. Most case studies involve people reporting the hospital they are in is in their home town, when it isn't. "Paramnesia" is commonly called déjà vu.
The Other
Capgras delusion is the delusion that someone close to the person, such as a friend, family member, or a pet, has been replaced with an identical imposter.
Fregoli delusion is the delusion that other people are actually the same person in disguise. Capgras and Fregoli delusions often co-exist.
Intermetamorphosis is the delusion that other people can change their appearance and personality at will, pretending to be the person they are basing themselves off.
Delusional companion syndrome (DCS) is the delusion that objects (often stuffed toys) are sentient, and have their own sense of self, wants and needs.
What can be done for a person with DMS?
Option 1: Nothing. Psychotic people can live happy lives without any intervention!
Option 2: Therapy. This can include either accepting the delusion / going along with it, or trying to change it. It can be used in combination with pharmacotherapy (medication).
Option 3: Pharmacotherapy. Antipsychotics and other medication can help reduce symptoms. It can be used in combination with talk therapy.
No matter what choice the person makes (and it should be up to the person), the desired outcome is reducing suffering (which might not mean trying to stop or slow the psychosis) and increasing their quality of life.

meta: implicit memory
Now that we've talked about how Steve recognised Bucky, let's talk a little about implicit memory for Bucky in recognising Steve.
As I alluded to in my previous TED talk about Bucky's ECT (here - warning, angst) emotional memory is very important.
Just how important is emotional memory? Here's a nerdy fact about a weird condition called "Capgras Syndrome".
Capgras syndrome, originally described by Capgras and Reboul-Lachaux in 1923, consists of a delusional belief that a person has been replaced by imposters, look-alikes, or doubles. (...) some authors proposing a deep right frontal lesion disconnecting it from the temporal and limbic areas with a subsequent disturbance in a sense of familiarity of people and places. (Case report here)
In layman terms, it happens when there is a disconnect between the part of your brain that recognises faces, and the part of your brain that stores memory, including emotions. Because the disconnect between recognition ("hey that's my partner") and emotion ("but I don't feel any love in reaction") is so big, and often coupled with a lack of emotional inhibition ("no I must be overthinking it"), they get into an escalating loop of "that looks my partner but I don't love them" and eventually rationalise this with "that must mean someone is impersonating my partner".

How does this relate to Bucky? Well he's the reverse of this syndrome. This phenomenon is commonly seen in delirious or brain-injured people - they may not exactly recall their loved one, but they instinctively feel a sense of comfort being with them.
As I discussed in the ECT meta, the ECT wipes his memories, favouring explicit memories (recall of events) over implicit memory (like emotions). He presumably receives extra conditioning to reduce any emotional changes during missions. He is unlikely to be ever given the context for a mission, and he's never needed his explicit memories to achieve his goals.

But suddenly, he's hit by a completely alien experience. Steve calling his name triggers emotions that he has not experienced for a long time - warmth, affection, tenderness and intimacy - because during his time in Hydra, most of his emotions are based around dread and humiliation and hostility. He's never had a target evoke such a strong positive emotional response, a very nostalgic response, yet he's unable to connect it to any memory. Abruptly, the machine glitches. The machine wants an explanation.

Pierce tells him that he knew Steve from another mission earlier in the week, and Bucky doubles down. The disconnect between these intense emotions and the absence of memory couldn't be reconciled. There's been case reports of Capgras delusion leading to homocidal acts, so this disconnect isn't something that the brain can dismiss easily. Bucky is struggling to make sense of all these conflicting hints of something being terribly wrong, but they don't give him the chance to work things through.

There's this interesting meta about the resemblance of Chris Evans to a young Robert Redford, and that might be the key to why Bucky was looking to Alexander Pierce for guidance and explanation, because he looked like Steve. Bucky is trying to hold onto any sense of familiarity in a life that skips months or years in between missions, but his trust in Pierce is upheaved by a man he can't recognise but knows he should.
Hydra puts him through ECT again, and in general traumas to the brain affect recent memories more than distant memories. This means on the Helicarrier when Steve appeals to Bucky, he has no recollection of their Causeway fight, hence the complete lack of expression when Steve speaks to him.

At the end of the fight, Steve returns to speak with Bucky. I’m going to resist the urge to analyse the entire scene because it will just dissolve into a gush about Sebastian’s acting. As soon as Steve addresses Bucky with, “You know me,” Bucky’s movements become hesitant. The Winter Soldier doesn’t know how to deal with these surges of emotions he has never experienced before, and Bucky is trying to break through the conditioning but can’t remember why.

The iconic phrase hits Bucky with a load of emotions so powerful and nostalgic that it stops him in his tracks. I think this scene is purposefully left open to interpretation. For me, I don’t think Bucky recalls Steve so much as he emotionally connects with what Steve has been shouting at him, “You’ve known me your whole life. You are my friend.” He still doesn’t know this person (because explicit memory being suppressed) but the emotions match what Steve has been saying. If that was the case, who was Steve, and who was he himself? Why was he told to kill Steve? Why does it hurt so much to see Steve bleed?
And why is it when Steve falls, Bucky feels like he is the one drowning?

Plane crash. Drunk driver. Drive by shooting. There were so many options. But out of all the things you could have lost your husband to, why did it have to be this?
The fear set in when you got the call about your husband Grayson who had been in an accident. The woman on the phone explained that it could have been much worse. She said he had suffered a minor head injury and should be fine. But it did nothing to quell the panic in your chest. You raced to the hospital, calling Ethan on the way who said he’d be there shortly after your arrival.
The tunnel vision took over as you ran through the maze of hallways in the huge hospital, desperate to find your husband. You came to an abrupt stop at a nurses station and asked where his room was. The flustered woman behind the desk gave you the number 415 and you continued running, the numbers on the doors blurring past you.
You skidded to a halt in front of the door and took in a huge breath when you saw him lying there, seemingly okay with just a bandage on the left side of his forehead. You walked into the room, a smile on your face to pretend you weren’t freaking out. But when Grayson turned his head and looked at you, he didn’t look the same. His eyes didn’t light up with recognition. He didn’t smile back at you.
The beeping of his heart monitor started to speed up as you got closer. “Grayson, honey are you alright?” You asked approaching him slowly like he was a frightened deer. He said nothing but his chest was heaving with his nervous breathing. “Baby it’s just me. It’s okay. You’re okay.” You said, your heart sinking. When your hand landed on his arm that’s when all hell broke loose.
“Don’t touch me! I don’t know you! What the hell did you do to my wife!? Where is she?! You’re not who you say you are! GET OUT!” Grayson began to scream and scramble away from you. He kept shouting and the nurses began to flood the room. He was hysterical at this point and when Ethan walked in it got even worse. Grayson began to cry and he was becoming violent to the point where he needed to be restrained. The doctors and nurses ushered you both in the direction of the waiting room but not before Grayson shouted one more thing, “IMPOSTERS!”

“Ethan I don’t know what’s happening! Why doesn’t he recognize us? The nurse on the phone said he would be fine!” You cried into his chest. Ethan was more upset than he wanted to let on but he just muttered a simple “I don’t know.”
A few hours of waiting and a doctor finally came to see you both. “What I’m about to tell you is going to be hard but I need you to bear with me okay?” the doctor said. You both nodded as he continued. “Your husband has suffered a traumatic brain injury. While he may look unscathed by this accident, unfortunately he hit his head in such a way that he now has Capgras Syndrome.” You looked at Ethan and he looked back at you, both of you confused. “Okay what is that? And how do we treat it so he can get better?” You asked.
“People who suffer from Capgras Syndrome have, as a result of their brain damage, lost the connection between the visual face recognition area in the right temporal lobe and the area of the brain that provides the emotional response to that face.” The doctor said and you tried to swallow down the bile rising in your throat. “Thus, when they see their wife’s face or another loved one’s face, they recognize it but they don’t have the warm feeling that goes along with it. They don’t experience that feeling of familiarity. Thus, they form the belief that this can’t be their wife or loved one although they look and act exactly like them, and even know everything that person knew about their relationship. They then believe that person to be an imposter.” The doctor finished speaking and all you could do was stare at him. Ethan beside you was growing more and more upset. “So how the fuck do we fix this? Tell me! Tell me how to fix my brother!” He wailed, tears streaming down his face. “Unfortunately there is no cure as of right now sir. There are clinical trials and studies but nothing that has had the reverse effect yet. I’m sorry. I feel your best course of action would be to admit him to the mental health and disabilities wing of the hospital because many people who suffer from this become violent and destructive.” The doctor said with a sigh.
“So what? What do we do? Leave him with you fucks and forget he ever existed because he doesn’t know me anymore? How am I supposed to forget my husband? How is Ethan supposed to forget his TWIN?” You cried, your hands coming up to cover the horrific sounds ripping through you. Ethan put his arm around you and let you cry into his chest as he cried with his head on your hair.
“I’m not asking you to leave him and forget about him. There’s one loophole to this. He will be able to recognize you over the phone because he can’t see you. His visual sense can’t overthrow everything else if he can’t see you. You can visit and see him through a two way mirror and speak to him on the phone but he can’t see you. I’ll give you some time to think about it.” The doctor said, patting your knee before leaving you with Ethan.

Three weeks later you made your way down the familiar hallway. You had visited Grayson many times in the past few weeks, each time getting a little easier.
You entered the room and took a seat in front of the two way mirror in the wall. You could see Grayson in the room on the other side of the wall playing with his hands. You lifted the phone and swallowed your nervousness as he picked up the receiver on his side.
“Hey baby. It’s me.” You said. “Hi sweetheart. I’ve missed you.” Grayson replied, his hand coming up to lay flat on the glass. “I’ve missed you too. More than you know.” You said with a sniffle. “The doctors have been really nice. They’ve explained what’s happening to me and even though I understand, it’s hard not being able to see you. Both of you. Always tell her I Iove her okay?” Grayson said with a sigh, a tear rolling down his cheek.
Your hand moved to rub over your rounded belly where you were carrying Grayson’s first and only baby. The little girl who would never get to meet her daddy.
“She loves you too. We both do.”
