Carmilla and Nastya probably offered Jonny help. Because there's no way they'd cast a spell on Jonny without trying to help him normally. I know those two understood how much of Jonny's condition was a trauma response. They're better than to not know and be reckless. Jonny had probably repeatedly refused and lashed when offered, and they knew, for the curse to end, Jonny would need to open his heart to help. He had to realize his faults and that he needed to let people in.. and that's what he did. And with Brian, he was probably able to heal. I find that so urghh... yknow... he was finally able to let Carmilla and Nastya help with Brian and all the servants. Growls.
