Why are so many young people getting cancer? Perché tanti giovani si stanno ammalando di cancro? Sintesi dell'articolo con spunti personali.

Per prima cosa sgombriamo il campo dall'associazione tra cancri, turbocancri e vaccini ad RNA che personaggi senza scrupoli stanno diffondendo per (cinicamente o incoscientemente, ognuno scelga) instillare dubbi e paure in chi in questo periodo si ritrova ad affrontare una diagnosi di tumore. Vorrei spiegare il mio accanimento su questo aspetto: una reazione molto frequente in chi riceve una diagnosi di tumore è chiedersi perché. Secondo me offrire ad una persona in un momento di grande fragilità una risposta falsa e che lo spinga a colpevolizzarsi (se avessi dato ascolto, se non mi fossi vaccinato, forse oggi non…) è riprovevole. Et de hoc satis.

E' vero che i dati ci dicono che c'è un'aumento di tumori, ed un aumento di tumori che si presentano in modo insolitamente precoce, ma è un fenomeno che si osserva dagli anni '90, i vaccini ad mRNA non c'erano.

Il punto è che il perché questo succeda ancora non si capisce. E non sarà facile capirlo, almeno per un po'.

La premessa è che il cancro, nella nostra cultura scientifica (che poi trova conferma nella nostra esperienza) è sempre stato una malattia statisticamente legata all'età (qui sotto c'è un grafico con l'andamento dell'incidenza di vari tipo di tumore in relazione all'età).

Un primo motivo è che le cause iniziali del cancro sono le mutazioni. Che siano dovute a fattori endogeni (acquisite in modo casuale nel corso della vita) o esogeni (causate dall'esposizione a fattori ambientali come inquinamento, alimenti, fumo, uso di sostanze chimiche…), solo poche di queste mutazioni avranno un'effetto sulla cellula, solo poche delle cellule che portano mutazioni riusciranno ad andare avanti. Per questo le probabilità che si verifichi un evento critico aumenteranno con il passare del tempo.

Ma una mutazione da sola non basta. Perché si abbia un tumore la cellula deve accumulare più mutazioni che compromettano aspetti diversi dei suoi sistemi di controllo. Questo succede, è nella nostra esperienza quotidiana, ma le probabilità che più mutazioni utili si ritrovino nella stessa cellula sono basse, e allora ci vuole ancora più tempo. Possono essere necessari anche decenni. E ancora altro tempo è necessario perché la cellula vada incontro ad ulteriori cambiamenti e deregolazioni che fanno la differenza tra una cellula fuori controllo che sarà eliminata ed un tumore che crescerà, sarà capace di invadere ed infiltrare altri tessuti, crearsi un sistema circolatorio privato e mandare in giro cellule metastatiche.

Per questo i tumori in genere si manifestano coi loro sintomi e si scoprono dopo una certa età. Hanno bisogno di tempo. Per questo, scoprire un tumore del colon ormai intrattabile in una sedicenne, o scoprirne uno con metastasi al fegato in una 32enne, è qualcosa di assolutamente anomalo. Nell'articolo vengono definiti "improbable forms of cancer", tumori improbabili.

Ovviamente tumori infantili ce ne sono, fin troppi. Tumori giovanili pure, ma sono rari, e particolari (spesso con una componente ereditaria), a causa dei meccanismi alla base in genere diversi da quelli dei tumori dell'età adulta ed avanzata (diciamo dai 50 in su). Che però si stanno presentando in aumento ben sotto i 50 (secondo alcuni modelli tra il 2019 ed il 20230 si osserverà un aumento del 30%). Ed anche se la mortalità per cancro globalmente è in calo, questo aumento potrebbe contribuire a riportare su la curva dei decessi nei prossimi anni. Anche perché nei giovani per ora non si fa prevenzione (controlli e screening) per cercare i tumori tipici dell'età più avanzata, quindi spesso questi tumori vengono scoperti in modo tardivo. Ma c'è chi sta già pensando di correre ai ripari ed anticipare di almeno 10 anni alcuni programmi di screening.

Ovviamente oltre a capire che sta succedendo qualcosa di strano, è importante scoprirne le cause. Ma non è facile.

Un primo problema è quello temporale. Anche in un caso veramente semplice da capire, come i tumori alla tiroide in chi da ragazzo (quindi con una tiroide molto attiva) in Ucraina e Bielorussia è stato esposto allo Iodio 131 liberato dalla centrale di Chernobyl, furono necessari comunque diversi anni prima di avere i primi casi. In pratica, anche in condizioni indotte in modo quasi sperimentale, c'è stato un periodo di latenza rispetto agli eventi iniziali. Diamo per scontato quindi che per capire quello che si osserva oggi non ci interessa qualcosa che è successo uno o due anni fa ma dobbiamo andare indietro nel tempo, di qualche decennio. E non sarà facile associare in modo retroattivo qualcosa che non sappiamo cosa sia, ma è successo 30-40 anni fa, ad un fenomeno che vediamo oggi.

L'altro problema è nei numeri. La tendenza è netta ma i numeri non sono tanto grandi da consentire studi solidi, e se per aumentare la numerosità del campione mettiamo insieme individui e malattie che apparentemente hanno in comune solo una diagnosi precoce, rischiamo di non scoprire i fattori discriminanti. Perché i tumori in aumento sono diversi per tipo, per età di insorgenza, per popolazione, e poi un tumore è per definizione una patologia multifattoriale che può rispondere a fattori molto diversi. La cosa più probabile però è che non ci sia una vera e propria pistola fumante da scoprire, ma una serie di fattori che singolarmente o in sinergia determinano questo risultato.

Di ipotesi se ne fanno tante che però possono spiegare alcuni casi ma non altri. Si va dai cambiamenti nell'alimentazione, sia in quantità che in qualità rispetto alle generazioni precedenti, all'obesità in netto aumento nei giovani, al consumo di alcoolici, a cambiamenti nel microbiota (che ha effetti sul sistema immunitario e sempre più correlazioni col cancro, ma che sarà difficile confrontare con quello di chi si è ammalato in età "normale"). Si stanno considerando anche effetti epigenetici dovuti all'esposizione a sostanze chimiche (disruttori endocrini) durante la gravidanza. Per questa ipotesi ci sono dei precedenti ormai ben caratterizzati, ragazze nate da madri che avevano fatto uso di un ormone sintetico (il dietilstillbestrolo) durante la gravidanza sviluppavano con frequenza insolita un tumore genitale raro ed in adolescenza, età decisamente insolita. Solo in seguito si capì che l'ormone disturbava il programma differenziativo di quei tessuti in una fase critica dell'embriogenesi per cui le bambine nascevano con una sorta di predisposizione a quel tumore. Può darsi che quello che vediamo oggi dipenda da qualcosa del genere. Sarebbe molto utile andare a studiare quelle che si chiamano coorti di nascita, ovvero un grande numero di bambini (e le loro madri) seguiti fin da prima di nascere. Di dati e di campioni biologici da analizzare in cui trovare contaminanti che potrebbero avere questo tipo di effetti però purtroppo non ce ne sono molti.

Stanno emergendo anche altri aspetti significativi a livello di popolazione. Il fenomeno sembra riguardare le donne più degli uomini, negli Stati Uniti gli ispanici ed i neri più che i bianchi, ed ancora di più alcune minoranze etniche native dell'Alaska. Ma sicuramente non si tratta di una questione etnica e di polimorfismi, quanto di aspetti socioeconomici. Livello economico, stili di vita, alimentazione, obesità, sicurezza delle abitazioni, accesso a prevenzione e cure precoci sono strettamente correlati tra loro, e possono essere un fattore importante. Lo abbiamo già visto col Covid. Ma è difficile che siano la causa diretta dell'aumento di incidenza del cancro tra i giovani.

E poi c'è il solito problema (ormai ci torniamo sempre più spesso) delle aree grigie, dei dati che non abbiamo perché riguardano quegli ultimi di cui ci importa poco. Come esempio, in Sudafrica i dati sull'incidenza dei tumori sono noti solo per il 16% della popolazione che è coperta da un'assicurazione sanitaria. Degli altri non si sa. Se tra gli altri ci fosse la risposta alle nostre domande, non lo sapremmo.

In sintesi, oggi oltre a farci le domande dobbiamo anche iniziare a raccogliere tutte le informazione possibili in cui, prima o poi, trovare le risposte.

R: Uma vez teve um passeio escolar pra uma faculdade próxima, pra conhecer e tals, era um único ônibus indo e vindo buscar os alunos na escola pra lá, na hora que o último ônibus chegou minha colega tava dentro, fui falar com ela pela janela, e tinha um degrau no chão, eu não tinha visto, então acabei caindo de cara no ônibus de frente a mais de 150 alunos da escola que estavam no passeio. Nesse dia machuquei feio minha canela, e ficou inchada e dolorida por 3 semanas, até hoje dói um pouco e ainda tenho uma marca.

Debo comentar que me dedico a la escritura desde hace 7 años, y había comenzado mi camino en el mundo del fanfic. Me retiré en el año 2019 para dedicarme por completo a proyectos independientes que ya están en su fase de culminación.
