this is me trying to work through my devastation but it’s 3am, i’m crying and writing this on my phone. :( it’s a blurb btw

you have to admit the timing of the call should be a little bit of a shock.
it doesn’t cross your mind when your girlfriends contact pops up on your phone, but in hindsight it probably should.
it’s not odd for sam to call you multiple times a day when she’s away on camp, if she has a spare minute here or there, so you are delighted when you see her face pop up on your screen.
you press the green button almost immediately, pulling your phone up to rest at your ear.
“Hiya chook, how’s the weather treating you?”
When you don’t get an immediate response you become a little bit worried, but nothing abnormal.
“Y/n? It’s Emma here.”
It’s when the voice of your fiancés coach hits your ears that you are immediately worried.
“Emma, what can I do for you?”
You’ve talked to Emma hundreds of times, but you can tell from her voice that this is different.
“Look, Sam’s just come off the pitch after a little incident, it’s looking like she’s done her ACL.”
Your gut wrenches, and it takes all of your power to stop your lunch from pouring out across the dining table your sat at.
“What?”
Your head immediately begins to reel, 9 months. That’s the Olympics, Emma’s last season at Chelsea, so many things that Sam has over this year that are now done.
Your mind goes back to Sam’s previous ACL injury, how she’d talked about how it had been the hardest thing mentally and physically for her, that it was almost the end of her career.
“It’s not looking good, we’ll have to wait until she’s had scans to confirm the severity but both her and our doctors are fairly certain it’s her cruciate ligament.”
You take a deep breath, fuck.
“Okay, okay. Is she okay?”
It’s a stupid question, of course she isn’t okay, she’s done her fucking acl, but your concern goes further than her obvious injury.
“She’s in a lot of pain, and she’s pretty torn up. Millie has been keeping her company but all she wants to do is talk to you.”
You nod your head, you know you shouldn’t have to prepare yourself to talk to her but you take your time to take a deep breath, keep yourself strong for her.
“Can you put her on for me please, Emma?”
You hear some bustling around.
“Of course, she wanted me to call you to get the facts straight, but i’ll put her on now.”
You hear a little bit more bustling before complete silence, and that seems to be your queue.
“Sammy, honey?”
You hear a deep, laboured breath crackling from the other side of the phone.
“I’m here.”
Her words are strung out, you know that Sam will be trying her hardest to keep it together, she simply isn’t one to be publicly over emotional, no matter the situation.
“Hi honey.”
You know the best thing right now is to leave her to talk about her injury, you leave ghe ball in her court.
“My whole year is over.”
You hear her voice break, and you know that she’s crying even though you can’t hear any sobs or evidence of tears.
“I’m right here chook, so are all the girls, whatever you need.”
When you hear a sob, it takes everything you have to not start crying with her.
“Need to see you, need you here.”
Sam’s not a needy person, so to hear her asking for something like this is concerning to you.
“How about I turn on facetime chook, will that make you feel a bit better?”
When you hear a little murmur of a ‘yes’ you click the button, waiting for it to connect once she accepts the request.
It’s a matter of seconds before you are met with the visual of Sam, her head resting on Millie's shoulder on a physio bed, tears cascading freely down her olive skin.
“Oh Sammy honey.”
She only begins to cry more, and you are fairly certain once this call ends you’ll be rushing straight to the bathroom to expel all of the bile that’s built up in your throat.
“I’m supposed to be captain, I need to be okay, I need to play the olympics, I couldn’t play the fucking world cup. This could be my last major tournament.”
You want to tell her that she’s being ridiculous, but it would achieve nothing, Sam needs to feel validated in her feelings right now, not like you and the whole world are against her.
“Sam this isn’t your fault, you couldn’t have avoided it, it was just a stupid freak accident, unfortunately it happens in the sport you play.”
Sam looks so broken, Millie’s matching her energy, the normally energetic blonde looking very sullen.
“What if this is it for me?”
You resist the urge to roll your eyes, it’s a stupid statement but a very real feeling that Sam has.
“Sam, how about I come to Morocco? I’ll catch the next flight out, and i’ll come and be there for you, is that something you want?”
You don’t want to step over her boundaries, but just the look on her face tells you that she needs to be comforted, she needs to cry and whilst her teammates are great, Sam is never going to be that vulnerable in front of them.
“You don’t have to.”
Her own statement contradicts every single feeling you can see inside her.
“I want to.”
Sam’s tears only begin to fall heavier.
“Please, I need you.”

When caring for a patient in cardiac arrest, it is important to recognize reversible causes for the arrest and address them. This is especially important with PEA and asystole, which often have reversible underlying causes. The mnemonic Hs and Ts can help you to remember the reversible causes of cardiac arrest.
Hypovolemia
Hypoxia
Hydrogen ion excess (acidosis)
Hyper- or hypokalemia
Hypothermia

Tamponade
Tension pneumothorax
Thrombosis (pulmonary embolism)
Thrombosis (myocardial infarction)
 In patients with metabolic acidosis, the administration of an initial dose of sodium bicarbonate (1 mEq/kg) may be indicated. If administering sodium bicarbonate, ensure adequate ventilation for removal of carbon dioxide or sodium bicarbonate may worsen intracellular and tissue acidosis.
Suspect hyperkalemia in all patients with acute or chronic renal failure and in those who had a wide-complex ventricular rhythm and tall, peaked T waves on ECG before arrest. Several measures may be taken to reduce potassium levels, including administering sodium bicarbonate, glucose and insulin, or nebulized albuterol. Sodium bicarbonate is preferred in patients with cardiac arrest caused by hyperkalemia because it causes a rapid shift in serum potassium level. Other therapies take much longer to work.
Suspect hypokalemia in patients with dehydration or overuse of diuretics. In hypokalemia, flat T waves, prominent U waves and possibly prolonged QT intervals may be seen on ECG before arrest. Treatment is with intravenous (IV) administration of a dilute solution of potassium chloride.

My ACLS instructor reduced me to tears after class today. I was wound hella tight on caffeine and could literally feel my body vibrating (like it still is right now 😬 ) and stumbled through my scenario, leaning heavily on my team. I thought I was going to have to retest.
Instead he tells me that I’m good at what I do, and I need to stop selling myself short as being “just” a shrink and barely being allowed to practice BLS, let alone act as a paramedic in my jurisdiction. That I’m not confident in myself and my knowledge, but I should be, since I’ve demonstrated that I have the knowledge and skills. That I have unique training that not a ton of people in my shoes have and that I can make a difference in a big way.
With a few more comments, I figured out that he sits on the state review panel that cleared me of everything. He recognized my name from the start of class. And he thinks my jurisdictional medical leadership is full of hooey. 
Basically he kept building me up to the point that I started crying. But then typical Marine, starts getting flustered and “Doc, sorry if I upset you. That’s not what I wanted.” So I explained that these were the relief tears, the knowing that someone thinks I’m good at what I’ve spent half of my life doing, even if certain parts of my leadership don’t. Someone who’s part of the bigger organization can see that. And then offered pep talks and introductions because there’s so much more beyond the world of that one little jurisdiction and I have so much to offer.
Not what I was expecting to come out of class today. Definitely not what I was expecting when he asked to see me after the test. My therapist will feel vindicated, since he’s been saying some of this all along

Blocking IPv6 everywhere
If you have some PCs on your network with IPv6 enabled, your PCs are probably looking for a DHCP server to get an IPv6 IP. 
And if a bad guy were on your network running a packet capture, they might see some details of the DHCP process, like the mac address of the requesting PC.
Then they can become a IPv6 DHCP server and start talking to the clients and receiving traffic and do man-in-the-middle type attacks. 
OK. so, that’s where we are now. Need to stop IPv6 on the network. And here’s how:
step1: on your switch, create an ACL, let’s call it deny-ipv6 step2: inside the ACL the action can be deny any any. (I think the first any is source, second any is destination). Point is that its blocking everything.
step3: go to the interface that you want to block IPv6 on and do the following:         ipv6 access-group deny-ipv6 in         ipv6 access-group deny-ipv6 out
this says to apply your ACL to IPv6 traffic on this interface for inbound traffic and outbound traffic.
Now we’ve blocked IPv6 for one port. Just do the same thing on every switch everywhere in your network, and we’ve blocked IPv6 everywhere.

Bradycardia:
First-line therapy is with atropine. Second-line therapies include transcutaneous pacing and β-adrenergic agonists. Consider implementing one of the second-line therapies immediately if the patient has second-degree AV block type II or third-degree AV block. Consider expert consult and transvenous pacing if first- and second-line therapies are not effective.
First-line therapy is with atropine, an anticholinergic drug that increases sinoatrial (SA) node firing to increase the heart rate. Give a 1-mg bolus intravenously every 3 to 5 minutes, up to a maximum dose of 3 mg. If the atropine is having no effect, do not wait to reach the maximum dose of atropine before initiating second-line therapies.
Use atropine with caution in patients with acute coronary ischemia or myocardial infarction because in these patients, atropine can cause adverse effects, including ventricular tachycardia or ventricular fibrillation.
Consider implementing transcutaneous pacing immediately if vascular access is difficult to achieve. If at any point atropine is not effective, consider transcutaneous pacing or β-adrenergic agonist therapy. Also consider transcutaneous pacing or β-adrenergic agonist therapy immediately if the patient has second-degree AV block type II or third-degree AV block.
Epinephrine or dopamine may be administered to patients with symptomatic bradycardia as an alternative second-line therapy. These drugs may also be considered when the cause of the bradyarrhythmia is an overdose with a β-blocker or calcium channel blocker. The initial dose is 2 to 10 mcg/min by IV infusion for epinephrine or 5 to 20 mcg/kg/min by IV infusion for dopamine, and then the dose is titrated to the patient’s response.
Atropine is the first-line therapy for patients who are unstable (experiencing hemodynamic compromise) and experiencing symptomatic bradycardia. Adenosine is the drug of choice for treating a patient who is unstable and experiencing a tachyarrhythmia.
