Cas heals Dean anyway, but only when he is asleep, because Dean was kind of under a curse too when he beat him, so he heals him he doesn't care what Dean said earlier, he just can't let him suffer pointlessly like that. He doesn’t deserve this. Dean discovers it but can't really do anything about it.
Notes:
This story happens after the episode 11x03, Dean refuses to be healed by Cas, after Cas beat him when he was under Rowena’s dog spell, because he feels guilty about almost killing Cas when he had the Mark of Cain.
It includes well between 11x03 and 11x04, because Dean still have some bruises in episode 11x04 "Baby" and refused multiple times that Cas healed him. Because Cas actually kind of already did. At least in my mind.

Question
So right after this PDA from our boys, Sam , who has a gun to Rowena's head, like looks down/away from Dean and Cas, and that's when Rowena makes her move and gets away.
Was Sam actually doing something or was he just trying to avoid the awkwardness of another romantic moment between his brother and Cas? Because that's what it looks like. And if that's the case, Rowena escaping as a result is one of the funniest things to ever happen on that show.

Dean's face transforms into the picture of grief. (Usually he shows panic, fear, or dissociation when it comes to Cas.)

So, what is the shape of grief?
In simple terms, it's the wrinkled brow and rectangular mouth. Above, we see it in the way his mouth falls open, pulling back at the jaw. Like this:

so glad I pushed thru s7-8 where I always got stuck
watching The Bad Seed (11x3) and it has everything. Demon and angel drinking together complaining about their bosses. Dean in a tourist costume for no reason. Rowena dressed up like Moira Rose in her black dress and big sunglasses and platinum blonde wig. Dean’s fetish porn taking over Cas’s attempted internet search. Uncle Crowley calling the most dangerous force in the world “Cupcake” and giving her a picture book. Cursed Cas unleashed on the world like a wild animal. Rowena setting up Dean and Sam for an uncomfortable car ride revealing secrets.
