The Weather
Similar to this time last year, COVID wastewater levels in many states have decreased and now remain at low levels except for the states of Tennessee and Missouri, which are at high levels. Arkansas, Alabama, Delaware, Minnesota, and Virginia have moderate viral levels detected by wastewater surveillance. This reminds us that it is important to continue the practice of precautions, especially among those most vulnerable to a COVID infection.

When considering trends in wastewater levels across all four regions, they have stopped decreasing as seen in the provisional data (gray shaded area). The national wastewater levels are indicated as “Low.” While lower wastewater levels indicate decreased spread, the risk for infection remains moderate especially as current wastewater levels remain slightly higher than previous low periods.

Wins
During the past few weeks, we have taken several actions against removing vital public health measures, especially the change in COVID isolation guidelines. This included the People’s CDC press conference from March 13, a recording and our press release are available. We also have a pre-proof of the People’s CDC External Review in the American Journal of Preventive Medicine Focus that additionally highlights the shortcomings of the CDC’s approach to public health and recommends a more equitable pandemic response. This week, we have assembled an expert letter asking the CDC to correct their COVID isolation guidelines.
Community groups continue to show us that it is possible to push back against corporate efforts to further privatize healthcare – and win. In Massachusetts, the Boston Center for Independent Living and SEIU 1199 led a successful drive to prevent cuts to publicly funded personal care attendants (PCAs) for disabled people. When we fight, we win.  
Variants
Currently, JN.1 remains the dominant variant in the US, and is 83.7% of circulating variants as of 4/13/2024, down from 88.5% on 2/3/2024. JN.1.13 has increased to 9.1%, up from 0.7% on 2/3/2024. Other variants comprise 7.2% of the remaining total. A recent study, published in The Lancet Infectious Diseases, provided additional information on recent COVID variants that shed higher levels of wastewater; however, this cannot discount that COVID transmission remains at higher risk during rises in wastewater levels.

Hospitalizations
Total new hospital admissions caused by COVID have decreased to 7,318 during the week of April 6, 2024. Although the number of new hospital admissions are lower than the past, many counties in the US continue to experience increases in new hospital admissions. Over 25% of all counties are experiencing an increase in new COVID hospital admissions between the last week of March 2024 and the first week of April 2024. Most concerning, we still do not know the total number of hospital-acquired infections, since reporting over these numbers halted in May 2023. Despite this decrease in new hospital admissions and wastewater levels are currently at low levels nationwide, total hospitalizations do not completely reflect the current amount of circulating virus.

Prevention and Precautions
The CDC recently released guidelines to improve ventilation and implementation of air purification to prevent the spread of infectious diseases, specifically airborne pathogens. A group of scientists have also jointly published a policy statement in the academic journal Science supporting the establishment of higher standards for ventilation and the importance of air purification in indoor settings. Two studies published in the last month have validated the significance and value of both ventilation and air purification in schools and childcare centers to prevent the spread of COVID.
Vaccine uptake remains limited. Only 22.8% of adults and 14.1% of children have received the updated COVID vaccine as of 4/11/2024 -  a slight increase from 21.1% of adults and 12.8% of children on 3/10/2024. The Bridge Program remains available for those underinsured or without insurance for no-cost access to these vaccines, but may end December 31, 2024.
Long COVID
Senator Bernie Sanders, as chair of the Senate Health, Education, Labor, and Pension Committee, proposed draft legislation to address Long COVID. This proposal aims to allocate $1 billion annually for a decade to the NIH for Long COVID research, establish a centralized research entity and advisory board, create a rapid grant process for clinical trials, develop a patient data database, and enhance public education on Long COVID. We ask that you share your thoughts to their official contact by email specifically on allocating funding for effective treatments and specific measures in the prevention of Long COVID by April 23, 2024. Currently, Long COVID Alliance has compiled a list of active opportunities in studies and clinical trials that people with Long COVID may participate in, which may help the clinical community contribute knowledge of Long COVID and potentially support the development of effective treatments.
Take Action
An invaluable home-based program that supports testing, evaluation, and treatment for COVID, Test to Treat program, is ending on April 16, 2024. Send a letter to your local representatives to ask them to help save the program that helps so many at-risk people!
It’s been over a month since the CDC released new, irresponsible guidelines on COVID isolation which are not substantiated by scientific evidence. Our fight to take public health out of the hands of corporate interests and protect our collective well-being continues. As part of our strategy to push back, we’ve put together an expert letter to CDC Director Mandy Cohen telling her to reinstate science-based COVID isolation guidelines. We urge the CDC to consider the highly variable length of infectiousness in their recommendations and to adopt a test-based approach for ending isolation. This letter is for public health professionals, scientists, healthcare workers, disability advocates, and others who consider themselves experts in public health. Sign this letter asking the CDC to correct their updated COVID isolation guidelines.
Avian Flu (Awareness Update)
The People’s CDC is monitoring Avian Influenza (AKA “Bird Flu”) as it has been spreading in many avian and mammalian species around the world at alarming rates. The current strain of concern is subtype H5N1 clade 2.3.4.4b and is highly pathogenic in poultry, causing systemic infections and rapid onset of illness and death among avian species (1). As such, this is called a highly pathogenic avian influenza (HPAI). This specific clade emerged in 2020 and has now been detected on every continent (2,3,4, 5). Over 500 species of birds and 360 species of other animals have been infected worldwide. Hundreds of thousands of wild animals have died from avian influenza since its emergence in 2020 (1, 6). Here in the U.S., at least 28 outbreaks have occurred in cattle farms (6, 7).
While human cases remain low, case fatality in humans has historically been around 50% (6). The outbreak among many non-bird animals is alarming and allows for opportunities for viral mutations that may lead to further infections among humans (1,6). The ecological consequences of such mass death around the world are currently unknown but guaranteed to be devastating as food webs are severely disrupted and conservation efforts suffer (1, 5). Humans are already feeling the direct impact of this as farmers are forced to cull entire flocks of poultry to control the spread, which may result in increased prices in the food supply (6). Humans will probably continue to feel the impacts of this historic spread, regardless of the number of human cases. We will continue to monitor the situation and share updates as the situation develops.

India was able to save more than 3.4 million lives by undertaking a nationwide Covid-19 vaccination campaign at an unprecedented scale, according to a Stanford University report.
The campaign also yielded a positive economic impact by preventing a loss of $18.3 billion, the working paper by Stanford University and Institute for Competitiveness titled 'Healing the Economy: Estimating the Economic Impact on India's Vaccination and Related Issues' released by Union health minister Mansukh Mandaviya on Friday suggested.
According to the Stanford report, the direct and total impact of vaccination varied from about $ 1.03 billion to $ 2.58 billion if minimum wages are considered within the age distribution category.
“The same, however, varied from about $3.49 billion to $ 8.7 billion if GDP per person employed (constant) is considered. The cumulative lifetime earnings of the lives saved through vaccination (in the working age group) rolled up to $ 21.5 billion. Moreover, since vaccination also saved the lives of the elderly, this indirectly helped prevent the health infrastructure from getting overwhelmed and thereby allowing for a more judicious use of the existing health infrastructure,” the report suggested.
Mandaviya said much before Covid-19 was declared a public health emergency by the World Health Organisation (WHO) in January 2020, processes and structures to focus dedicatedly on various facets of the pandemic management were put in place...
The Stanford working paper refers to a Lancet modelling study which estimated that in India around 3.4 million deaths were prevented by vaccination in the year 2021, an estimate based on officially reported deaths in India.
The paper also highlighted the impact of the lockdown and referred to the health ministry’s statistical analysis that the Covid-19 tally could have reached about two lakh (0.2 million) without lockdown by April 11, 2020.
Due to lockdown measures, the actual cases only went up to about 7,500 by April 11, 2020, making a case for the lockdown stronger."

The culprits are the usual suspects: this year’s strains of influenza, COVID, and RSV. And though flu-like illness levels have been above baseline nationally for several consecutive weeks, the CDC warns that we still haven’t hit the peak. [ ... ] The CDC also sent out an alert to healthcare providers this week highlighting low vaccination rates for the three viruses.
I never stopped wearing a mask on crowded public transit. An increasing number of commuters are resuming the practice.
Beyond vaccination, for everyone, the best way to prevent the spread of respiratory illnesses like the flu and RSV is regular handwashing; avoiding touching your eyes, nose, and mouth; and staying home if you get sick. High filtration face masks still work great, too, and not just for avoiding COVID.
Most of the time if a place offers COVID vaccines it also has flu vaccines. But just to be safe...

An Australian politician has revealed that “more than 30 federal departments and agencies” are paying a transgender and queer lobby group for “advice and staff training on so-called ‘inclusive’ language.” Senator Claire Chandler, a Senator of Tasmania for the Liberal Party, said in an Op-Ed published on her website today that the federal Department of Health had last year replaced a pregnancy COVID-19 vaccination guide’s fifty references to ‘women’ with the phrase ‘pregnant person’. Noting that “removal of specific sex-based language undermines the accuracy of health advice for pregnant women,” and the “overwhelming majority of Australians don’t find the re-labelling of mothers as ‘birthing parents’ or ‘pregnant people’ inclusive” but instead “unnecessary, sexist virtue-signalling,” Senator Chandler was motivated to contact the Department of Health to uncover what was driving its “ideological capture.”

In January, Senator Chandler queried the Department of Health about its “mind-boggling waste of resources during a pandemic.”
This month, she learned:
“The Department’s answer was that it receives ‘a benchmark and suggestions’ on ‘the use of inclusive language’ from the lobby group Pride in Diversity. Services Australia was presented with a “Gold Employer” award by the same lobby group this year, apparently around the same time it was starting to re-label mothers as ‘birthing parents’.”
Pride in Diversity is a program by the New South Wales-based ACON, which is described on its website as a “health promotion organisation” campaigning for “sexuality and gender diverse health.” The tax-exempt non-profit offers nationwide “services to assist employers, sporting organisations and service providers” with “inclusion” through its three programs, which aside from Pride in Diversity are Pride in Sport and Pride in Health + Wellbeing. ACON ranks employers by its “benchmarking instrument for LGBTQ workplace inclusion.” Employers who best conform to ACON’s standards are given incentives in the forms of high rankings on the Australian Workplace Equality Index (AWEI), which is published by ACON, as well as awards. Senator Claire further revealed: “All up, more than thirty federal Departments and Agencies have been paying this lobby group for advice and staff training on so-called ‘inclusive’ language.” This “influence of lobby groups over the public service” has been evident, as “the Office for Women declares that anyone who identifies as a woman is a woman, … Sport Australia encourages males who identify as women to play women’s sport,” and “Australian Bureau of Statistics claimed that humans can change sex over the course of their lifetime (a claim it was forced to amend after I asked them at Senate Estimates if they’d checked it with biologists).” Further, the scheme has had “detrimental” effects on women’s human rights in Australia and internationally, with law and policy “increasingly allowing males to self-identify into women’s spaces and facilities, even housing male rapists in women’s prisons,” she said.
The Senator commended Sal Grover, founder and CEO of the app Giggle, for advocacy that resulted in the word ‘mother’ being restored on Medicare forms after it was temporarily replaced “with the de-sexed term ‘birthing parent’.” However, she stressed that fixing one form will not stop what is rapidly becoming systematic, with Australian taxpayer money intended for public services being used to fund an ideology that “drives … appalling disregard for women.”

People across at least 10 states have now been infected by BA.2.86, a highly mutated variant of the virus that causes COVID-19 that authorities have been closely tracking.
According to data tallied from the global virus database GISAID, labs have reported finding BA.2.86 in samples from Colorado, Maryland, Michigan, New York, Ohio, Oregon, Pennsylvania, Texas, Virginia and Washington.
Estimates suggest BA.2.86 still remains a small fraction of new COVID-19 cases nationwide.
"The diversity is less than what appears. Many of these lineages actually have identical spike sequences. We've observed this before, where we see convergent evolution and viruses evolving to have the same substitutions," said Natalie Thornburg, a laboratory branch chief in the CDC's Coronaviruses and Other Respiratory Viruses Division.
Thornburg, who was speaking at a meeting of the agency's vaccine advisers last week, said it was still too early to know "if BA.2.86 will be of any significant circulation."
Health authorities do believe BA.2.86 is continuing to spread widely around the world, after scientists first voiced concern in August over the strain's large number of mutations.
"We are concluding this because some of the people infected with BA.2.86 do not have known links to other infected individuals and did not recently travel to an area with known cases of illness from BA.2.86," the CDC said Friday in a risk assessment.
Too few sequences of the virus have been reported to show up on the Centers for Disease Control and Prevention's biweekly variant estimates, which still show that a long list of closely related XBB variant descendants are driving virtually all infections around the country.
Research on BA.2.86 so far has so far largely relied on pseudoviruses, which are other viruses mocked up in a lab to mimic BA.2.86's distinctive mutations. Better findings will need to use viruses grown from actual samples of infected patients, a process which is now underway.
"CDC has generated two authentic isolates of BA.2.86. One confirmed and one putative. We are in the process of distributing BA.2.86 viruses to multiple labs to do transmission studies, more neutralization studies, against lots of different kinds of sera," said Thornburg.
For now, officials have expressed "guarded hope" about signs the current late summer wave of COVID-19 driven by other variants has passed its peak.
Meanwhile, scientists have already tracked BA.2.86 beginning to evolve into at least two different branches. Cases from both sublineages have been spotted in the U.S. so far, among the handful of cases reported globally so far.
"That's likely the tip of the iceberg, given that we know we don't have complete sequencing coverage," Kirking said.
