Jews help Syria’s innocent victims in Bulgaria

Former pop musician Yank Barry is teaming up with celeb boxers, aiming to knock out hunger and homelessness among Syrian refugees, while Jewish group Shalom offers medical care.

SOFIA, Bulgaria — In escaping the horrors of the Syrian civil war, Hadirdrami Yantar hoped he would find solace in Bulgaria. Lured by dreams of a better life in a Western country, the 39-year-old Syrian and his family of four paid smugglers to cross the Turkish border and arrived in the town of Harmanli, only to be met by a harsh reality.

Living as squatters in the makeshift trailer park that serves as a camp for new arrivals, Yantar’s family were forced to brave low winter temperatures with only outdoor fires to keep them warm.

SOFIA, Bulgaria — In escaping the horrors of the Syrian civil war, Hadirdrami Yantar hoped he would find solace in Bulgaria. Lured by dreams of a better life in a Western country, the 39-year-old Syrian and his family of four paid smugglers to cross the Turkish border and arrived in the town of Harmanli, only to be met by a harsh reality.

Living as squatters in the makeshift trailer park that serves as a camp for new arrivals, Yantar’s family were forced to brave low winter temperatures with only outdoor fires to keep them warm.

Unable to handle the growing number of refugees, the Bulgarian authorities are helpless to offer anything more than overcrowded camps in poor condition with little medical care or food.

“We count on external assistance,” admitted Bulgarian Prime Minister Plamen Oresharski during a meeting with Jewish Canadian philanthropist, pop musician/jingle writer Yank Barry on November 13. Driven by personal ties to Bulgaria and his humanitarian mission, Barry is among the most recent contributors to join the relief effort to aid the Syrian refugees in Bulgaria, the European Union’s least wealthy member.

Insuffisance cardiaque: l'adrénomédulline pour prédire le risque de décès et guider le traitement par diurétiques

ATHÈNES, 27 mai 2019 (APMnews) - La mesure de l'activité de l'adrénomédulline dans l'insuffisance cardiaque aiguë permettrait de prédire le risque de décès à 1 an et de personnaliser le traitement post-hospitalisation, concernant en particulier les diurétiques, selon une étude européenne présentée samedi au congrès Heart Failure de l'European Society of Cardiology (ESC) à Athènes.

L'identification des patients à haut risque et la définition de l'intensité du traitement post-hospitalisation constituent un défi dans la prise en charge de l'insuffisance cardiaque aiguë. Des études pilotes ont suggéré que l'adrénomédulline quantifiait la dysfonction endothéliale et le risque de mortalité associé, et qu'elle reflétait la congestion résiduelle.

Nikola Kozhuharov de l'hôpital universitaire de Bâle (Suisse) et ses collègues ont étudié ce biomarqueur chez 1.886 patients atteints d'insuffisance cardiaque aiguë, en Suisse, au Royaume-Uni et en France. Les résultats ont été présentés en session late-breaking trial.

Les concentrations plasmatiques de MR-pro-adrénomédulline (MR-ProADM), précurseur stable, et de la forme biologiquement active de l'adrénomédulline (bio-ADM) ont été mesurées dans les 12 heures suivant l'arrivée du patient et à la sortie de l'hospitalisation aiguë. Les chercheurs ont examiné les interactions de ces dosages avec les différents traitements administrés à la sortie de l'hôpital (diurétiques, IEC, ARA II, bêta-bloquants, anti-aldostérone) sur la mortalité.

Les patients ont été suivis 1 an. Il y a eu 514 décès (27%).

Les courbes de survie à 1 an montrent une augmentation significative de la mortalité chez les patients ne prenant pas de diurétiques et dont le taux de Bio-ADM était supérieur ou égal à la médiane, par rapport à ceux qui prenaient des diurétiques, selon le diaporama de la présentation. Pour les patients dont le taux de bio-ADM était inférieur à la médiane, la mortalité était similaire avec et sans diurétiques.

Les patients ayant des taux de bio-ADM supérieurs à la médiane avaient un risque de décès augmenté de 87% par rapport aux patients en dessous de la médiane, précise un communiqué de l'ESC. La MR-proADM était même encore plus fiable que la bio-ADM pour prédire la mortalité ainsi que le risque combiné de décès et de réhospitalisation pour insuffisance cardiaque.

"L'observation que des patients ayant une bio-ADM élevée ont une mortalité bien plus élevée s'ils ne sont pas traités par des diurétiques à la sortie de l'hôpital a des conséquences cliniques immédiates", commente le Dr Kozhuharov, cité dans le communiqué. "Les raisons d'arrêter les diurétiques au cours de l'hospitalisation comprennent la détérioration de la fonction rénale et l'hypotension. Notre étude montre que les patients devraient être ré-évalués pour les contre-indications avant la sortie afin que les diurétiques puissent être ré-introduits si c'est approprié, en particulier s'ils ont une bio-ADM élevée."

Il souligne toutefois dans son diaporama qu'une interaction significative ne prouve pas la causalité, et qu'un essai randomisé contrôlé est nécessaire afin de prouver que le recours aux diurétiques en cas d'activité de l'adrénomédulline augmentée améliore le pronostic.
