New Zealand schools have introduced a climate change resource that suggests children “eat less meat and dairy”, even though teachers will not know how much meat or dairy any child in their care has eaten. Opinion pieces in the papers have called for the reduction of meat and dairy in hospital menus, not usually generous sources of such foods, despite the well-known risks of undernutrition, especially of protein, in the frail and elderly. Globally, the influential and once-objective medical journal the Lancet has hosted Eat Lancet, a coalition of vegan and vegetarian technocrats backed by processed food manufacturers, and promoted their agenda. The Guardian newspaper accepted an £626783 grant from the backers of Impossible Foods to run a series of articles against animal agriculture.

These initiatives, aimed at remodelling our food supply in a way that favours the multinational food processing and seed-and-chemical corporations, whose control of many aspects of farming and diet is already problematic, have run far ahead of the scientific community’s efforts to understand the health effects of such dietary change.

Our hunter-gatherer past

The Neolithic Revolution was the first alteration in human affairs that is generally considered worthy of the term Revolution. In Marx’s terms, it saw a change in the means of production sufficient to form new classes aware of their identities, and thus a change in the relations between people. Early humans had fed themselves in an opportunistic, hunter-gatherer fashion that tended to favour a diet of animals supplemented with plants where and when these were available. Large animals made the best meals but gathering activities could collect many smaller ones, as well as eggs, grubs etc.

The people of the Mesolithic era discovered that some animals could be herded and some plants grown in gardens (not usually by the same community, because one activity favours nomadism and the other favours a sedentary habit) but these activities, which greatly improved food security after the decline of the prehistoric mega-fauna due to hunting and climate pressures, tended to occur at the communal level and probably did not create major class differences between the people involved.

The invention of farming

The Neolithic Revolution, which unleashed the human potential for war, creativity, and social division, resulted from the identification of the germs of plants (specifically grains and legumes) as durable sources of energy. If grains were grown (I will use grains in the wider sense of “cereals”, after Braudel, including other dried germs such as peas) and there was a surplus, this surplus would still be edible over the next year, a year when drought or pests or diseases might wipe out the other food sources that hunter-gatherers depended on. This advantage was offset by the nutritional poverty of grain-based diets, so that tuberculosis probably became an endemic disease during this period,[1] but the existence of a less-perishable surplus allowed the diversion of part of the population away from food gathering for large parts of the year, and saw the creation of armies and other workforces.

In Europe, the Neolithic Revolution is dated at around 10,000 BC and its arrangements are a matter of prehistory, but in China this change occurred later and the written record around Bi-gu or grain avoidance includes folk-memories of conflict between grain eating and grain avoiding peoples.

The history of colonisation is the history of the conquest of lactose-intolerant peoples by lactose-tolerant populations, and of non-grain eaters by grain-eaters. In the Indian sub-continent, a combination of dairy herding and a cereal diet high in legumes uniquely allowed the survival of a substantially vegetarian population, and saw the conservation of genes favouring reproduction on such a diet, including genetic polymorphisms still rare in European populations (adaptive mutations only predominate where many individuals without them have failed to survive or reproduce).[2] That the Indian social system became more aggressively class-based than any other is probably no co-incidence; prejudice against meat-eating is still used as a tool of social control against minorities, while meat-eating is one way young Indians today identify as modern and egalitarian. However there were some important exceptions to the trend – the Aztecs were a hunter-gather people who conquered and dominated the Mesolithic agriculturalists of Mexico, and the Mongols were nomadic herders and hunters whose greater stamina and independence allowed them to defeat the rice-fed armies of the Chinese Emperors (after conquering this breadbasket, the successive Mongol Khans seem to have eaten and drunk themselves to death).

Early vegetarian ideology

In the European and Asian cereal-based societies the poorest classes went without meat, supplementing cereals when possible with buttermilk or blood pudding which were more economic replacements. The rich ate as much meat as they could. The idea that an entire society might avoid meat is a recent one with its roots in religious practice, and, insofar as it has any political basis, this flows in two distinct streams – the eco-fascist, in which meat avoidance is a sign of “purity”, most humans are a burden on the Earth, and the Indian vegetarians are of course Aryans. This is something like the vegetarian vision that Adolf Hitler picked up while studying anti-Semitism with Wagner’s heirs at Bayreuth.

And then there is a Marxist-Anarchist, and latterly Intersectional, version, founded on a valuation of animal rights as inseparable from, and a logical extension of, human rights. Vegetarianism was a frequent obsession of the early British Socialists; G.B. Shaw, who derived most of his energy from dairy fat and lived to the age of 94, made himself into a well-known example, and the idea was sufficiently entrenched among the British Socialists and their milieu that H.G. Wells preserved its internal contradictions for posterity in The Time Machine. In his far-future vision, humanity has evolved into two separate species. The Morlocks are descendants of working-class meat-eaters, the Eloi of leisure-class vegetarians – all Wells’ loathing is reserved for the Morlocks, yet it is obvious they are (still) the engineering brains keeping their world running and the Eloi fed. The Eloi are useless for anything but enjoying the sunshine and feeding the Morlocks, and the discordance in Wells’ progressive values as he describes both species is as shocking as anything else in the story.

The first large-scale experiment in plant-based protein was attempted by the Bolsheviks. As usual, it’s hard to separate the roles played by idealism and cynicism in the story, but the bare bones are that the Soviets found their initial attempts to remodel the countryside rebuffed, blamed this on the recaltricance of the kulak class, and set out to destroy them. The problem being that the kulaks, owning most of the cattle and sheep across the Russian Republics, helped to feed the people. Beginning in the 1920s, soy experts from the USA (then the Western world’s leading soy producer) were among the many foreign technicians imported into Russia, and soy processing plants were built and soy production increased to 283,000 tonnes in 1931, the year Stalin unleashed enforced collectivisation and the terror against the kulaks (and also the Kazakhs, a herding people who suffered the largest proportionate loss of life during this period). This led to the loss of millions of animals, either killed by their dispossessed owners or mismanaged by their inexperienced new owners. The soy project was hardly able to prevent the massive famines that followed, and by 1935 soy production had dropped to 54,000 tonnes. Though soy milk would later prove useful during the siege of Leningrad, by the 1930s soy had probably only served one purpose, as a statistic needed aforehand to quell the objections of pragmatic delegates to the destruction of the kulaks and their livestock.

Today we face the revival of this idea, of plant protein that will create a world with no need for animal protein, and the remodelling of life in the countryside, with the new impetus of climate change as its driver. Livestock cycles natural carbon, meaning there is no net addition of C02 to the atmosphere – and its contribution to the shorter-lived methane precursor has not changed since 2000 (methane rises have been due to fracking, methane itself AKA “natural gas”, landfill, and rice production; methane-emitting animals have always existed on Earth in substantial numbers, and have not created a novel situation in the sense that the discovery of coal, oil and gas did). We have recently seen how much global disruption is required to reduce fossil fuel CO2 emissions to 2006 levels, levels which will still warm the planet if they continue. It could be still be worth reducing agricultural cycling of CO2 through methane, which is more warming than CO2 if this is cost-free, but is it?

Why humans evolved as meat eaters

Animal foods, and especially red meat, supply a constellation of nutrients not found together (if they are found at all) in any plant food. Nutrients are those chemicals essential for the functioning of the human organism, and plants, but not livestock, can survive well without nutrients such as amino acids, fatty acids, vitamins and minerals that are essential to humans. Surviving as a vegetarian or vegan is possible for some (perhaps assuming the genetic variants referred to earlier are present) but to thrive requires knowledge of these nutrients, where to find them, how to process the foods that supply them, or how to supplement them. Thriving as an omnivore or even a complete carnivore does not – nutritional sufficiency is the reason we evolved eating meat and other animal foods long before we learned there were such things as essential nutrients.

The reasons for avoiding meat or all animal foods can have a class basis – veganism may be taken up by educated middle-class adults, more likely to be exposed to “health food” ideas and aware of the need to supplement, some of whom then commercialise their habits as social media “influencers”. Meat avoidance is also being adopted increasingly by educated middle-class children for identity or compassionate reasons, but the poor may also avoid meat because of its cost when a loaf of bread or a packet of flavoured noodles can be bought for a dollar; these two motivations sometimes coincide when students in temporary poverty make a virtue of what they perceive to be a necessity.

Does the meat-avoiding behaviour of young people have unintended costs? Several observational studies have looked at the characteristics of meat-avoiding populations and found alarming increases in depression, anxiety and self-harm.

“The majority of studies, and especially the higher quality studies, showed that those who avoided meat consumption had significantly higher rates or risk of depression, anxiety, and/or self-harm behaviors. There was mixed evidence for temporal relations, but study designs and a lack of rigor precluded inferences of causal relations. Our study does not support meat avoidance as a strategy to benefit psychological health.”[3]

How can we explain these correlations? Why should we assume that they are causal?There are several lines of evidence to support a causal link: 1) several nutrients found in meat and animal foods are important factors in mood and cognition; vitamin B12, iron, carnitine, DHA, choline and tryptophan are some examples.[4] 2) the fatty acid mix in dairy and red meat has a similar composition to that of amniotic fluid and breast milk which has anxiolytic (anti-anxiety) effects in young animals.[5] 3) soy is a convenient and cheap replacement for animal protein; soy processing in Western diets results in a 10-fold higher level of the estrogenic contaminant isoflavone than that found in Asian diets.[6] Soy isoflavone causes anxiety behaviour in young female animals, and there is evidence supporting psychotropic and hormonal effects in humans.[7,8,9.10] Interestingly, while right-wing critiques of soy eating focus on effects it can have on young men, the scientific evidence for adverse effects in younger females, converting to HRT-like benefits after menopause, is stronger.[11] 4) other toxins found in plants, such as salicylates and oxalates, as well as problematic proteins such as gliadin/gluten and zein, may be present at higher levels in meat-free diets (but are not unique to them). A vegan mince sold in Countdown supermarkets is simply a coloured blend of soy protein and gluten, a protein linked to the risk of schizophrenia.[12] In the New Zealand context it would be relatively easy to confirm or dispute some of these associations. Everyone admitted to hospital for longer than a day supplies their dietary preferences. The dietetic preference data from psychiatric admissions could be both linked to outcomes over time and compared with the population average distribution, or the distribution in a ward where diet is least likely to play a role in admissions.

Iron deficiency in women

Young women in New Zealand are the most likely to report being vegan or vegetarian in surveys, as elsewhere in the world. Vegans in the Gender Studies field generate papers linking meat to masculinity, with the implication that this masculinity is toxic and might be improved by a plant-based diet.[13] The corollary of this belief – that women may therefore be weakened by meat-avoidance – is never considered. In a 1980 essay by Gloria Steinem called The Politics of Food (in the collection Outrageous Acts and Everyday Rebellions) she describes some of the cultural constructs by which women are deprived of the good nutrition which men use to stay dominant. The belief that men need to eat red meat more often than women may have been valid when the average man was more likely to have to survive an attack by a wild bear than the average woman, but today it is mainly women who suffer from serious iron deficiency. The rate – and the cost to the health system – is increasing in New Zealand as more women give up meat. Iron deficiency anaemia in early pregnancy is associated with neurodevelopmental disorders in children, not an outcome that will increase the mother’s autonomy.[14]

In Georg Büchner’s 19th century “working class tragedy” Woyzeck, filmed by Werner Herzog with Klaus Kinski in the leading role and the subject of an opera by Alban Berg, the title character, a soldier, is subject to experimentation by a sadistic army doctor. The experiment involves Woyzeck living on nothing but peas. Peas may supply a complete protein, but Woyzeck goes insane; the deprivation being the final straw in his alienation. James Cameron, the film-maker responsible for Avatar and Titanic, is investing heavily in pea protein as if this were his gift to New Zealand. I am not sure whether he has watched Woyzeck – one would think he has.

Plant-based vs meat-based

Again, we have the specificity of plant germs as commodity; their low cost of production, long storage life and versatility of processing outcomes makes them an ideal investment and a robust one, as poverty and adversity increases their consumption, as we saw during the 2020 Lockdown Event. However, a plant-based burger is nutritionally greatly inferior to a meat burger, and that burger is often the most nutritious single food item many will people eat in the course of their day. The current push to eat a plant-based diet for “planetary health” is something that all the multinational food processors have signed up and provided funding for, and why not – Coca Cola, Unilever, Nestlé have always sold us plant-based foods. We notice that while iron-deficiency anaemia increases in New Zealand with the reason in plain sight, Nestlé scientists here in NZ are developing a more potent form of supplemental iron to add value to their products as their parent company backs the push to reduce meat. (As usual, it’s hard to separate the roles played by idealism and cynicism in the story). But, you may well ask, isn’t eating meat linked to an increased risk of cancer and heart disease? These associations are small to begin with, but they are also intensely confounded by social class and educational status. Supposing a factory that makes a carcinogenic chemical is hiring. Who is more likely to apply for that job – a meat eater (who will likely have a bigger family to support, among other considerations) or a vegan? Who, so to speak, eats all the pies, and needs food that is filling and nutritious without having to give it much thought? Who is more likely to work two jobs and be exposed to the disruptive metabolic effect of shift work? Carcinogen exposure and shift work are just two of the confounding variables ignored in diet epidemiology. (That meat-eating in Western populations may symbolise or associate with labour itself – as it did for H.G. Wells when he wrote The Time Machine – is not a consideration I have found discussed in the epidemiological literature.)

Certainly one can think of mechanisms that might link meat to disease, as one can with any food, but one can also think of protective mechanisms; several of the nutrients found mainly or only in animal foods are required for various antioxidant and immune defensive enzymes, and some like carnitine and EPA even have a place in the management of heart disease. The argument against meat-eating should not be confused with the argument for sometimes rationing a valuable food that is in short supply. The wartime rationing of meat in the UK is thought to have improved the health of the poorest by guaranteeing a greater supply than they had had previously, at a more affordable price. In Europe, the peasants who supplied the cities with meat, dairy and luxury foods such as oysters were sometimes forced by network disruptions to consume these foods – which many of them had never tasted before – with benefit to their own health.

The plant-based agenda can scarcely be expected to recognise these benefits, or understand the argument summarised by Williams and Dunbar (with regard to the vitamin nicotinamide and amino acid tryptophan in their tuberculosis paper), that if better data collection and analysis resulted in us ”…returning to our egalitarian past and redistributing meat or its components that supply NAD (avoiding both the highs and the lows between individuals and over individual lifetimes) [this] may be more effective than subsidizing corn grain (while the increased prosperity from unlocking human potential should pay for the intervention).”[1] Progress – which includes unlocking human potential from the chains of preventable mental and physical disease – depends on good data, and we do not yet seem to collate the data required to know whether or for whom plant-based diets are safe in New Zealand.

‘Supernatural’ season 15, episode 15 screener secrets: We’re ‘Highway to Heaven’-ing this bitch

This week on Supernatural, Amara returns and are angels solving people crimes now? Hypable previewed Supernatural season 15, episode 15 “Gimme Shelter,” so read on to find out more.

After a sweet and fun return to ease us back into the world of Supernatural last week, things are heating up pretty dramatically – I knew there wouldn’t be much more time for messing around.

“Gimme Shelter” sees Supernatural dip its feet into what the Winchesters currently believe is their big plan – eliminating Chuck by also taking down Amara, resulting in what they believe will be a cosmic-being-free balanced world. But first, they have to find her. Sam and Dean get a pretty good lead on her location, which results in a very interesting conversation between Amara and the boys – especially with her most favoritest Dean, of course.

Meanwhile, Castiel is persuaded into taking Jack to investigate a nearby case in Missouri – which all three adults suspect is probably the work of a human criminal – for the sake of humoring Jack and keeping him both busy and supervised. On the way home, they have a very interesting conversation of their own.

Spoiler Warning: This article contains generalized spoilers for Supernatural season 15 episode 15, “Gimme Shelter.” If you do not wish to be spoiled at all, do not read this article in advance of the airdate.

‘Supernatural’ season 15, [10] episode 15 screener secrets

1️⃣ During the filming of this episode (27 January – 5 February) Jared Padalecki and Jensen Ackles both spent much of the week at home with their families in Austin, a detail which was made clear on their and their wives’ public social media accounts – possibly the result of scheduled time off as they’ve mentioned occasionally requesting? The result is that the episode is weighted much more towards Cas, Jack and the murder investigation they’re chasing than towards Sam and Dean, but on the flip side, the Sam and Dean arc is more crucial to the long game of the show, so what it lacks in minutes, it makes up for in impact.

2️⃣ However, the episode still begins and ends in a grounded family group way, at home in the Bunker – one of those “we know we should be doing this together but there are Reasons we have to split up” situations. This detail, in my opinion, really speaks to the motivation of the creative team towards honoring the four leads as parts of a whole – in earlier days, this kind of episode would have been two entirely non-touching threads. This one is, if not a tapestry, at very least a braid – tied up together at both ends, and intertwined in the middle.

3️⃣ You might have seen pictures or ominous trailer footage of Castiel and Jack digging a hole at the crossroads. We all know what that means! However, don’t worry. They simply want to talk – and the demon they summon has some really interesting – and dare I say positive? – news about the state of Hell under Her Most Gracious Majesty Queen Rowena. Let’s just say the demon is actually pretty friendly… and extremely bored.

4️⃣ The two main guest stars on Cas and Jack’s side of the episode are both actors who have been briefly featured on the show before, in a couple of pretty famous episodes – one from season 2 and one from season 5. I don’t think there’s meant to be any meta or Easter Egg element to this, just the usual Vancouver casting industry cycle (see the ‘Weren’t You In Another Episode‘ reference page on the SuperWiki) but one of them is one of those cute “I appeared on Supernatural as a child and now I’m here as an adult” situations, and the other, well… the original character’s very name has become the stuff of Supernatural legend, and if I were in charge of this episode I would have put the actor in a particular piece of footwear and made sure we got a shot of it, just for kicks.

5️⃣ Castiel steps into a prayer circle when the church group members are meant to give a testimony – presumably of their journey so far and their relationship with faith. That’s what Cas chooses to share, at least – in a non-specific, humanized way – and fans of the character will be moved to hear the ways he verbalizes his own growth.

6️⃣ Speaking of growth, some of Dean’s is spelled out for him in the most miraculous way by Amara. After Sam and Dean meet up with her and have a conversation about Chuck that’s ultimately a bit of a non-starter, Dean returns to ask her another, more personal question. Her response gave me legitimate chills. It’s a very weighty mic drop and the combination of the level of impact and the level of clarity (it’s entirely airtight, no room for interpretation) feels like the culmination of all the self-actualization work the show has been doing on Dean in the last four years. (I wish I could tell you Sam got a big special moment like this in the episode, but he doesn’t. Amara’s return was always going to be Dean’s thing.) Amara’s speech to Dean… it doesn’t feel isolated, like the idea of it was invented just for this episode. It feels more like concrete evidence of what the show has been trying to prove for ages. And the funny thing is, Amara is the anti-Chuck, right, and all season, we’ve learned about the version of the story Chuck thinks is good, and we’ve been told to root against that. Chuck’s version of Supernatural isn’t how the writers really feel. But I think Amara’s might be. Dean has obviously struggled to see what she tells him, all in one piece, but here it is – this was the point, laid out on the table, from the entity behind the curtain – both onscreen and off. Amara knew what she was doing, and so did the writers. This was always, always the point.

7️⃣ Even before this massive scene, Amara’s return is just great. Emily Swallow does such an incredible job with this character – she really is the anti-Chuck even without the whole writer comparison. Swallow imbues this character with such an incredible peace and stillness in comparison to Chuck’s histrionics – this was true in the way she spoke and behaved even in season 11, but this Amara also feels kindness and patience and tolerance. She radiates power, even when she’s also slightly goofy. There’s no fight, there’s no antagonism, but the boys in her presence are like little fish in a vast ocean – they quickly realise they have no real control in this conversation. The way that we leave her indicates she’ll be back and has more to say or do, and what she shared during her reunion with Sam and Dean makes me really curious about the role she’s due to play in the show’s endgame.

8️⃣ I’m not very religious but I really like the version of a church group or ‘faith-based community,’ as they say, featured in “Gimme Shelter.” Supernatural has a shaky history in terms of how the show portrays people in-universe who believe. Sometimes they’re treated like a joke, or stupid, or dangerous, or hypocrites, but occasionally civilian acts of faith are shown as great and powerful things, even in a world where we know that what they believe in isn’t strictly accurate. That concept became an even bigger question mark for me when we got the reveal that Judeo-Christian God is not only absent, but our actual villain. However, this was a really nice look at why faith can still be a framework for a good way of life – loving thy neighbor – for some people, no matter the truth about Chuck. The episode also features a callback to writer Davy Perez’ very first Supernatural episode “American Nightmare” in terms of the way that some people have weaponized faith and religion to the detriment and harm of others or even themselves, but this factor does not negate the positive point mentioned above.

9️⃣ Supernatural alum and newly minted director Matt Cohen really got the full old-school Supernatural episode experience when it comes to leaning into the spooky horror element. The murderous case-of-the week featured in this episode is heavy and lingering on the gore and even contains a little bit of a jump scare, so view responsibly.

🔟 So, um, you know that line, in this week’s teaser trailer? The line that a lot of people are freaking out about because it seems to pertain to something important that we know about Cas’ fate that Dean and Sam aren’t aware of? Yeah, it is 100% absolutely not about that at all. It is about something super important, but it’s not that. It’s also the last line of the episode, but trust me – it’s not a cliffhanger and it’s not a red herring and it’s not a twist. The information is gleaned within the episode and you’ll know exactly what Cas is telling Dean about after seeing it – narratively, that’s the reason it isn’t in the episode, because the show clearly assumes you’ll get the picture and can skip a rehash of information. But what you were probably expecting – maybe even hoping for – it’s not that. You’re gonna have to hang on for that one.

We want a baby!

My husband, Cam, and I have been trying to make a baby for over one year now. I had a history of ovarian cysts so I was worried about fertility even before we started trying. I overthink things a lot and get stuck in my head, but I also love going to the doctor, so I figured why not go get checked out. My mom got pregnant with my sister and I very quickly. My sister got pregnant very quickly with all three of her kids. Other than the occasional cyst, there was no reason for me to worry. In Fall of 2017 I went to my family doctor and asked if there were any preliminary tests I should do to see if I’ll have a high risk pregnancy or have trouble getting pregnant. We did a pap smear and progesterone test and that was it. I was told my progesterone was low but aside from that everything was totally normal and we should have no issue getting pregnant. Cam and I were engaged at the time, but knew we wanted to start trying for kids pretty early on in our marriage. We started in February 2018 with the mindset of “if it happens, it happens.” We were both pretty uninformed at that point. I wasn’t really tracking ovulation or any other daily symptoms, we weren’t scheduling sex, it was literally just not using any kind of birth control method. We got eloped in April, and my next period was due at the beginning of May. I missed it! I took a pregnancy test every day from the first day of my missed period for a week….all negative. I called my OB and scheduled a blood test, there’s no way my cycle is this long, it’s been almost 40 days, I’m for sure pregnant. I was so excited. I started planning how I was going to tell Cam, my family, his family, how I was going to hide it for the next 8 weeks, everything! The blood test came back...also negative. My OB told me I was not pregnant and that since my cycles usually lasted 32-37 days this was “normal.” Turns out I just didn’t ovulate that month, my cycle lasted 58 days. That crushed me. That was not normal. While I was waiting for my next period I decided we should buckle down more. I did my research and we started trying every other day during ovulation. This new schedule threw Cam off a bit, he was under the impression that I was fertile every single day. I blame California sex ed, I had the actual sex talk with one of his friends a few months later. When Cam and I were both on the same page we both agreed we would officially start trying. I scheduled another appointment in July after I’d had another full cycle. I wanted to know what to do next. Am I just not going to ovulate regularly anymore? Are my eggs bad now? I’m only 27, why is this happening? My OB gave me a list of tests and medicines we could do before referring me to a fertility specialist. Insurance doesn’t usually cover anything with fertility and if we did everything it would cost in the thousands.I had a thyroid test, hormone tests, an ultrasound, and HCG test where they insert a dye into the uterus and see if it there are any blockages (that one hurt effing bad) all between July and January. I started Clomid in November, a hormonal pill to induce egg production. We did two rounds, with no luck. By this point I was frustrated and crying every time I got my period and it was taking a toll on our relationship. Every period felt like a loss. Between being sad a lot of the time, losing drive to help out around the house, and not talking about how I was feeling with Cam, we struggled. We had some long conversations, and what I was reading as him not caring, was him trying to be strong for both of us. We have gone to couples counseling previously so we were able to use tools from that to help us through this. I asked to be referred to a fertility clinic. My OB prescribed me one more round of clomid and sent us on our way to our local fertility clinic. Even though I had specifically asked for one more round of clomid, I didn’t end up taking it. After such a long period of time of scheduling intercourse...it was taking a negative toll on our relationship. We took a one month break from trying before seeing the specialist. Before we could go to our first appointment, Cam had to go get tested. He was bizarrely excited to go and bragged to his friends that he was going to the “spank bank.” LOL His results came back great and we were ready to get started on our fertility treatment plan. Our doctor is great, we were pretty positive about our chances of conceiving and laid out a plan for the next few months. He also explained why exactly we were having trouble...which I had never really received a clear answer from my OB so I didn’t know it was even figured out! I have PCOS which is commonly misdiagnosed as Endometriosis. I have acne, struggle with weight gain, have irregular periods, and I don’t always release fully developed eggs or an egg at all (hence 58 day cycle). My cysts were being caused by underdeveloped eggs essentially burying themselves in my ovaries. For the next 3 months I would take Femara (another fertility drug). The first month we would try on our own, the next two would be with intrauterine insemination, or IUI. The doctor seemed very confident that we would be able to conceive on the first try. We left feeling confident and with lots of fun paperwork outlining our instructions. -Take Femara as prescribed on days 3-7 of your cycle -Have intercourse on day 10 or 11 -Abstain until you see a surge on an ovulation test -Take ovulation tests days 12-15 -Have intercourse every 36 hrs once you surge - If you do not surge, call the office Romantic, right? We got to day 13 and I was getting nervous. What if we missed it? What if my surge doesn’t read? Did I do this round of Femara for nothing? Cam called the fertility clinic, they told us to go ahead and start trying every 36 hours, and continue testing. Day 16 came and I still hadn’t had a surge, I called in and they told me to come to the office that morning for an ultrasound. The tech said I had “a very good looking egg ready to release any day now.” Perfect! I went in for another progesterone test the following week and it was higher than it ever had been before! This is the month, it had to be, everything was looking good! 11 days later my period started. I was crushed again, but I didn’t cry much this time. I don’t know how to explain exactly how I felt. All I know is this meant we were done trying on our own. It was the end of the road for conceiving even a little bit naturally. We were one step closer to IVF, or surgery, or adoption. Don’t get me wrong, we’re open to all of those, and want to adopt in the future regardless. But not yet, I want this.

The 27-year-old physician recruiter in Roanoke, Virginia, was perfectly happy with the NuvaRing, a flexible vaginal ring that women insert monthly to release hormones to prevent pregnancy. But her insurer, Anthem, stopped covering the branded product and switched her to a generic version in early 2020. Force said the new product left her with headaches and feeling irritable and short-tempered.

After talking to her OB-GYN, Force tried an IUD. But that made her feel worse: She had bad cramps, gained 10 pounds and developed severe hormonal acne. Plus, she was charged $248 for an ultrasound her provider used to guide the insertion of the device, a charge she successfully fought.

Force also considered a couple of birth control products approved in recent years: a non-hormonal vaginal gel called Phexxi and a vaginal ring called Annovera that can be used for a year. But Phexxi isn’t covered by her employer health plan, and she would owe a $45 copayment for Annovera.

Despite the ACA’s guarantees of free contraception coverage, Force’s experience illustrates that even for women whose health plans are subject to the law’s requirements, obtaining the right product at no cost can be onerous. New types of contraceptives aren’t automatically incorporated into the federal list of required methods that insurers use to guide coverage decisions. In addition, some health plans continue to discourage use of even long-established methods like IUDs by requiring providers to get approval from the plan before prescribing them.

Consumer advocates who have studied the issue say a process is spelled out in federal rules for women to get the contraceptive they need, but far too few people know that is an option.

Ultimately, Force went back to the generic version of the NuvaRing, despite the side effects she continues to experience. She’d prefer to be on the branded NuvaRing, which didn’t give her problems, and the ping-ponging from method to method has left her exhausted and furious.

“I cannot believe what hoops I have had to jump through between September 2020 and June 2021,” Force said, “between switching from the generic NuvaRing to the IUD and then back, fighting my insurance and OB-GYN’s office on the ultrasound charge.”

In a statement, Anthem said, “Anthem health plans cover 222 contraceptive products at $0 cost share on our ACA Preventive List. We cover at least one product” in each of 18 categories of contraception methods approved by the FDA.

Contraception is a very personal choice, and what meets one woman’s needs may not meet another’s. If avoiding pregnancy is a woman’s top priority, a virtually fail-safe method like an IUD may be the right solution. But for someone who’s considering getting pregnant soon, a readily reversible method like a birth control pill might be the best option. Side effects are important to consider as well, since women respond differently to the hormones in various birth control products.

Before the ACA required no-cost birth control coverage, researchers estimate, up to 44% of women’s out-of-pocket health care spending went toward contraceptives. The ACA required most commercial health plans to cover a comprehensive list of FDA-approved methods without charging women anything. Church plans and religious nonprofits as well as employers and schools that object to contraception are exempt from the coverage requirements. Plans that were grandfathered under the law are also exempt. Uninsured women don’t benefit from the mandate either.

But the federal rules do not require health plans to cover every contraceptive. After the ACA passed in 2010, the federal Health Resources & Services Administration developed guidelines for women’s preventive services. Those guidelines say women should have access without cost sharing to a list that covers the 18 FDA-approved methods, including oral contraceptives, vaginal rings and cervical caps, IUDs, implantable rods and sterilization. Under federal rules, health plans must cover at least one product in each category.

But neither the HRSA guidelines nor a birth control chart published by the FDA addresses newer methods, including the gel Phexxi, which regulates vaginal acidity to reduce the odds a sperm reaches an egg. It was approved by the FDA last year.

Nor do they incorporate fertility-awareness mobile apps the FDA approved in recent years such as Natural Cycles, which tracks a woman’s temperature and menstrual cycle to avoid pregnancy.

“There’s a real need for new guidance that keeps up with new methods,” said Mara Gandal-Powers, director of birth control access at the National Women’s Law Center.

Many insurers have balked at covering Phexxi, said Rameshwari Gupta, director of strategic markets for Evofem Biosciences, which markets Phexxi. A box of 12 single-use applicators — consumers use one before having sex — costs $267.50 without insurance, she said.

“When I started talking to payers, they all said, ‘Where are you on this FDA chart?’” she said.

According to an FDA spokesperson, the birth control chart is for consumer education purposes only and “was not created with the intent of driving coverage decisions.” The agency is in the process of updating it.

In a statement, HRSA said it is reviewing the evidence on contraceptives and expects to complete its review late this year. If it opts to make revisions, it will publish draft recommendations to update the women’s preventive services guidelines. These will be finalized after a public comment period and become effective a year later.

At this point, health plans consider Phexxi a spermicide and are required to cover only one type of spermicide without cost sharing, said Kristine Grow, a senior vice president at AHIP, a health insurance trade organization.

“If Phexxi is indeed considered a new ‘method’ of contraception, both the FDA and HRSA would need to make this clear,” she added.

The vaginal ring Annovera, approved by the FDA in 2018, is typically covered by health plans, according to Grow, though it may not be available without cost sharing. The average retail price is $2,457 a year, according to GoodRx.

One way plans have made it difficult for women to access certain contraceptives, even those on the list of approved methods, is by requiring that their providers get approval from the insurer first, often by providing documentation that the product is medically necessary.

Under UnitedHealthcare’s coverage policy for Phexxi, for example, before coverage will be authorized, members must have documented reasons that they are unable to use eight other contraceptive methods, including oral contraceptives, the contraceptive patch, a vaginal ring, injections and spermicides. Providers also must attest that they have counseled patients that Phexxi is less effective at preventing pregnancy than some other methods.

In a statement, UHC said it covers “a broad array” of generic and brand-name options, and it follows scientific evidence to develop its list.

Yet consumers have a way to get the specific drug that is most appropriate for them, according to a report by the National Women’s Law Center.

Under federal rules, if a doctor or other health care provider determines that a patient needs a particular contraceptive, even if it’s not on the list of approved products for the patient’s plan, the insurer is required to have an expedient process for the patient to seek a waiver.

“It’s not up to the insurance company whether to cover that method; it’s up to the provider,” said Adam Sonfield, executive editor for policy analysis at the Guttmacher Institute, a research and advocacy organization focused on women’s reproductive health.

But according to the National Women’s Law Center report, many insurers, patients and their providers aren’t aware of the requirement, and state agencies don’t enforce these so-called exceptions policies.

If patients run into trouble getting the method they want, “we typically recommend filing an appeal with their insurance provider,” said Gretchen Borchelt, vice president for reproductive rights and health at the National Women’s Law Center.

Stephanie Force said she was unaware of any process she could have used to get the NuvaRing covered without cost sharing. Neither her health care provider nor the insurer mentioned the possibility.

She recently had an appointment with a new provider, who she hopes will be a better advocate for her.
