Ohhh what's your politics and intrigue themes from one dm who loves them to another. I just dropped some fucking big intrigue shit tonight and I'm so down to hear about someone else's.
Personally I'm going hard on "corruption is lined with good intentions and it's the most vulnerable who get caught in the crossfire." as the city of Waterdeep has made a deal with the Xanathar guild as a better option, ignoring the way it's being leveraged against the unofficial wards and poor of the city. Also the government is being infiltrated by intelligent devourers from a mind flayer working with them so THEY'RE HAVING FUN WITH THAT. I love the mess. Complex scheming and shit is great please tell me about yours.
OOOOOO, first of all, I LOVE what you have, mind flayers and intelligence devourers are so fun and having them infiltrate the government is great. Glad to hear from another intrigue enjoyer!!!
Ok, to first bit of a TL;DR, almost every part of the game has some sort of corruption associated with it. Some of that is just flavor, but most of it is stuff I've pulled on in some way. Now, to get into it and show why a TL;DR is necessary, I'm going to start with the history of where the PCs are currently, the Kingdom of Menlin XD
So, the Kingdom of Menlin is the longest-running monarchy in the world and currently rules the entirety of one of the two major continents in the world. It goes back thousands of years, all the way back to when the first monarch, a mortal child of the god Torm, established a righteous kingdom. Now, because it does go back so far, there have been MANY scandals and dethrone attempts. Enough that some think that the current monarch, King Vittorio, isn't even related to the first king, although it doesn't really doesn't impact much.
However, although Menlin is the longest continuously existing kingdom, it doesn't mean it always ruled the entirety of the continent. While it's done so previously, this newest rule only traces back about 500 years ago, where older elves can remember living under a different kingdom before being taken over, either peacefully or with force. Safe to say that there are some people, including some of those in power that negotiated to stay that way when their kingdom was taken into Menlin, that don't want to stay under Menlin rule forever. Nothing has progressed so far, but with other things I'm gonna talk about, there may start being talks of rebellion.
Getting into the more present day, there's casual corruption and intrigue everywhere. Some of it is obvious and well known, some of it isn't. For example, in the northern part of Menlin, the most well known place for corruption is the port city of Tevaris, a cutthroat place where guards are known to be able to be paid off for just about anything, and as long as you don't get caught or at least have the money to get away with it, you will. This corruption kinda radiates outwards, so most people who don't want to deal with it don't go near it. However, it's kinda left alone and the nobles in charge of both the city itself and the province in which it's in profit from it (the kingdom's broken down into provinces that have ruling nobles assigned by the king that then assign other nobles to rule different cities on their behalf).
The party has actually dealt with some noble political schemes before, during the 3rd arc, Wedding Woes, where they found out an innocent woman had been secretly killed so a politically charged wedding would happen. It was confirmed at least that the footman of the city's ruling noble was in on it, but it's still not confirmed whether it was ordered by the noble (although it 100% was, there will be no justice for the order however, as the footman, along with the two guards that were loyal to him, will be blamed and executed as scapegoats).
Speaking of guards, the entire system of the guard is corrupt in some way, shape, or form. Some of it is greed, some of it is allegiances other than to the city/kingdom, some of it is sheer incompetence, and some of it that being a guard attracts bad people, and for many cities, the culture of the guard is incredibly toxic. Because the party has had to interact with the guard, they have seen every version of this play out.
However, not even adventurers are safe. The kingdom has commodified adventuring into "The Adventurer's Guild", a subsection of the military that pulls on the ancient tales of yore and the promise of fame and fortune. Essentially, it hires people that will go out and do the more dangerous things that the guard isn't equipt to handle or simply isn't their job to handle (like something out beyond the city limits). With a permit from the guild, parties have greater freedom in what they can do, can pick up contracts and jobs from the guild, and can even use spells that are illegal for a normal person to use. However, there are downsides. Your image can be exploited and catered specifically by the guild, and some of the money made with your contracts goes back into running the guild, which can be especially hard for new adventurers. It's an exploitative system, and the few who do get famous and rich and high level without dying get extremely close with the crown, meaning they can sometimes get away with things that they shouldn't. For example, some spells are simply banned, like Power Word: Kill, in order to make the general populace feel safe. However, if an adventuring party with a task from the crown used it to accomplish their mission, it's going to be swept under the rug.
There's one final background corrupting thing I'm going to hit on before I talk about the actual campaign, and that's capitalism, baby!!! There's a company called Smartshine that's existed for a while now that's exploding in size and taking up a large percentage of the magical item market. They're hiring a bunch of wizards to make magical goods, and sending out scouts to search for new talent and buy up components, recipes, and blueprints. They're getting connections to the crown, and will soon start being its main supplier of magical items as there were hard to come by before. With this being the first time in history any company has been this size, there definitely won't be anything that comes from it...
Just kidding, it's going to be a shitshow. Devils and other extraplanar entities all want a hand in the cookie jar, and from most of the executives being/will be corrupt, it's going to be bad for most people on the Material Plane.
Ok, now with all that background info and talks on corruption done bc its very fun to talk about worldbuilding, the main bulk of the first half of the campaign is going to be unwinding the layers of conspiracy, lies, hidden information, and corruption that surround the incident that started the entire campaign: some sort of entity was summoned that killed a dinner party group off one by one and caused them to be trapped in the mansion in some sort of pocket dimension controlled by the entity.
And this is where we get to the Doomsday cult. It's definitely not going to stay that, but I haven't come up with a name for them yet XD. Basically, about a 1000 years ago, it was discovered by a very small group of people that the world in which they are living isn't the first. The world and planes at large are on a cycle, with even the gods being different from cycle to cycle. However, there are some remnants that survive, like a book protected in a case of incredibly strong magic to survive even the collapsing of the planes into themselves that explained how to bring these gods back, promising godhood for those that do. So, there's a conspiracy known only to those who plan on being the ones to be gods to revive the old gods, kill the current gods, and become a new pantheon to rule this world. (Information is spread so that no one outside this tight circle knows the true goal and also to have it be really REALLY hard to trace information, aka, more arcs for me!!!) The dinner party entity was one of the first to be brought back, the goddess of discord and distrust. (Most of these gods are bad news lmao)
Going back to the plot, the party is trying to figure out what the entity was and how Valdis, the host of the dinner party, learned how to summon it, which they just realized he went to the other continent to get. The current lead they have is the architect that built the mansion, but because he's missing and his mansion is locked down, they have to get into the architect's mansion in order to see if he's alive or not (he's not). They're being helped by a member of the guard named Sorovar, who transferred from another city to here about two weeks ago and is in charge of the investigation. However, he actually was sent to make sure evidence that the architect had was destroyed, and items that those in charge of this entire plot needed got back to them. The architect was actually killed by a poison letter that was forged to look like it was from Valdis. The evidence that the architect had would include a letter from someone else saying there's some really suspicious stuff going on with Smartshine. This would eventually lead to the players finding out that one of the founders created it to facilitate the research and construction of the largest mass-death weapon ever seen to blow up the captial city of Menlin with the intent to bring back the old god of death.
There's multiple other plots going on, most of which iiiii need to still flesh out. There's one going on the other continent with vampires and the old goddess of winter, but that's not for a long while. But basically, the world is currently a powder keg, and the fuse has been lit.

Synopsis: In the ruins of the Multivex Corporation, Seward Trainer has completed repairing the device that will regenerate his withered body. Gaunt warns Trainer that he will tolerate no further failures. That's when they are visited by Gaunt's mysterious employer, who orders Seward to hurry as he is preparing to initiate his plans that will finally see the destruction of Peter Parker and Spider-Man. Seeing who this person comes to a total shock to Trainer, who realizes that Ben Reilly is in deadly danger and tries to think of some way he can warn his friend.
Meanwhile, in Queens, Peter and Mary Jane are arguing over the facts of an old Bugs Bunny cartoon as they wait for a bus. The bus soon arrives and Peter's pregnant wife boards it so she can ride into Manhattan for a shopping trip with her Aunt Anna. As soon as Mary Jane is gone, Ben Reilly drops out of the trees right on time for his meeting with Peter. At that moment, at the Daily Bugle, Ben Urich has gotten an unmarked envelope by registered courier. It appears to be a letter from Liz Osborn who has information she wants to provide him and asks for him to meet her at the Bugle at 10 pm that evening. Ben considers the good fortune he has making friend with Liz Osborn when he wrote his book on the Osborn empire. However, he is unaware that others in the building have also received similar black envelopes.
Later, Seward Trainer has managed to sneak away from his keepers and made it to Ben Reilly's apartment. However, as he knocks on the door he is ambushed by Gaunt, who is decked out in new cybernetic armor. Digging through the rubble for Trainer's body, Gaunt discovers that Seward somehow managed to slip away. While at the Daily Grind, Shirley Washington has just hired a new waitress named Alison Mongrain. Although Mongrain is warmly welcomed to the team, Shirley and her staff are put off by her cold personality. Back in Queens, Peter brings Ben Reilly up to the attic of their Aunt May's house. Peter explains that he and Mary Jane need to clear some space when they move in, and he wants Ben to go through things because they are his memories as well. Ben is touched by this, as when he was on the road for five years he lived a very minimalist lifestyle. Looking through all the belongings, Ben finds a photo of Peter Parker with his Aunt May and Uncle Ben. This brings both men to tears, as they both miss the two people who were like parents to them. That's when Peter opens up and tells Ben that when he was first told that he was a clone he freaked out, but he eventually learned from Ben's example how to live with the truth.
Back in the city, Seward Trainer has survived Gaunt's attempt on his life and trying to figure out what to do next. Spotting someone reading an issue of the Daily Bugle inspired Trainer to go to the Bugle building in the hopes of finding Peter Parker. That's when Gaunt catches up on Trainer and attempts to kill him again. However, Seward manages to flee the scene by hopping into a subway train. However, Gaunt rips open the subway car in the hopes of getting at him. Later, at the Osborn Industries Plant located at Islip, Long Island, Liz Osborn has taken a more active role in the company she inherited when her husband died. She is on the phone with her boyfriend and lawyer, Foggy Nelson. She has just got a letter from Ben Urich asking her to meet with him at the Daily Bugle that evening and asks Foggy to tag along with her. In Queens, Peter and Ben come across a box full of Peter's old books. They both reminisce about the good memories they had reading these books as a child. When they find their favorite book, Peter Pan, it prompts Peter to tell Ben something that he has kept secret since Aunt May's death. He tells her that on May's last day the pair took a trip to the Empire State Building where she revealed that he knew that Peter was Spider-Man for years and that she was very proud of him. He admits he kept this to himself, particularly after he learned he was a clone, as he wanted to keep that memory of his own. Ben understands and tells Peter that they need to stop letting themselves get distracted and get back to work.
Meanwhile, at the Daily Bugle, Paul Sousa the director of maintenance of the building interviews two recent hires. A lot of the staff suddenly quit and he has had to quickly hire new people. After reading the glowing references the two men received from their former employers -- Oscorp and Multivex respectively. At that moment, Seward Trainer emerges from the subway outside the Daily Bugle. However, he is attacked by Gaunt again and is forced to flee down an alley. As he begins making the distance between himself and Gaunt, Seward runs right into the arms of Gaunt's mysterious employer. Having grown tired of this loose end in his plans, the mystery man snaps Seward Trainer's neck, killing him instantly. By this time, Peter and Ben have become distracted again when they find Peter's old Battleship board game. They are interrupted by Mary Jane who calls to tell Peter that she and her Aunt Anna are going for lunch at the Daily Grind and asks her husband and Ben to join them. They agree, unaware that Gaunt's mysterious employer has been listening in on their conversation and has other plans for the Parkers and Ben Reilly.

Galleryyuhself has a new look and a new attitude. One of the goals of this Graphic Design Tumblr has been to amass information on work done locally and regionally in the field of Graphic Design.
 As an Educator I am interested in the future of Design in the Caribbean and Trinidad and Tobago. I want to know what we sell to the public, how we shift and change our habits and tastes and what the future will hold for our purchasing attitudes. I ask the question, how does Graphic Design affect my island and other islands? Is Design keeping up with social responsibility? The Plot so far (pt. 3)
Continuing the plot summary of the Graveyard Shift now on the new blog for it! If you want to read what happened previously, here are parts 1 and 2, along with a link to the masterpost!
Picking up from Yikes and Azule waking up from the nightmare, there's a collective panic as those who were in the nightmare ensure that their friends and coworkers are who they say they are. Frizzle (one of the NPCs from the first arc and was in the dream) confronts Todd, and Yikes and Azule confront Husk. Thankfully, no one's actually been replaced, and they hurry to finish traveling to Dradow.
While Yikes was recovering from the 4 levels of exhaustion from the werewolf bite and the dream, there was a bit of downtime. During this, Yikes and Husk talked to Anzor and confronted him about various suspicious things that he had noticed him doing. It culminated in them both asking if Anzor was undead, which Anzor unhappily confirmed, showing what was under the bandages covering his body: mostly dead flesh with ever-open wounds slowly oozing blackened blood, and an open, exposed torso devoid of all organs except for a glowing red stone replacing his heart.
BACKSTORY TIME: He was used as an experiment for a necromancer looking to unlock the secrets of lichdom by tying a soul back to a dead body to live in undeath. While the necromancer succeeded with Anzor, he wasn't able to figure it out for himself before paladins of Kelemvor burst in and arrested him. Anzor was almost killed, but one paladin stuck up for him, saying he was clearly sentient, a person with a soul, and deserved a chance at life. Being taken in by the temple (although unable to enter it due to Hallow), he studied and prayed for hours and days to Kelemvor, hoping for an answer to help him. Kelemvor responded in a vision, bestowing powers, an oath, and an enchanted pendant that would ease his suffering and cover the fact he was undead. He entered the temple for the first time and was officially a paladin to Kelemvor.
With downtime mostly over, Yikes had the party meet up with a contact of his named Bramble, a hyena shifter and bardic professor at the local college. Dradow is a city known for its two colleges of magic, one for wizards called Clockworth’s Academy of Abjuration, and one for bards called Autumnrun’s College for Debators, Orators, and all things Eloquent, which Bramble works at. He's been working on a book for the last 10 years about shapechanging, the races associated with it, and the myths about them. Yikes was interviewed for the book years ago, and now he's taking Husk to meet him to get more information on changelings. During this cheerful reunion, a student comes in and asks if Bramble has seen a friend of hers, Falthrax, an older dwarf who's also a student. After searching where he'd been previously, the party found a seemingly sentient book called The Price of Beauty (yes it's the module from Candlekeep Mysteries). Telling it their problems caused it to open a portal to a dirt path lined by stone lilies in the middle of a jungle. The trees are recognized by Husk as this is part of the same jungle they lived on the other continent in the country of Llaethpotel. The party venture in, assuming this is where the missing dwarf went, and Bramble promises them he will help on the other side. And with that, the portal closes, and they are on a different continent.
Following the path, they find the Temple of the Restful Lily, one dedicated to the god Sune. They are greeted by Saeth, a human man who IMMEDIATELY drew the suspicion of the party. This suspicion increased after Yikes attempted to charm him, and he attempted to charm Yikes right back. They rested in the sauna for a little, where they met Zevi, a veteran from the bordering empire of Ascopia and sent here to recover from a leg wound. Yikes was able to determine the sauna was filled with fey magic and noticed very slight movement in the water. After pushing Zevi out of the room, Yikes and Husk were able to coax the naiad that lived in the pool out of hiding by showing they were fey as well. She introduced herself as Cyrena, and she told the party the temple had been taken over by a coven of hags, and her friend and founder of the temple, Sylvarie, had been missing for a decade, presumed dead.
With a day of investigating, they discovered Falthrax and another victim of the hags in what used to be Sylvarie's tower which the hags use as their home now. They also uncovered cursed paintings that seemingly tied a hag curse to these victims. With the break in noticed by a gargoyle, the party was on a time limit to get these victims out. Zevi stepped up, additionally distracting the hags, and after ordering Bramble to open up the portal using sending, they got the victims through and a fight began. The party killed the hags, but when faced by Saeth, revealing himself to be a cambian all along, Todd was able to persuade a negotiation instead, for a trade of information: the hags' recipe for the cursed paintings for Saeth, and information about the fey and changelings for the party. Saeth reveals there used to be a changeling settlement deep within the forest, which was destroyed 3 decades ago, making Husk question why their archfey, Nonna, who lives in the forest, didn't tell them about that. Saeth leaves with the hags' recipe content, and the party returns to the temple to discover a massacre. The hags and Saeth had killed the other visitors of the temple before chasing the party to leave no witnesses.
It's a somber moment to mourn the dead, but their work isn't over. They find Slyvarie in the basement of the abandoned shrine of Sune, who has been transformed into a medusa and petrified Yikes and Todd. However, Cyrena was brought along per her request, and she quickly convinced Slyvarie everything was ok, and she would finally be safe. A few professors from the bardic college were sent for to un-petrify the characters, and they finally headed back to Dradow.
They received an award from the headmaster of the college for their excellent work saving their missing student and were allowed to keep the Price of Beauty. The party now has a fairly easy way to travel to a base of operations outside of the continent if they ever find themselves in trouble.
And with that, unfortunately, Azule and Todd leave the game here. Azule was almost outed as a necromancer and left the circus, and Todd, with a deadline to fulfill a deal to their warlock patron looming, decided to leave as well.
The party does add one new member to their ranks: the NPC Frizzle! She had been given the ability to not sleep by her patron after the nightmare as a show of good will, and she decided to stop being so scared and stand up for herself and her friends. With that, Yikes and Husk invited her to join them to try to take down Margylia, and while she is still terrified, she agreed.

Corneal Substructure Dosimetry Predicts Corneal Toxicity in Patients with Uveal Melanoma Treated with Proton Beam Therapy

Abstract
Purpose
The present study examines the relationship between dose to corneal substructures and incidence of corneal toxicity within six months of proton beam therapy (PBT) for uveal melanoma. We aim to develop clinically meaningful dose constraints that can be used to mitigate corneal toxicity.
Methods and Materials
Ninety-two patients were treated with PBT between 2015 and 2017 and evaluated for grade 2+ (GR2+), intervention-requiring corneal toxicity in our prospectively maintained database. Most patients were treated with 50 Gy (RBE) in 5 fractions, and all had complete six-month follow-up. Analyses included Mann-Whitney, Chi-Square, Fisher Exact, and receiver operating curve tests to identify risk factors for GR2+ toxicity. Bivariate logistic regression was used to identify independent dose-volume histogram (DVH) predictors of toxicity following adjustment for the most important clinical risk factor.
Results
The six-month PBT GR2+ corneal toxicity rate was 10.9% with half experiencing grade two and half experiencing grade three toxicity, with no grade four events. Patients with anterior chamber tumors had a higher risk (58.3%) for toxicity than those with posterior tumors (0%) or posterior tumors extending past the equator (25%, p<0.0001). On univariate analysis, larger COMS size was associated with increased toxicity rate (p<0.004). DVH analysis revealed that cutoffs of 58% for V25, 32% for V45, 51.8 Gy (RBE) for max dose, and 32 Gy (RBE) for mean dose to the cornea separated patients into groups experiencing and not experiencing toxicity with 90% sensitivity and ≥96% specificity. Bivariate logistic regression indicated that corneal V25, V45, and mean dose independently predicted for toxicity after adjusting for tumor location.
Conclusions
Patients receiving PBT for anterior uveal melanomas experience a high rate of GR2+ corneal toxicity due to increased corneal dose. Anterior location and corneal DVH parameters independently predict toxicity risk. We propose dosimetric constraints to facilitate treatment planning and toxicity mitigation.

In making no-case submissions before Principal Magistrate, Judy Latchman, the lawyer told the court that the charge against Westford was bad in law because constitutionally Westford, as a government minister, was not a public officer from 2011 to 2015. "A minister of government is excluded from this definition of a public officer," Todd said. He reasoned that Westford and Cummings were jointly charged but there was no case against the two and moreover Westford should not have been before the court because her charge was bad in law. He also contended that Cummings could not have been charged with acting in consort with Westford to commit larceny because, in the first instance, the former Public Service Minister should not have been charged.  The lawyer further said the court could not substitute the charge based on the current facts and still hold Westford answerable to a court of law. He added that there was no evidence that Cummings unlawfully diverted monies signed and accounted for to her own use. "The evidence is that she received monies. There is an inference that monies were in these envelopes," he said.  "The evidence led by the prosecution does not in any way implicate Cummings in conducting any illegal act," Todd added. The Attorney-at-Law said the prosecution also failed to show that anything done by the former Principal Personnel Officer at the Public Service Ministry was a criminal act or that she acted in consort with someone to steal monies and to deprive the government from their use. Todd said the prosecution did not establish that monies were stolen, Cummings stole GY$10 million, was responsible for that cash, responsible for collecting and counting it and collected it by signature. Cummings and Westford were charged in February, 2016 with stealing GY$639 million between August 2011 and April 2015 from the Government of Guyana. The money was allegedly stolen in 24 different tranches ranging from GY$3.4M to GY$75M in one instance. The Strokes - Someday Nosso pai deu de presente pra gente no aniversário de quinze anos nossos primeiros CDs originais. Comprados na Tok Discos do Iguatemi, perto da primeira praça de alimentação, onde passou a ser loja de roupa e depois loja da TIM. A franquia já tava nos finalmentes, assim como o costume de consumir música em suporte físico (Só bem depois é que fui saber que da tragédia nelsonrodrigueana alencarina da falência da Tok Discos, acompanhada da mudança no mercado e de uma provável má gestão). Como dois adolescentes público-alvo do comecinho dos anos 00, compramos os ROQUES entry-level que gostávamos da MTV. Eu escolhi a coletânea do Nirvana que tem “You Know You’re Right” e ele escolheu o primeiro album dos Strokes (era uma edição especial que vinha com um DVD com clipes e faixas ao vivo, mas sem a capa original, que era fantástica). Eu não dava o devido valor na época (talvez por não querer dar o braço a torcer, alguma competição imbecil entre irmãos do tipo sou-mais-do-roque-que-você), mas hoje em dia gosto bem mais dessa música e desse álbum dos Strokes do que na época. E nunca mais comprei uma coletânea na vida, o que mostra quem tinha razão na época, quem ganhou a competição imbecil quase vinte anos depois.

In Silico Trial of MR-Guided Midtreatment Adaptive Planning for Hypofractionated Stereotactic Radiation Therapy in Centrally Located Thoracic Tumors

Purpose
Hypofractionated (>5 fraction) stereotactic radiation therapy (HSRT) may allow for ablative biologically equivalent dose to tumors with a lower risk of organ-at-risk (OAR) toxicity in central thoracic tumors. Adaptive planning may further improve OAR sparing while maintaining planning target volume (PTV) coverage. We hypothesized that midtreatment adaptive replanning would offer dosimetric advantages during HSRT for central thorax malignancies using magnetic resonance imaging (MRI)–guided radiation therapy.
Methods and Materials
Twelve patients with central thorax tumors received HSRT using MRI-guided radiation therapy. Clinically delivered regimens were 60 Gy in 12 fractions or 62.5 Gy in 10 fractions, with low-field magnetic resonance (0.35 T) volumetric setup imaging acquired at each fraction. Daily gross tumor volume (GTV) and OARs were retrospectively redefined on fraction 1, 6, and 10 MRIs, and GTV response was recorded. Simulated initial plans prescribed a dose of 60 Gy in 12 fractions based on fraction 1 MRI. Midtreatment adaptive plans were created based on fraction 6 anatomy-of-the-day. All plans were created using an isotoxicity approach with a goal of 95% PTV coverage, subject to hard OAR constraints, to represent clinically ideal OAR sparing. Plans were then compared for projected OAR sparing and PTV coverage.
Results
Patients demonstrated significant on-treatment MRI-defined GTV reduction (median 41.8%; range 16.7%-65.7%). At fraction 6, median reduction was 26.7%. All initial plans met OAR constraints. Initial plan application to fraction 6 and fraction 10 anatomy resulted in 8 OAR violations (5 of 13 patients) and 10 OAR violations (6 of 13 patients). All fraction 6 violations persisted at fraction 10. Midpoint adaptive planning reversed 100% of midpoint OAR violations and tended to reduce the magnitude of OAR violations incurred at fraction 10. In 40% of fractions (2 of 5) in which OAR violation resulted from initial plan application to fraction 6 anatomy, PTV coverage was increased concomitant with violation reversal.
Conclusions
Midtreatment adaptive planning based on tumor response may be dosimetrically advantageous for sparing of surrounding critical structures in HSRT for central thorax malignancies and could be applied using either an online or offline paradigm.

Influence of total body irradiation dose rate on idiopathic pneumonia syndrome in acute leukemia patients undergoing allogeneic hematopoietic cell transplantation

Abstract
Purpose/Objectives: To determine the relationship between dose rate and other factors with the development of idiopathic pneumonia syndrome (IPS) in acute lymphoblastic (ALL) and acute myeloid (AML) leukemia patients undergoing total body irradiation (TBI)-based myeloablative conditioning for allogeneic hematopoietic cell transplantation (HCT).
Materials/Methods: From 2006 to 2016, 202 acute leukemia patients (111 ALL, 91 AML) ranging in age from 1 to 57 years (median: 25) underwent allogeneic HCT at XXX. Pre-transplantation conditioning included cyclophosphamide (120 mg/kg) with (68%) or without fludarabine (75 mg/m2) followed by 13.2 Gy TBI given in 8 twice daily fractions of 1.65 Gy over 4 days. Dose rate varied based on linac availability and ranged from 8.7-19.2 cGy/min. Patients were stratified by receipt of high (>15 cGy/min) (HDR) (56%) or low (≤15 cGy/min) dose rate (LDR) (44%) TBI for all 8 fractions. IPS was defined as pulmonary injury based on clinical symptoms, radiographic evidence, and/or pulmonary function testing within 100 days of HCT in the absence of concurrent infection.
Results
IPS developed in 42 (21%) patients between 4 and 73 days (median: 16) after transplantation. HDR TBI was associated with a higher rate of IPS compared to LDR TBI [29% versus 10%, P<.01]. On multiple regression analysis, HDR remained a significant predictor of IPS [HR: 2.6 (95% CI: 1.2-5.3), P=.01) and this led to inferior 1-year overall survival (60% versus 76%, P=.01) and increased 1-year non-relapse mortality (28% versus 15%, P=.02).
Conclusions
TBI dose rates ≤15 cGy/min reduce the risk of post-transplantation IPS and improve overall survival. LDR TBI should be strongly considered as an easily implemented parameter to improve the safety of pre-transplantation TBI-based conditioning.

Radiation Induced Cerebral Microbleeds in Pediatric Patients with Brain Tumors Treated with Proton Radiotherapy

Abstract
Purpose
Proton beam radiotherapy (PBT) has been increasingly utilized to treat pediatric brain tumors, however, limited information exists regarding radiation induced cerebral microbleeds (CMBs) among these patients. The purpose was to evaluate the incidence, risk factors, and imaging appearance of CMBs in pediatric patients with brain tumors treated with PBT.
Methods
A retrospective study was performed on 100 pediatric patients with primary brain tumors treated with PBT. CMBs were diagnosed by examining serial MRIs including susceptibility-weighted imaging. Radiation therapy plans were analyzed to determine doses to individual CMBs. Clinical records were used to determine risk factors associated with the development of CMBs in these patients.
Results
The mean age at time of PBT was 8.1 years. The median follow-up duration was 57 months. The median time to development of CMBs was 8 months (mean 11 months; range 3-28 months). The percentage of patients with CMBs was 43%, 66%, 80%, 81%, 83%, and 81% at 1-year, 2-years, 3-years, 4-year, 5-years, and greater than 5 years from completion of proton radiotherapy. The majority (87%) of CMBs were found in areas of brain exposed to ≥ 30 Gy. Risk factors included maximum radiotherapy dose (P=0.001), percentage and volume of brain exposed to ≥ 30 Gy (P=0.0004; P=0.0005), and patient age at time of PBT (P=0.0004). Chemotherapy was not a significant risk factor (P=0.35). No CMBs required surgical intervention.
Conclusion
CMBs develop in a high percentage of pediatric patients with brain tumors treated with proton radiotherapy within the first few years following treatment. Significant risk factors for development of CMBs include younger age at time of PBT, higher maximum radiotherapy dose, and higher percentage and volume of brain exposed to ≥ 30 Gy. These findings demonstrate similarities with CMBs that develop in pediatric brain tumor patients treated with photon radiotherapy.
