I weighed myself again this morning. I’m 7 pounds under what I was when I started seeing a GP again.
I was already feeling down on myself last night because I felt it was necessary to reschedule a meeting with my case manager for today to try to get in contact with my credit card company. And then feeling sick this morning and having another instance of urinary urgency and finding out I’m losing weight again and am even worse than before? Fuck!!
So I’m very weird today. Haven’t gotten out of bed yet other than trips to the toilet. Really, really didn’t want to go to therapy but like. I know logically that is when I need therapy most so I was going to force myself to do that at least. But then my therapist canceled the appointment instead. I could email him about how I’m feeling, but reaching out when I feel like this is so difficult. If I even manage to post this instead of deleting it, it’s a miracle.
I should get out of bed. But I don’t want to. I don’t want to possibly have to interact even just briefly with the roommate who is home right now. My partner is out on an errand for a little longer. But even if they were here to be a buffer I still don’t want to be seen today. I feel awful. I haven’t been crying much but I feel like I must look like I have been. I just very much do not want to exist today thank you, why do I have to have a body that requires things like food and a toilet and water? And god, my fucking medication. I take 5-6 pills every day currently and it’s only going to go up from here.
Why can’t I just. Know what’s wrong? Why am I losing so much weight? What do I need to do to make it stop? I’m so bony that I can’t even comfortably lay on my side anymore because my knees hurt to be stacked on top of each other and if I don’t stack my knees my hips and spine hurt from my back not being straight. And I feel awful even complaining about being skinny on here but like. It’s negatively affecting my QoL, so I’m gonna complain about it. And it’s scary! I’ve lost almost another 10% of my body weight in like 6 weeks! I don’t understand how, because I’ve definitely been eating better with the holidays happening!
And I still have to call the Patient Care Coordinator back about how the surgery referral hasn’t resulted in any kind of appointment yet. The ganglion cyst on my wrist is starting to cause me pain more frequently.
(Very open to suggestions about things I should be asking my doctor to look for at my physical in a couple of weeks, so if you see this and think “hey that sounds like what I/this person I know was dealing with maybe this blogger should ask their doctor to do this test” please leave a reply)

Ganglion cysts will often resolve spontaneously in 1 year. Can aspirate them, but there's a 50% recurrence rate in 1 year. If painful, can refer for surgery.
Our overall approach to treatment is generally to start with nonsurgical interventions which consist of observation or needle aspiration, given the limited morbidity of the lesion and the potential for spontaneous resolution. Surgical therapy is generally reserved for patients who have persistent or recurrent symptoms in spite of initial conservative therapy.
Observation — We suggest reassurance and observation for patients who are asymptomatic or do not want any intervention. Over 50 percent of patients may experience spontaneous resolution of ganglion cysts and do not require intervention. Published series of spontaneous resolution indicate that the majority of the spontaneously resolving cysts disappear within a year [16-18]. A brace can be used temporarily based on clinical symptoms, but use should be limited to avoid muscle atrophy that can be associated with long-term immobilization. Systematic joint inactivity due to pain from a ganglion cyst is also not recommended for the same reason for prolonged bracing. In general, pain severe enough to significantly limit activity would be an indication for surgical intervention. (See 'Surgical therapy' below.)
Ganglion cyst aspiration — For patients with bothersome symptoms, we suggest aspiration of the ganglion cyst. However, the patient should be informed that more than half of ganglion cysts treated with aspiration will recur within approximately one year [16]. In the case of volar cysts, caution is needed in order not to damage neurovascular structures, particularly the radial artery and the palmar cutaneous branch of the median nerve. We perform aspiration with an 18-gauge needle and a 3 mL syringe, or larger volume, depending upon the estimated volume contained by the cyst.

The ganglion cyst i've had removed from my dominate hand twice is back. The doctor was like "i want to drain it and shoot a steroid into it" because this is a different doctor from the guy who did the two removals, and I was like "okay" only he didn't numb my hand. And that needle was HUGE. It hurt so bad. And now several days later my hand just feels really tight and I have to wear a brace for two weeks and I have a feeling that I'm gonna need another surgery.

Naturally the minute I have the energy and motivation to write again after months of nothing my ganglion cyst in my wrist decides to make an appearance.
It's been 6 months since my second surgery. I've been battling this for over five years. I don't have the energy anymore.

My first surgery...
Having a ganglion cyst surgically removed in a couple weeks. Apparently, because it's on the underside of my wrist I can't just have it drained due to my radial artery being in that area. So I literally only had two options, leave it alone (In this case, it could possibly stay on my wrist for years being a general nuisance OR it could go away and almost certainly come back. It could also become very painful at any time due to being so close to my nerves) or get it removed...
Given those two options, I thought it best to just get it removed.
Problem? It will be my first ever surgery (unless you count my wisdom teeth). It's on my dominant hand (right), and I'm an artist...also, I hear there's a good amount of pain I'm likely to be in during the first few days even with the pain meds.
Not looking forward to this. Very sad, very scared, not happy. But I hate having something on my wrist that (while not very painful right now) could easily become very painful and impede my ability to work with my hands.

Hahaha no, sorry ortho I can't wait till the end of October or November for a hand appt.
That's a long ass time to be unable to really use my left hand! If this ganglion cyst wasn't like directly over a pressure point I'd probably deal with it till then because what's one more bodily irritation on top of my list of other issues?
When I called my pcp to tell them to resend it as urgent (as they had instructed) I felt like I was tattling on ortho like "Wweh! They won't give me a sooner appt they're so mean!". But I get it! Other people are also having hand issues.
I'm off for a week to try to get the inflammation down because everything I do irritates that thumb joint, and I can't imagine that a 3 month+ wait will make that better (esp with rush coming up...)
I will make dumb decisions to get myself out of pain loooong before then, like borrowing one of my wife's draw needles or jabbing it with a scalpel blade to aspirate it myself. Sad that that's the wiser option -in Ye Olde Days they used to smack them with bibles and I'd probably break my damn hand hitting it with a heavy art book or on a tree or something.
Please just, someone professionally poke this thing and stop this pressure/pain in my hand, I don't want to be a dumbass about it!

I'm reviewing questions. These are topics for review:
Microscopic colitis
You can begin induction of buprenorphine for OUD 8 to 12 hours after last use of opioid. If you wait longer, the pt is likelier to use. Reminds me of a pt I had in clinic who wanted to start buprenorphine, but couldn't because she'd taken methadone a couple days before or something like that.
Ganglion cysts. I remember being lectured to about this in med school. Apparently some people hit it with a bible. Lol. They usually self-resolve. No need to tx unless they're causing symptoms. You can aspirate them.
Kids with severe eczema or egg allergies shoulde be tested for peanut allergy with IgE or skin prick testing.
ARBs are the least likely anti-hypertensive medication to exacerbate erectile dysfunction.
You can give dexamethasone for malignant bowel obstruction at 4 mg bid to qid.
Bullous impetigo is due to staph aureus. Treat with TMP-SMX.
Blood on a dipstick could be a false positive. It could be myoglobin or hemoglobin or something. So you have to to urine microscopy to diagnoise microscopic hematuria (defined as 3 or more RBCs per hpf). Going further with a referral to urology for cystoscopy depends on the pt's risk for urologic malignancy. If high risk of urologic malignancy, then go for cystoscopy. Low risk pts can repeat urinalysis in 6 months.
Look up the PPROMT (Preterm Pre-labour Rupture of the Membranes close to Term) trial published in 2015.
Number needed to treat is 1/absolute risk reduction. It tells you the number of pts you need to treat in order for one patient to benefit.
Review the brachial plexus including muscles/action/innervation.
Antisocial personality disorder is diagnosed in those 18 y/o or older.
Apparently SSRIs like citalopram can increase risk of upper GI bleed by 55%! I didn't know that!
