One thing I love is how Baru’s hemineglect is written. I study neuroscience and I’ve never read fiction that attempted to describe this condition, but it’s written very accurately here (as one would expect, since Seth Dickinson also studied neuroscience). The seeing-but-not-seeing of half of her vision is nailed, but her “alien limb”, the fact that half her mind is “closed off” from her but still processing things, is really good. Like, when Apparitor threw the pen at her blind side and she still instinctively caught it, even though she didn’t consciously know it was coming; or when she vaguely felt that she should be seeing something, and turned her head and suddenly saw the sails through the window; or my personal favorite, when she mentioned while she can’t read or even know she’s seeing words in the right half of her vision, she can still get a clue as to their meaning, as though part of her is reading them and just unable to communicate them to her primary consciousness - which is a real thing that can happen! It’s fantastic! I’m so tired of books being wildly inaccurate about the human brain and here it’s detailed wonderfully! (There are also theories of varying validity that this, as well as other so-called split-brain disorders cause your consciousness to split into two, meaning there’s a locked up mind inside baru that’s unable to communicate beyond these reflexes and vague feelings - which I’m sure won’t be relevant in the next book, no, not at all, I haven’t seen spoilers -)
Of course there’s the obvious elephant in the room - while I thought I misread, apparently the blow to her head was the right side. The hemispheres of the brain have dominion over the opposite sides of your body. So if she hit the right side of her head, the left side of her vision and cognition should be compromised. But I obviously - since Dickinson knows his shit, and even Farrier I thinj called it out - believe this was purposeful. Because I would not be surprised at all if this is actually a somatic disorder - meaning it’s purely psychological; there’s no damage to her brain, but her trauma is manifesting as hemineglect. Baru herself comments at the end of Traitor that her condition almost felt too fitting - a perfect metaphor for everything she had gone through (and had put others through.)

Hemineglect has unveiled a new studio work: "Radial Arm"
October 2022 - Hemineglect has released new studio work:"Radial Arm". This release highlights the incredible passion, creativity and focus that fuel the band’s music. What makes "Radial Arm" stand out is the variety on tap: the band went above and beyond to create a sound that feels personal and understated, but also very spontaneous and easy to relate to for people of all musical backgrounds. Fans of progressive rock, alternative music, post-rock or art-rock will definitely connect with this. The song kicks off with a mesmerizing palm-muted guitar, reminiscent of classic tracks such as Pink Floyd’s “Run Like Hell.” However, the lush textures, emotional vocals and deep piano chords are reminiscent of Nine Inch Nails or Massive Attack, among others.
Ultimately, “Radial Arm” is a very energetic, yet hypnotic and mesmerizing song. It comes highly recommended to fans of artists such as King Crimson, Radiohead, Tool, The Mars Volta, Joy Division, Yes, The Police, Portishead, and Sigur Ros, only to name a few.
Hemineglect managed to carve its space within the music scene, and create something that’s a genuine calling card for Hemineglect and its music.
Find out more about Hemineglect, and listen to "Radial Arm". This release is now available on some of the very best digital streaming services on the web.

So hemineglect means that you can't attend to one side of your visual feild/motor side. Patients with (left-side) hemineglect will only draw the right side when asked to copy a drawing and will miss things in their left feild of vision.
When asked to draw a house with a fence and tree on the right side, a patient could name the items there but wasnt able to copy them down. When asked to cross out a page full of lines, she only crossed out the ones on the right.
Now here's the interesting part. The page of lines was moved so she could see it completely in her left visual feild. Still she only crossed out those on the right.
Why's this interesting? Because it's not that what she saw out of her left eye wasn't being processed. When she looked at something she split the item itself in half, no matter how it's being looked at. So she"d split the page in half and neglect the left side... even though it was in clear view in her right visual feild/completely on her right side.
She looked at it "as separate entities, divided each in half, and drew the right half* (-Ogden)

Stroke Rehab:
Stroke = reduced or interrupted blood supply to the brain. 85% of strokes are ischemic. 15% are hemorrhagic.
Stroke sxs depend on where the stroke is and which vessels are affected.
Stroke syndromes depend on which vessels are affected. MCA affected -> aphasia, hemineglect.
Dx stroke with non-contrast brain CT. First do non-contrast head CT. Can do MRI. CTA or MRA shows where the stroke is. Always do imaging before you do treatment.
Ischemic stroke tx = tPA within 3 hours of sxs. C/I if active intracranial bleeding, trauma, spinal surgery, high BP, low platelets. Can do intrarterial thrombolysis, mechanical thrombectomy, anticoagulant tx.
Subarachnoid hemorrhage tx = reverse anticoagulation.
Post stroke complications = dysphagia, loss of bladder control, contractures, increased risk of DVT (so give heparin prophylaxis).
CVA prophylaxis = eliminate risk factors, antiplatelets, statins, anticoagulant for pts with a fib. Carotid endarterectomy is done if the artery is 70-90% stenosed; it’s removal of the inner lining of the carotid artery. It removes the plaque.
For stroke rehab eligibility, pt must be neurologically stable, able to tolerate it, able to communicate.
Functional Electrical Stimulation = delivers electrical shock to muscle as pt attempts to do task.
Virtual rehab improves balance and gait.

“I am writing to share my concern regarding a series of unusual and troubling cases affecting medical professionals across the country. It manifests as a selective form of hemineglect in otherwise neurologically intact individuals. Despite functioning at a very high level in most aspects of their jobs, affected individuals are most commonly physicians. They appear unable, in certain instances, to perceive the presence of junior members of their health care teams. Notably, other components of the physical exam are likely to be normal, and focal neurological deficits are very rarely present. Thus, affected individuals are at risk of going unrecognized unless they are evaluated under specific circumstances that are likely to elicit signs of neglect. Medical students pose the greatest challenge to affected individuals, and their presence is most likely to be neglected by these individuals as a result.”
An entertaining yet provoking article on medical training and its result on behavior by Amara Finch at Emory School of Medicine

To make a gross generalization and oversimplification, much of the history of Western philosophy can be boiled down as attempts to answer either—or both—of the following questions: How do we know things, and does that even matter?
Most thinkers (woof—I’m definitely coming back to that term down the road!) up to and including the modern era were principally consumed with the former question.
The idea that our senses may be unreliable is not a strange thought to us SLP’s. We’ve all known a patient with hemineglect, a person with hearing loss whose “spouse always mumbles,” or a neurodiverse child that just needs some different input. But what about the rest of us within a standard deviation of the mean? Can we we trust that the world is as it seems? And if so, how?
Call it imprisonment in the cave. Call it the evil genius hypothesis. Call it the Matrix. Ultimately there’s always a possibility that we are all just characters in The Sims ∞, or maybe are constantly mistaking our wives for hats, just doing so less frequently than an Oliver Sacks patient. If there is a “we” in any sense of the word at all.
That this is not the case is frequently one of those assumptions mentioned last time. We take them a priori, but to do so means that we’ve been complacent in the answers we give ourselves when we stop to think “how” or “why.”
Assuming we stop to ask ourselves that at all.

Hemineglect introducing a new EP: Is Obsolete
Dark, soothing soundscapes and angular textures with a punchy feel.
December 2020 - This weird year is coming to a close, but there is still time to rake in some amazing new music! In fact, we’ve been discovering some of our favorite releases of the year over these last weeks. Such is the case of Hemineglect’s most recent studio EP, “Is Obsolete.”
This EP is highly recommend if you are a fan of artists such as early Bloc Party, Sonic Youth, Joy Division, Omni, Minus The Bear, or Preoccupations, only to mention a few.
The record blurs the lines between post-punk and alternative music, going for a direct and edgy sound with a darker texture. Over the span of 7 tracks, the band set out to showcase a diverse array of influences, including the angular post-punk of “Made of Meat,” the melodic gamelan-like textures of “Sleep Teeth” and the experimental musings of the final title track.
Find out more about Hemineglect, and listen to their music on Bandcamp or Spotify.
You can also find one of the tracks on The Noise Cult, a Spotify playlist dedicated to exploring shoegaze, post-punk, and alternative music with a darker twist.

7/10 to the wee hours of 7/11, we enjoyed the city of Halifax, Nova Scotia. Top to bottom:
-In Point Pleasant Park in Halifax, there is a haunted beach! Black Rock Beach is the site of a gallows and is haunted by the victim of a wrongful hanging. . . this was before the shipping facility set up shop behind it.
-there’s also an abandoned fort or two in the park
-as well as many elf palaces
-HVAC in a country that gets six months of winter
-United Statesian weirdos in a calmer world
-Hemineglect performing at Menz and Mollyz, the gay bar which hosted our very first show in Canada on this tour. Hemineglect was fantastic, and the first of the really wonderful musicians and performers we have been enjoying every night we have played a show here in Canada, so far.
-What self-respecting gay bar in Halifax, NS, is complete without a stuffed moose head above a slew of drag queen portraits? Canada, you’ve welcomed us with open arms! Thank you!

Fun fact; there is at least some evidence to suggest that there is not a single “seat” of consciousness.
If you sever certain connections between each hemisphere in the correct manner (this has usually been done as a last-ditch treatment for a form of epilepsy, I believe), you are effectively left with two independent processes; each side of your brain has independent perceptions, concepts, and impulses.
You could view this in two ways, I suppose; the first is that when you split the brain, you cut one process in half, effectively, resulting in two processes, each descended from the “original”. The other perspective is that everybody has both of these processes as a “default”, but when the brain is not split, they can communicate fast and efficiently, thus preventing independence.
Where this gets odder is when you realise that not only does each process control one half of the body, split down the middle (more or less, there are some inequalities between the two processes; for example, only the process in the left hemisphere can talk, since that’s where the language processing bits of it are), complete with potential conflict between them, but also each process can only see half of the visual field, again split down the middle.
The left side of the body will only respond to things in the left hand side of the visual field, and the right hand side of the body will only respond to things in the right hand side of the visual field.
So here’s a thought; hemineglect is typically caused by injuries to a particular location in the right hemisphere of the brain, and typically consists of the inability to perceive anything on the left hand side of the visual field, the body, etc, the inability to even conceive of there being a left hand side of things. It sounds to me like a possible cause of hemineglects is that, if the “two processes” perspective is true, then this might be caused by the process in the right side of your brain being killed.
Of course, I’m not really an expert in any relevant fields (like, say, neuroscience, psychology, biology...), so, you know, what I’ve said here is probably worth taking with not just a grain of salt, but a few piles. It’s worth taking a look at this stuff for yourself, frankly.
