#I try not to inundate this blog with critical stuff
Text
Okay I finally figured out how to articulate why the Loki ending (and the final costume) is not hitting right (because frankly, it’s more than I expected from the show, and under other circumstances it might actually feel like it fits)
It’s like the writers since TDW looked at Avengers Loki and believed every lying word he said. They believed his god speech. They believed his glorious purpose speech. They believed he wanted to rule the world. And from that, they concluded that his biggest flaw is pride.
Everything in the series(and Ragnarok) is based on Loki being too proud. He needs to be taken down a peg—by the characters, by the story, everything. He needs to learn humility. So by the end, he goes to his finale knowing “what kind of god he needs to be”. Because he needs to be a better god, right? And the costuming reflects this: as a vessel of the story and Loki’s “lesson”, it strips away the armour, the leather, the finery, and shows a more humble god. The costume does its job well.
But humility isn’t Loki’s flaw. It’s Thor’s flaw. Thor’s entire first arc was about humility. But Loki? Loki’s flaw is envy.
Loki’s story was supposed to be about not belonging, about finding his place and finding (and accepting) himself. And tbh, the ending of the series could have worked really well with that! If it had been more about purpose, about finding Loki and where he belongs, it might have been a decent ending. But the path to get there never reflected any of that. It was all about stripping him down instead of putting together his pieces. Being a god was never the problem.
And I still think the mcu’s biggest mistake was separating Thor and Loki in the first place

I’m back!
Tumblr media
So, I kinda quit using this blog a while back. There are some notes about it in my old pinned post and probably on my fanfics, lol, but basically, I felt overwhelmed by fandom and everything going on irl and I crashed. I just withdrew from like, everything. I still logged in on Tumblr but I just liked things to later view them with the boyfriend or laugh at them again myself.
Honestly, it’s not that fun, just doing that, and not really that fair to people who put effort into their posts like art, music, writing, etc., or even just people who are very passionate about a subject and may want it to reach others that are the same. Because I know I’m like that!
But anyway, this is going to be my new pinned post. So! (More under the cut, please keep reading!)
About Me
Tumblr media
You can call me Prowess, I’m a FtM transmasc (he/him please)! I use this space to elevate my personal works, such as art or writing, while also occasionally posting about topics I am passionate about. I realized I was trans in 2020 at the age of 19 years old, in the midst of a pandemic and college fallout, so as you can imagine, I’m still riding the rollercoaster of emotions that come with that. I’m also ADHD... so expect some posts about those sort of things.
But that is hardly the focus! I am big on art and writing. While I am mostly obsessed with Pokemon, I am a lover of stories, and so generally, I love most fiction, even the really bad stuff. Along with my Pokemon stuff, you can expect posts involving ARK: Survival Evolved, The Tales of Miraculous Ladybug & Cat Noir, animated movies (Luca, Toy Story, Spirit - you name it, I probably love it), and plenty of other stuff.
While I have too many older posts to go through and try to sort out the unclean tags, going forward, I am going to use these sort of tags: subject (whatever the post is about, for example, Pokemon), characters depicted, media (whether it’s art, writing, animation, or a mix), and any warnings that may need to go with it (for example, cussing when stronger language is involved, or racism if racism is getting discussed, etc). Memes or one-off posts will likely be tagged with simply “memes.” While this is pretty true for most any blog, this is so you know if you need to block certain tags! For example, if you hate Miraculous Ladybug, you’d just blacklist ‘Miraculous.’
I will also reblog a lot of posts asking for help or boosting awareness. I will try not to inundate the blog with these, but as I will likely have to make a post asking for help at some point myself due to unfortunate irl circumstances, I want to pay it forward in advance. These will be tagged with simply “help needed!”
Below are some projects I’m working on + what you can expect from this blog.
Pokemon Retold
By far, my biggest project to date, is Pokemon Retold.
This is an expansive written anthology of all the pokemon mainline games as far as generation 8. Red, Heart of Gold, Omega Ruby, Platinum, Black, Black 2, Y, Ultra Sun, and Sword are all intended to get rewritten and posted on both my FanFiction.Net and Archive of Our Own accounts. I may also post some chapters here, but I admit Tumblr makes that difficult sometimes.
These stories are very personal to me, as I started writing them when I was struggling in college with undiagnosed ADHD, and felt like my life was falling apart and I was alone. I started writing a retelling of Sword, but the more I wrote, the more disappointed in it I felt, so I started writing Black instead and scrapped Sword altogether.
I’ve had some rough road since, but Black is complete, Black 2 is also complete, and Ultra Sun is (mostly) complete (but with plans to be heavily altered), while Red is underway!
WARNING: These stories can get DARK, and I would rather my readers be over 18. They are not dark with the intent to squick out or upset people, however; they are dark with the intention of exploring thoughts and ideas I find too difficult to think about directly. There are more personalized warnings at the beginnings of each story. With that out of the way, please just try and move on if you find yourself disliking a specific story or part of a story of mine... If you want to give constructive criticism, you are more than welcome, but tearing into me and my writing just because you didn’t like something helps nobody. Keep in mind I write these for fun and as a form of self-therapy throughout some of the worst years of my life.
Furthermore, I edit them a lot. And I do mean a lot. My FFN profile acts as a “changelog” for what has changed in the stories over the course of the time they’ve been posted.
Pokemon Retold posts are tagged “Retold.”
My FFN Profile
My AO3 Profile
Pokemon Retold: Black has been added to TVTropes! I honestly don’t even care that this person digs into it a couple of times, I was so incredibly flattered they took the time to do that at all. (But be warned the page does have some spoilers!) I have no idea if you enjoyed the story or not upon reading over that page, kind stranger lol, but I really appreciate that you took the time to do that nonetheless! <3
Creatively Destructive
This incredibly well-titled story is an idea I have for a retelling of Miraculous Ladybug. I know, I know - absolutely never been done before /s
While I know there has got to be a thousand retellings of this show by now, based on just how.... terrible, the writing is, I want to try anyway. Because I genuinely love the premise of the show! I’m just constantly disappointed by how it seems to never go anywhere with its plot or characters.
So, Creatively Destructive will be a fanfic, and when I have more information on it, I’ll post it under the tag “creatively destructive.”
Roleplays
Me and my boyfriend love to RP a lot. From Monster Hunter to Pokemon, we do a lot of different stories. Sometimes, I really get inspired by what we come up together and I want to share it with others! Be it quick art based on a scene from the RP or a quick rundown of what occurred in an RP, you can catch these in the ‘Prow RP’ tags.
Currently, we are having a lot of fun with an RP based on the Hoenn pokemon games that I am sure I’ll be posting about!
I may also be open to RPs with others in the future :> Don’t be offended that I’m not open to them already... I’m just a very easily overstimulated person.
The Ballad of the ARK
ARK: Survival Evolved is a lot of things. It’s an amazing concept, an open-world game where you befriend dinos and fight alongside tribemates against other tribes, or work together to survive the hostile environment. But the true tragedy of this game goes so much deeper than that, and I want to explore that in writing, starting with my story, my experience, and then graduating to where I think the issue primarily stems from.
This strange memoir will be posted under the tags of “Ballad of ARK.”
Commissions
I hope to take commissions for art and writing in the future!
However, I need to figure out the parameters of what I’m willing to draw (and what I can feasibly draw), as well as what I can promise when it comes to writing, not to mention pricing.
That’s not to say I’m not open to ideas though! Feel free to message me/send me asks anyway and we’ll see if we can work something out. :)
Although I am not in a financial disaster just yet, I feel like I am not far from one. This is why I hope to try to build on commissions in the future. I know they will never support me, but I would still like to try to be useful and helpful to some. I currently live with my grandmother, having left my abusive home with my father, and although she generously allows me to live with her for the low cost of merely helping with some electricity and my own food, I struggle to remain abovewater financially due to difficulty finding work (thanks to issues with ADHD and physical problems following a bout with COVID in early January 2021).
If you would like to help anyway, feel free to subscribe to my Patreon, or make a one-time payment via PayPal. I promise no matter what or how much it is, your generosity is greatly appreciated!! <3
My Patreon

My Ko-fi
(Note that my Patreon is... very outdated and needs to be updated. I will do that soon!)

i saw you talk about “blocking etiquette” in one of your posts and i was wondering if you could briefly explain that? my blog is a baby (i joined in march) so i really don’t know any of the unspoken social rules haha. i feel i should know this before i say anything ever thats dream critical lmao
also saw you have a ton of asks so feel free to answer or not there’s no obligation ! take care of urself it seems like a lot to handle rn
I'd like to preface this by disclaiming that I've been on Tumblr since 2014, so I think I have a good sense of blocking etiquette as it's evolved with Tumblr culture, but I might still be presumptuous/wrong about some things.
Anyways. So, on Tumblr, there's generally always been a "block first, ask questions later" mentality. No matter the fandom, people kinda just unfollow or block people at will because Tumblr's algorithm is so shit at recommending things for people that unfollowing/blocking are really the primary ways to tailor what you don't want to see. So, if you frequent a search term/tag search, like "dream smp," you'd likely block someone who posts things frequently to that tag, but whose content you dislike. Blocking is, obviously, still used as a means to stop interacting with just plain assholes, as it is on every social media site. But more times than not, Tumblr users block blogs for neutral reasons related to how they want to tailor their social media experience, rather than a personal vendetta against someone.
Frankly, from my perspective, this was the main use of block lists back in the day. They weren't so much ways to truly hate on anyone's blog since anyone deemed a true asshole would likely spur a reporting campaign against them, rather than an addition to a blocklist. No, Tumblr blocklists were almost always so people who frequented certain tags simply... didn't have to put up with some people. If there was a wave of problematic shippers inundating a fandom/character tag, of TERFs inundating trans/feminism tags, or something similar, they would likely be put on a blocklist simply because Tumblr users wanted a comprehensive list of people they might come across, but didn't want to have to see posts from. Along a similar vein, a lot of mcytblr blogs advocate simply blocking people who post imagines in the main mcyt/dream smp tags, rather than harrassing each and every one of them into fixing their tag habits. Like, they're just annoying, really, and this social media site is so big that just blocking the ones who frequent the tag is so much easier than anything else.
One notable difference about Tumblr blocking as compared to Twitter blocking is the malintent behind Twitter blocking. Twitter fandom subtwts will advocate for blocking someone particularly if they're being cancelled/a Twitter user simply dislikes them enough. In addition, I've seen more than one Twitter user unironically say that they block people who unfollow them, as if following/unfollowing them were a personal affront, and blocking people were some component of this social game that reeks of cliqueness and high school levels of maturity.
That isn't to say that Tumblr blocking etiquette doesn't have pitfalls, because it has many. The main one I've noticed is that, especially in smaller fandom spaces, someone who's considered problematic might be enmassed blocked by lots of users in that fandom, and effectively be shut out of that fandom. If they're put onto a blocklist, a problematic post is circulated enough, or they're blocked by enough "big blogs" in the fandom, these users can be kept from reblogging, liking, or sending asks to a large part of the fandom they participate in. Vague posting doesn't really get you anywhere on Tumblr because of how insular each blog is, as opposed to Twitter, where screenshotting someone's profile who's blocked you and/or complaining very obviously about someone blocking you without naming them is commonplace and can be easily spread throughout a subtwt. This means Tumblr blogs who were mass blocked end up reaching much less members of their fandom, cannot really stand up for themselves in the face of mass blocking unless they're infamous enough for people to recognize their name, or participate in the discourse that promoted their blocking in the first place. While I haven't really experienced this, all of this is taken from what I've observed mutuals and recognizable blogs going through over the years.
Another note is that this website is kinda fucked, coding-wise. We all know this. This means that there are a million holes in the blocking system that make it even more annoying to navigate when people have blocked you, and that ends up getting more people blocked than maybe should be.
First of all, those imagine blogs aren't just posting in the main tags for shits and giggles, or to be cumbersome and clutter the main tags. They're doing it because Tumblr is dying, and even in a fandom as big as mcytblr, it's almost impossible to boost/promote your original content. The Tumblr algorithm sucks at spreading awareness about popular posts, more and more Twitter refugees means less and less people who fucking reblog > liking posts, and basically the only way posts can blow up anymore is by being found through a tag that is trending or frequently browsed through the "most recent" setting. This includes dream smp and many smp character names, so obviously imagine blogs are going to take advantage of this and maintag. It's the reason mcytblr constantly tags "m*necraft" despite being told off, time and time again, for doing so by mineblr. And it's the reason I've likely been blocked by many people- because I had to make the decision between spreading awareness of a post that took a lot of effort and that I thought was important enough to main tag despite being critical/negative, or keeping mcytblr happy by not cluttering their bias's tag with a crit post. Sure, some of mcytblr likely blocked me because they thought I was annoying/disagree with me, and that's fine. But I'm sure many blocked me for the same reason I block imagine blogs and mineblr blocks dream smp stans- because they wanted to peruse their fave's tags, and they simply blocked a random blog that posted something they didn't like, without really thinking much of it.
Second of all, side blogs kinda fuck up how blocking works. It's annoying as hell to see a post you really like in the main tag/search, only to find that the person blocked you upon trying to reblog. Because of sideblogs, people will oftentimes block a sideblog because it's that user's fandom blog, without blocking their main, so the user is left kinda seeing their stuff time and time again, without being able to interact with it.
Speaking of how fucky the code is because of the blogging system, reblogs make it even worse. Most social media sites simply make it so a blocked user cannot see another user's profile, posts and all. But Tumblr not only allows this, it kinda fucks up the search function by sometimes allowing people you've blocked/who've blocked you to show up in main tag searches, it didn't take into account the fact that you can still see an OP's post when someone else reblogs it, and Tumblr didn't think to just fucking fix all these holes and just wholly omit someone's posts if they've blocked you, or you've blocked them. So, despite blocking people, you can still be logged in and see all their shit, and they can see yours. It's kind of annoying as fuck.

Hi! I’m kinda new to atla, and came across some of your anti-zutara posts and Im trying to understand sth, if Zuko & Katara’s friendship itself is not problematic or misogynistic then why would a future romance be? I dont interpret their scenes as romantic exactly but I enjoy their pairing bc they do have a solid bond built on respect, understanding and work well as a team, and there’s potential for this to grow in to a romance with an interesting dynamic, esp after a few years of friendship 1/2
Your main anti-argument is the colonizer argument, but you also actively ship Zukka so I’m not sure how that works? Anyway don’t you find that that argument itself takes away from the point of Zuko’s entire arc, and assumes he wouldn’t dismantle the Fire Nations corrupt system? And there’s no reason to assume he would force Katara in to a role she might not be comfortable with or that Katara won’t have full say in how their relationship progresses or takes place. (oh and I’m not white lol) (2/2)
well, there’s a lot to unpack here.
first of all, platonic chemistry doesn’t automatically translate to romantic chemistry! from the little we’ve seen of their actual friendship, I think zuko and katara have something akin to the dynamic zuko could’ve had with azula had they not been forced at each other’s throats. the significance of katara being there for zuko during the final agni kai while azula is completely alone is thus heightened by this interpretation. if you think they have romantic chemistry, who am I to dissuade you? I personally think their similar intensity and flaws would make for a volatile relationship, but would lead to a friendship built on love and understanding, so that’s the interpretation of their dynamic i prefer. also zuko reads as a staunch kinsey 6 to me, but again, that’s a matter of opinion.
that said, i think that if you interpret any of zuko’s actions in trying to win katara’s respect as being because he has feelings for her, well, that framing is kinda creepy. especially the scene where he saves her from falling rubble and refuses to let go of her body. if you think zuko was intentionally trying to cop a feel here, then why exactly would you want him to be with katara, since he clearly doesn’t respect her. and then don’t even get me started on the people who think that time he tied her to a tree was sexy.... like, framing their relationship as romantic just doesn’t sit well with me for that reason. but anyway.
i think people have either gotten genuinely confused or deliberately obfuscated the distinction between criticizing a ship and criticizing a fanbase. my problem with zvtara in a vacuum is just that i think they’re not compatible romantically (despite having a lot of platonic chemistry and being one of the most compelling dynamics in the show!). I feel the same way about sokka and toph, and it’s just a matter of my preference for sibling dynamics over romance when it comes to these pairings! (i also think the age gap between sokka and toph is creepy even as adults because when you meet someone when they’re twelve there’s always gonna be a power imbalance there and there’s no way sokka isn’t aware of that. but i digress.)
to be clear, the reason i have been so vocal about my dislike for the ship is because it exists solely within the fan imagination, and that imagination is viscerally racist. I have said before i do not think shipping zvtara is inherently racist. I know there are black & brown women who ship it, and i’m not gonna act like their lived experiences matter less than a personal preference, but i think when white people advocate for this relationship, it’s really, really easy to fall into racist tropes. you said you’re new to the atla fandom, which means you haven’t been inundated with racist misogynistic bullshit that treats katara like a fetishized object for years the way i have. I don’t hate zvtara in a vacuum, but stuff like “fire lady katara” and pocahontas aus and whitewashed fanart – that shit takes a toll man. I’ve never seen any of that kind of racist bullshit when it comes to kataang fans, and like…. there is a reason for that!
for what it’s worth, if anyone thinks i’ve not been critical of people who ship zvkka falling into similar racist territory, then they must be deliberately turning a blind eye because i complain about zvkka as it exists in the fan imagination constantly. there are certainly racist zvkka fans, but something you need to keep in mind is how few there were until recently. before a couple months ago, when netflix decided to ruin my fucking life, only a handful of people watched atla through anything resembling a queer lens. when the show first aired, the idea that zuko could be interpreted as gay would’ve been absurd to 98% of the fanbase. so i wasn’t inundated with racism from zvkka fans because there were no zvkka fans. now, people on this cursed website watch everything through a gay lens, and it’s a nightmare. because they don’t even care about the characters, they just want an excuse to ship two teenage boys, and it’s deeply obnoxious and kind of concerning. anyway, i could talk all day about zvkka tropes i’ve seen that infuriate me, but that’s not this post.
like i’ve said in the past: I don’t think zvtara is ...inherently racist. what i do think is racist is the way it’s interpreted a majority of the time. if not for the stream of evil bullshit i’ve been inadvertently exposed to regarding this pairing over the years, i would simply say, “eh it’s not for me,” and move the fuck on! but unfortunately, i cannot do that, because i feel a responsibility to condemn racism when i see it??? I know you’re not trying to be deliberately obtuse, but please keep context in mind when it comes to questions like this, instead of getting defensive. I couldn’t care less about anyone’s personal tastes, as long as they’re not promoting things i’m morally opposed to, such as racism or misogyny. it’s possible to enjoy a thing and simultaneously be critical of it & the discussion surrounding it. that’s kind of the whole point of this blog.

yo, more star trek, less american politics, nobody cares about that
Hi anon, thanks for the ask--I hope you're well and staying safe 💛💙
Firstly, I really truly want to thank you for not swearing at me or telling me to "f*** off." I've gotten several of those kinds of messages lately and I very much appreciate that you kept your request/criticism civil. 💜
This might sound naïve, but I want to give you the benefit of the doubt since I have no idea if you're a Tumblr vet or a new arrival, if you're a minor or not, if you actually follow me or not, etc. etc.
I understand that not everyone cares about US politics. And I know I'm not an important person. But I am a person, and I do happen to care very deeply about US politics. I've lived right in the middle of the country my whole life, I'm 30 years old [which I only mention as proof of my sincere and legitimate disillusionment], I'm affected every day by the decisions and [in]actions of our local/state/federal officials, I've witnessed too many times the harmful consequences of large portions of the electorate being apathetic about what is supposed to be a participatory government, and I'm exhausted by the dangerous willful ignorance currently being exhibited by many of my friends and fellow citizens. Since the nation is in quite a bit of turmoil right now, it's highly likely that I'll continue to occasionally post or reblog information related to sociopolitical issues which hit close to home for me, because this blog is one of very few places I have to express my opinions and do what I can to support the causes I believe in.
A lot of people don't read tags, I know, so if you saw reblogs from me that didn't duplicate my tags, or if you skip over them in general, I want to assure you I try to be as vigilant and diligent as possible about tagging things I post and reblog (I have moderate obsessive compulsive disorder, so meticulous organization is... sort of important to me 😳). All of my recent posts concerning American and/or world politics have been tagged with #politics, #us politics, #tw: politics, and #tw: us politics so I would encourage you to put some or all of those on your filter list (bonus: then you won't have to see appropriately-tagged political posts by anybody on your dash 😉).
All of that said, I completely sympathize with using Tumblr as a means of escaping reality, I do. My blog has mostly been that exact sort of thing for me; I make a lot of posts about mental health, yes, but otherwise I like to think I'm pretty on-brand with K/S content and that I don't inundate people's dashes with content unrelated to my main focus. Fair warning, though, I do intend to keep posting/reblogging activism-related resources as I come across them during this period of extreme unrest in the States/around the world, as well as, I'm sure, an occasional political or current-events-type post here and there in the future. I've come to understand just how much of a privilege it is for me even to have the option of ignoring the news and escaping into my fictional interests when I get overwhelmed, and that newfound understanding has me wary of escaping too often and becoming complacent about the struggles of other humans, people who I might be in a position to help simply by paying attention. So...
TL;DR if you follow me but don't want to filter your dash by the above tags, I encourage you to consider unfollowing; while I'd hate to see you go, I don't want the content I post to trigger or upset anybody. If you don't already follow me, I wish you nothing but the best and would advise you not to spend too much time on my blog in particular if political stuff bothers you. Either way, just please do whatever is healthiest for you.
Be safe, and LLAP friend. 💛💙🖖

INTRODUCTION + RULES
Heyaaaa, name’s Ann, and this here will be a little blog where I can express my small hobby for writing. As you may have gauged from my blog’s name, it’s primarily going to be Enstars and IDOLiSH7 related, so hopefully y’all enjoy that. Just before we head on though, here’s some rules and things to keep in mind when requesting, etc.
Rules
1. Absolutely no NSFW topics please. I’m very uncomfortable with the thought of them in general, and if you could refrain from suggesting them then that would be amazing.
2. Keep general sensitive topics such as suicide and depression when requesting to a minimum. I may write on them occasionally, but I usually prefer not to, as I know some people may feel distressed over these sorts of things.
3. Scenarios and headcanons are both okay, I am happy to do either! I also can do text message format where you’ll have [Character name] : <text>, something like that.
4. Fluff, angst, AUs, x Readers, !personalities, all of these are fine, and I’m quite open towards any other new requests. You can ask for Character x Character if you like but I may not be able to write them to the best of my ability, if you’ll forgive me for that...
5. Have fun and enjoy reading!
General things
- I will try to write in order of the requests at which they come in, so depending on how many I have, it may take more or less time. I ask for your patience whilst you are waiting for me to finish writing.
- I am still a minor so I still attend school, meaning I am a busy bee, and I may not get around to finishing requests during stressful times, so if you could please understand that I do not ignore things you have sent me, more so I am just simply inundated and lack the time to write.
- I am fully accepting of constructive criticism if you have any! Letting me know both the ups and downs of my writing helps me to improve! However, I will not tolerate rude comments made towards me about the way I write or myself in general. As they say, treat others the way you want to be treated. Mindless insults thrown at me will be met with a fat uno reverse card.
- Be specific in your requests if you want certain elements to be included. If you simply ask me “Can I request headcanons/a scenario for [character]” then I’m going to interpret that in my own way and write whatever I am feeling for, so do not complain if it is not the result you were looking for.
- There’s a lot of debate about the pronouns used for both Arashi (ES) and Anesagi (I7). I am happy to refer to both of them as per the request details, so if you prefer them to be called by she/he and her/him, then make sure to give me a little notice when requesting. If you do omit the pronoun preference, I will stick to using them/they go keep it neutral.
Well, that’s all from me, regarding my blog rules and stuff. Happy reading everyone!

A Time Capsule
I’ve been lurking across several fandoms spanning a decade now, since my days of reading “Bones” fanfics on fanfiction.net. Before any inkling of Ao3’s existence. Maybe longer, my memory is murky at times.
I’ve never made a splash in any fandom, so to speak. I’ve always been content to stand shrouded in anonymity, residing on the edges of fandom, never an active participant. Perfectly at peace to never have a voice. Never brave enough to want to be heard. It has only been in the last few years that I discovered Tumblr and felt comfortable enough in taking advantage of its anon feature to interact mostly with The 100/Bellarke crowd, “conversing” with one user in particular. In the instances I chose to speak, there was safety in knowing my words never had an identity attached. A safety that lent itself to sending anon asks a fairly common activity until I wrote one recently sharing a remnant of my “The 100” viewing experience. The warm response from the users who read it left me smiling for the rest of the day. Their reply took a direction I didn’t expect. They encouraged me to take credit for my words under my username, which of course, I didn’t have, not being a Tumblr user.
I was flattered by the response, bolstering me to continue the line of conversation with another ask and was met with reiterated sentiments.
In the wise words of one of those awesome people,
“I was the ultimate lurker for a long, long time. I had a Tumblr account for four years before I ever made a single post, and even then I had to be talked into it. And you know what? When I finally starting “talking,” it was so freeing! Even if no one else was listening, even if I was speaking into the void, I was no longer dependent on anyone else to share my thoughts and opinions. I could do that myself.”
I took the compliment but waived the advice. Tumblr is made of communities built upon sharing and I have always been unto myself an island. It goes against my shy, introverted nature to take part in a community. I have no business pretending I have a place there. None at all.
And yet, despite my misgivings, the idea wouldn’t leave me as I believed it would. I started to genuinely ponder the merits of creating a blog.
There are strong reasons to support the affirmative.
First, the utilitarian benefits. In the absence of a blog, I turned to alternative methods of archiving appealing posts. If by some miracle, the item count of my browser reading list hasn’t yet ascended to the thousands mark, it most assuredly rests in the hundreds. My camera roll queue has indubitably reached the thousands count, currently sitting pretty at 3,300. I shudder to think of the sheer number of my bookmarks. One hundred and eighty notes on my phone. The final frontier has been broken, at last, habitually inundating my laptop with screenshots. Long has it been overdue to clean house.
Second, I find writing to be a herculean undertaking I enjoy in the moments it doesn’t drive me to the brink. A slow-going process, but when I’m able to appreciate the fruits of my labor, marvel at the polished product, I often feel quite proud. Writing is a skill I’ve lost touch with over years of disuse but found incrementally returning while expressing my opinions via Tumblr asks. Like any skill, it can be honed with time and practice. Transferring my streams of consciousness onto written medium challenges me to think critically, ask myself if my POV genuinely holds true or falls apart, requiring further reflection. If nothing else, it’s a good way to process thoughts and emotions. I find it easier than and therefore preferable to oral communication. I am a perpetual editor, always amending my statements which can’t really be done as effectively in speech.
Third, if there was ever a time to join the Tumblr fandom I’ve found a home in for the last three years, why not in time for the show’s last ride? The night I signed up for Tumblr coincided the first day of “The 100” cast and crew filming their 100th and poetically final episode. Around the same space of time, we got a release date and the nostalgic goodbyes of a few cast members rolled in. I know when Bellarke crosses the last threshold, I’d want it plastered all over my dash and I’d be able to make it happen.
But where there are pros, the cons inevitably follow.
Do I really need a further distraction from my responsibilities, spending additional hours and expending more energy I should not spare online? The too easy potential for more hours behind a screen when prone to headaches and horrid habits of not regulating my eating and sleeping schedules? The answer is a clear and resounding “No.” Would maintaining a blog be harmful to my mental and emotional health? Remaining anonymous has historically done a fine job of insulating me from general rebuke, which has mitigated the risk of reproach at least. No corner of the internet can be designated as a safe space. I knew I would in all likelihood have to work diligently to curate and be responsible for my experience, leading me to doubt how the effort could possibly be worth it. How could it be worth feeling exposed, self-conscious? Constantly second-guessing myself, debating whether or not my thoughts are best kept within the privacy of my mind to avoid stepping on anyone’s toes? Combating the periodic skepticism that my thoughts possess value worth writing?
There was always the lingering possibility I was overthinking the decision to my detriment, as is my norm. After all, it seemed silly and dramatic to regard one obscure little blog in a sea of hundreds of millions of social media users as momentous. But I know myself better than that. It is a really fucking big deal for me.
I vacillated between both sides of the argument for days before deciding not to follow through with the venture.
And then one night, a single stray observation ran through my mind. One observation became another, became another and before I knew it, I had formed the grounds for an entire meta post. It didn’t end there. More ideas filtered through. I expanded on those ideas. More traction gained. Another meta formed. More jumping off previous points. Before long, I had mentally written the foundations for four metas. And I was so excited and proud of forming these connections to this puzzle without even trying that I wanted to share it. I sat down to write them in my trusty Notes, outlining, trying to jot the main points down before they fizzled away from memory. I saw how long-winded these spiels had gotten sans the full writeup, subsequently rationalizing…well, not blowing up someone’s inbox is just good manners, isn’t it? And terribly inefficient to boot. More to the point, it seemed a disservice to myself to censor my rumination to fit the small confines of a Tumblr ask box.
The part of me that wanted to push forward envisioned what the future of my blogging efforts may look like. That part knows that this blog is for me and only me. What makes me laugh, what makes me cry. Smile. Rage. Flail. Think. Whatever the hell I want. I get to say what I want, however, I want. It’s incredibly nerve-wracking. It’s also exciting, thrilling, and yes, freeing. The notion of carving out a tiny space for me to fill to the endless brim with whatever brings me joy makes me…really damn happy. It’s not an easy feat to accept and harder to retain. I should be ok, so long as I never forget that I get to be in control of what happens here. It’s within my right to block anyone I don’t want to engage or associate with. It’s my full right to not care what anyone else has to say if I don’t want to. Block out anything negative I don’t want to endure with only a few clicks. If I decide I want to walk away, permanently or otherwise, for any reason, it’s within my right to do that too. It’s comforting.
There was a time when I “knew” I would never sign up for an Ao3 account until one of my favorite authors withdrew the majority of her stories from public consumption. I “knew” I was never going to post commentary until I did. I “knew” my username would never be seen by anyone aside from me, never to be affiliated with my commentary until it was.
I did. Each and every time I thought I would never, I did. I broke my own barriers with patience and some courage. Maybe the most intimidating aspect of something new is simply the beginning. I said earlier that I’ve been an island for nearly as long as I can remember. It’s still true, I don’t expect overnight results. It’s probably going to be true for a long time. Perhaps forever. But maybe it’s all the more reason why I should take this step toward peeking out of my self-imposed shell. Do what scares you, or whatever it is they say.
I wish I could say it was enough to reverse my earlier verdict.
Nope, I had to agonize some more.
What can I say? Fear is a damn powerful inhibitor.
Lo and behold, as if the universe took pity on me, I got the chance to communicate directly with the same awesome lady whom I quoted above and she kindly offered some more merciful wisdom to a truly maddeningly indecisive individual:
“When you create a blog, you are STILL anonymous. You have a username, yes, but it doesn’t lead back to you unless you want it to. You still have your personal privacy. Tumblr isn’t Facebook. If you want to disclose personal information, you can, but you certainly don’t have to.
And second, your blog is for you, not for anyone else. It’s for you to express your own opinions. Or create gifs or other visuals. Or just repost what other people create. You can be on every day, or just once a week. It’s also a great way to save stuff you might want to look at again. And then… and then… when brilliance suddenly hits you, you have somewhere to let it hang out! 😁”
It was much I had already considered, but it helped immeasurably to have my reasoning reaffirmed from an external source I respect. I logged into Tumblr for the first time the very same night.
After much deliberation, an uncharacteristic burst of bravery and a grueling four hours I owe to technological ineptitude, I have, tentatively and cautiously, opted to give this Tumblr thing a go.
With luck, a day will never arrive when I dust this preamble off for a much-needed pep talk. Instead, it is my hope that one day, this memo-to-me will stand as proof that I don’t always need to be afraid of the unknown. Not all endeavors have to be as frightening as they may appear. And if I can apply this attitude to all else suppressing my personal growth, I might just be peachy someday.
Bearing this in mind…
…here we go.

Tsunamis!
Since summer is [finally] here and the weather’s [finally] warmed up and school is [finally] out for vacation, this month’s post is going to be about tsunamis and disaster preparedness for the beach.
Unless you live at the beach, this gets into some tricky questions about how prepared you need to be for a disaster, even if you’re just going away for vacation.  As with all disaster preparedness, people have different levels of comfort.  I think the bare minimum for spending any time at the beach involves knowing the warning signs of a tsunami and what to do.  Someone who is more prepared [paranoid?] might look up inundation maps and disaster plans for every town they visit on the coast.  (They’re here for Oregon, if you’re wondering.)  I have lived and worked at the beach for most of my life and haven’t done that, so don’t feel bad if it’s not part of your vacation planning.  We’re all just trying to balance normal life with our best efforts to survive something that may or may not happen in our lifetimes.
The Oregonian has a pretty good article summarizing the situation.
The State of Oregon Department of Geology and Mineral Industries has tons of information on their tsunami site.
 A quick note on semantics
Years and years of physical oceanography courses mean that I can’t help myself here.  Tsunamis and tidal waves are not the same thing.  A tsunami is caused by something that displaces a large volume of water (earthquake, landslide, meteorite impact, volcanoes, etc).  Tidal waves occur constantly and are caused by the gravitational attraction of the moon and sun.  The more you know, blah blah blah.
What is a tsunami like and how worried should I be?
Like all things in nature, people have all kinds of similes for what a tsunami is like.  “It’s like a moving wall of water” or “like a river flowing on top of the ocean” or “sounds like a freight train”.  Like most descriptions of the ocean (and I’ve probably heard them all), they’re not inaccurate but are totally insufficient to explain the phenomenon.
If you Google “tsunami videos” you’ll come up with a lot of hits, largely from Japan in 2011. I’m not going to link to any of them, because I find them hugely disturbing to watch.  If you have any illusions about surviving a tsunami by “swimming it out” go spend some quality time on YouTube and get back to me.
This doesn’t mean I live in fear of tsunamis, any more than I’m afraid of riding in the car (far more likely to kill me).  It does mean that I’m aware of what to do in the event of an earthquake or tsunami warning.
 Two different tsunami scenarios
A tsunami is a bit different than an earthquake.  In Portland, you either feel an earthquake (danger!) or you don’t (no danger!).  At the coast, you may feel an earthquake (immediate tsunami danger!) or you don’t (still possibly tsunami danger!).  If the earthquake is far offshore, it may be undetectable by people on land but still generate a destructive tsunami.  For example, a major earthquake in Japan can cause a tsunami in North America (and vice versa).  Nerd out on this!  The good news is that an earthquake in Asia will have a travel time of 4 to 20 hours to the west coast of North America, so even though you haven’t felt the earthquake there’s time for word to come down from official channels.
What to do if there’s a tsunami coming
This is, on the surface, pretty straightforward.  Head for high ground, the higher the better.  This can get tricky in towns like Seaside, which have a river between the beach and the evacuation zone.  Watch for those tsunami evacuation signs, which will look something like this:
Tumblr media
This is probably an instance when it’s a good idea to follow the crowd, as long as the crowd is moving away from the beach.
Head to higher ground immediately if you’re in the coastal inundation zone and you
a) feel an earthquake,
b) see the water rapidly receding from the shore, or
c) see the water rapidly rising like a time-lapse of the tide coming in.
Follow instructions, if available, if you
a) hear the tsunami warning sirens, or
b) get a tsunami alert on your phone (sign up here).
If you get either of those signs with no other information, just head to high ground immediately and stay tuned for more instructions.
I’ve cited Ready.gov quite a bit in previous blog posts.  However, their tsunami advice is:
“If an earthquake occurs and you are in a coastal area, turn on your radio to learn if there is a tsunami warning.”
THIS IS LITERALLY THE WORST ADVICE I HAVE EVER HEARD. THIS ADVICE WILL KILL YOU. Bad Idea! If you’re at the beach and you feel an earthquake, don’t go find someone with a radio.  Don’t sit on the beach and search the app store for something that’ll connect to local internet radio or tell your phone to tell you if you should evacuate.  This is, really, the only thing you need to know about tsunamis and I cannot state it strongly enough:
If you are at the beach and you feel an earthquake, or see tsunami warning signs on the ocean, HEAD FOR HIGHER GROUND IMMEDIATELY.  Do not collect your stuff, do not go back to your car, do not tune your radio, do not wait for instructions from an authoritative figure, do not pass Go and definitely do not collect $200.  Start moving inland, and do it as fast as you can.
This British girl on vacation in Thailand recognized the warning signs of a tsunami (without feeling the earthquake) and, although there were only minutes before the tsunami struck, was able to get the beach evacuated and saved everyone’s lives.  Have you done anything that great lately?
What if you can’t move out of the inundation zone?
If you can’t move to higher ground at any speed, your next best option is the roof or upper floors of the highest building you can find.  I hesitate to give this advice.  In Japan, there were a number of buildings built to withstand both the earthquake and a tsunami, and were used as evacuation centers.  In 2011, the tsunami was larger than anything predicted, and people were washed off the roofs of places they had been told were safe for evacuation.  I find this heartbreaking so avoid it if possible.  Also, none of the buildings on the Oregon/Washington coast are, to the best of my knowledge, expected to withstand an earthquake and/or a tsunami.
We’re out of the inundation zone and feeling pretty good about our location.  Now what?
Now is definitely the time to wait for official instructions to come down from above.  Tsunamis often come in sets of waves for more than an hour and the first wave is usually NOT the biggest.  Do not go back into the inundation zone until you get the all-clear.  This is probably an instance when it’s wise to not follow the crowd unless you’ve heard the all-clear for yourself.
 Emergency Kits and Evacuations
Unlike my past posts, there’s not a lot to do for prep for a tsunami.  If there’s a warning related to an earthquake in Asia, there’s likely time to evacuate via car.   If you don’t live in the inundation zone, hopefully you’ll be able to get home. Alternatively, hopefully you have sufficient supplies in your car.  We keep a Go-Kit backpack in our car, which is a good idea for a whole host of reasons- we’ve got some minimal prep while on vacation, we’re better prepared if we’re split up or running errands during an earthquake, etc.  But I’ll be honest, it’s kind of a pain in the ass to be moving around between the dog in the way back and passengers in the back seat.  I see that it’s not going to work for everyone.  As always, balance your emergency prep ideas and your actual management of day to day life.
Here’s what I would consider minimal to keep in your car (Note that this does not match what I would consider to be a full evacuation kit):
-          First aid kit
-          Blankets and/or towels.  Space blankets are great.  We also keep real blankets and towels in our car, less for emergency prep and more for cold weather/rain incidences in the winter and laying out in the park/impromptu swimming in the summer.
-          Rain coats for everyone (this gets hard in the spring/fall because I keep pilfering from the car when the weather changes and then I’m left without in an emergency)
-          Water- how much depends on how many people are in your family or can be expected to be in your car
-          Some kind of snack item, like suet or granola bars
-          Cash for gas or other supplies in case the power is out.
But there are a lot of tsunami evacuation zones that don’t involve you being in your car (don’t go to the beach and sit in your car the whole time!).  As I stated above, you don’t get to go back to your car to get your kit before heading to high ground.  Not to put too fine a point on it, but your overall odds are better if you survive the tsunami without your kit than if you drown with it.  However, if you’re in or near your car during an earthquake, definitely grab your kit.
FAQs/Things I hear about tsunamis that are just wrong
Can’t we just evacuate to the school?  They wouldn’t build a school in the tsunami zone!
Ah, sweet summer child.  Yes, we’re willing to put schools in areas that are both in the tsunami zone and are also virtually impossible to evacuate from.  We’ll also put hospitals there, and we’ll locate all of our administrative and essential life-saving services there, too.  [Don’t climb up that high horse, Portland, you live on a fault line and the odds are very high you send your kids to school every day in a building that’s not seismically retrofitted in any way (and has lead in the water to boot).]
If there’s going to be a tsunami, where’s the best place on the Oregon coast to watch it come in?
Right down on the sandy beach.  Natural selection is a critical part of the evolutionary process, I’m glad you’re willing to chip in!
I’m going to wait for when the Coast Guard evacuates people.
I’d recommend against this, since the Coast Guard will not be organizing civilian evacuation in the event of a tsunami warning.
If we hear there’s supposed to be a tsunami we’re just going to take our boards down to the beach and surf that thing!
Yeah, you guys are still as stupid as when we were all in high school together.
Is there any point in trying to evacuate if the tsunami comes that fast?
Absolutely. Those who survived the 2011 tsunami in Japan were largely those who immediately headed for the highest ground they could reach.  Increase your odds and give it a try, what have you got to lose?
Other tips for your beach vacation
-          If you don’t know how to swim, don’t try to learn in the ocean.
-          Keep an eye on the kids, even if they’re not yours.
-          Swim parallel to shore if you’re caught in a rip tide.
-          Avoid Mo’s, the clam chowder is better anywhere else in town.  In any town.
-          You should get out of the water when your lips or fingers turn blue.
-          Be nice to the locals.  You are not one of them and that’s okay.
-          Don’t play on floating logs.
-          Yes you forgot your coat, and yes you’re just going to have to buy a sweatshirt in a silly boutique.   It’s 90% of the coastal economy.

Blogging thoughts
So people have been liveblogging (live-blogging?) Cat Returns, and it’s neat, and I kinda want to do something like that, but I have nothing interesting to offer for that movie.  It’s all been covered, I think.  I said my piece on the cat parade, and Baron’s dubbed voice.  Really the only other thing I could add is the body type problem. (Note to self-do a psot on TCR’s body type problem.)
But I kinda want to do a nerdy commentary type thing like that.  On something I’ve watched a lot.  That I have interesting stuff (I think....?) to say, that hasn’t been said before, that I have some knowledge about.
Oh.  Webber’s Phantom.  I’ve got the 25th anniversary DVD, it’s a really good production, which also does some things differently (mostly in ways that are a bit better, a couple not, which I can ALSO talk about!).  And it’s a go-to gun garbage watch for me when I’m not feeling great.  And I’m going to have a couple days next week alone in the house where I can really crank the volume up (a must) on the big screen TV (also a must).
And maybe even do a tiny bit on that perfume-commercial pretending to be a dramatic scene in the movie version.  Just cause.
Only downside is I have to try and figure out a set of tags that will filter out all the rabid Webber Phans who can’t take critiques.  Or I’ll just be inundated with screams of “Agh, how can you criticize this masterpiece opera?!”  ANswer: It’s not an opera.  Or a masterpiece.  (The first is objective, unarguable, by-definition fact; the second is my opinion and I just don’t care what you think because you don’t know the difference between opera and musical theater bwaha.)  ((I’m not a snob, I just care about definitions.  Wagner wrote operas, but they were not masterpieces either.  But if I was sad instead of laughed at the people who said Webber’s Phantom was opera, the number of classical music message boards that started with ‘discuss your favorite opera’ and devolved into ‘Phantom versus Les Mis’ flame wars would have broken me years ago, and the despair would have transformed into rage and I’d have destroyed the world by now.  Mocking laughter is better.))
I don’t care except that I’d have to figure out how to delete comments from my page so I don’t have to scroll through them, and it took me this long to figure out how to block the pornbot followers so they wouldn’t show up.

How to Meditate Every Day
Believe it or not, I can teach you how to meditate in less than a minute.
You don’t need a complicated method. You don’t need an elaborate breathing technique. You don’t need an unpronounceable mantra.
Instead, just look up at the sky or out the window and take that moment to center yourself. Take a few deep breaths, and step outside the drama of the day. Try this for 30 seconds.
Did you try it? Congratulations. You just meditated!
You’ve just gained a valuable tool to help deal you with the stress of daily life.
Meditation is a very natural thing. It’s something people used to do all the time without even knowing it.
Any time you’re just hanging out by yourself, sitting on the subway, waiting for a doctor’s appointment, or standing in line at the supermarket is an opportunity to meditate.
Of course, if your mind is racing and going in a million different directions, that’s not meditation.
But whenever you just let it all go, enjoy the scenery, take it all in, and “veg out,” in those moments, you are meditating!
But most of us don’t have that anymore.
What do we do now during those moments? We pull out our phones. We no longer give our brain that space.
Thanks to phones and other devices, we’re inundated with information all the time. We’re losing our ability to just exist in the moment and the ability to process our thoughts that this existence gives us.
It’s especially an issue with our kids.
I remember sitting in the back of the car as a kid with nothing to do but stare out the window.
As boring as it may have been, it was an important part of my development. And when I used to walk two miles to school every day, my brain wasn’t overflowing with stuff; it wasn’t hyped up with all these devices kids now stare at all the time.
How to Meditate
The most natural way to meditate is to just be with yourself.
There’s a lot of information about how to mediate and it can be intimidating because we’re raised to follow rules and we think if we can’t follow those rules, we can’t do it.
That’s your inner saboteur messing with you. All the meditation books, apps, and podcasts are useful tools, but you don’t necessarily need them to get started.
So keep it simple.
You’ve already looked at the sky and breathed for a moment. That was your first step. Make a point of doing something similar a few times during your day, whenever you can.
The next time you’re waiting in the checkout line, don’t open your phone. Instead, focus on your breath. Do this when you’re waiting for your kids after school, or even when you’re at a red light. Just focus on your breath.
Once you’ve mastered taking advantage of those small moments, consider setting aside some time to do it.
When you wake up tomorrow morning, just put your hands on your belly and breathe. It doesn’t have to be 15 minutes; just three to five minutes is fine.
From there, continue practicing in whatever way works best for you.
It can be as simple as setting aside time to sit in a chair, lie down, or go on a walk. Whichever one you do, remember to focus on your breathing.
It doesn’t have to be complicated — you don’t need to change anything about the way you breathe; just focus on it.
You don’t need to judge the way you breathe, you’re just paying attention to it.
The Benefits of Meditation
Meditation is a good way to help you not judge yourself in general.
We all have this “judge” in our head that attacks our behaviors — what we could/should/would have said or done.
And if you’re looking at your phone all day, if you’re constantly processing external information, you’ve eliminated your ability to process these internal thoughts, so they amp up.
But this judge, these destructive thoughts, they aren’t you. It’s not the stuff that’s at your core.
This is stuff was borrowed from living life, from years and years of family, other people around you, work, and school. You weren’t born criticizing yourself this way.
When these thoughts pile up in you, they can be destructive. They make you reach for that donut. They make you skip your workout. They make you angry at your friend. Again, remember, this stuff isn’t you!
The magic of meditation is that it can help you find a way to deal with these negative thoughts.
Once you’re comfortable with taking time to slow down and breathe, set aside a time to meditate and use that time to address your negative thoughts.
Don’t try to get rid of them; watch them in your head, like clouds. You don’t need to rope them back. You also don’t need to go along for the ride and follow these thoughts as they intensify.
Instead, let those thoughts move like clouds past you. If you get caught up in a negative thought, stop and go back to focusing on your breath.
The ability to watch those thoughts like clouds, to not get attached, and instead focus on your breath gives you a new spiritual muscle.
You become an observer, and you realize these thoughts are not you, so you can let them go. You’re no longer in the world of distraction and destruction.
You have a place somewhere inside you that’s calm. It doesn’t mean that you don’t care; it doesn’t mean you don’t feel. It just means you’re not reacting to those negative thoughts.
While a daily meditation ritual is wonderful, there’s something very important about applying this practice into your day-to-day life instead of just adding it to your to-do list.
These short moments help you break patterns. They throw a stick in the works.
There might be a couple of clouds that may have been bothering you for a while. These moments give them to a chance to pass over you a few times and then they’re gone.
This benefits both you and the people around you. If you’re about to have a shouting match with your son or daughter, take a few moments, breathe, and look back at your child.
You won’t scream. You won’t shout. You’ll talk it out. Your child will see you use this spiritual muscle and learn from you.
Just as it is with exercise and food, everyone has different needs when it comes to meditation.
But the very least, we all can benefit from looking up at the sky for a few minutes a few times a day and just breathing.
Beyond that, it’s not about what anyone else needs or says you need. The goal is to know exactly what you need and when you need to do it.

Design Challenge Judge Daniel Crowe: One Cool Diabetologist Living with Type 1
Please welcome a brand new judge this year in the 2011 DiabetesMine Design Challenge, Dr. Daniel Crowe. He is Medical Director of the Diabetes Program at Southboro Medical Group in Massachusetts, and a long-time type 1 himself.
As a diabetologist who wears his very own insulin pump and CGM, Dan brings a unique perspective to this contest and the prospects of innovative diabetes tools:
DM) First off, can you tell us what it's like to be a type 1 patient AND a diabetes doctor?
DC) Every day patients have the opportunity to learn that I too have diabetes, and they usually really sit back and say, "Wow, you understand!" That's almost always what they say.
We also have a team of diabetics. My nurse practitioner is a type 1 on a pump, and her nurse is, too!
So when we have patients who are new to us and not sure if they want a pump, we say, "Do you want to meet some pump patients?" Then the three of us come in the room, and it's really powerful.
What are your thoughts about being involved in this competition?
I keep switching hats, as I'm trying to understand it from a patient and a clinical world viewpoint.
As a patient, I'm very familiar with the daily frustrations with technologies not meeting the challenge. I'm excited to see some really innovative people coming up with things that will actually 'shift the paradigm.'
As a physician, the issue everyone's worried about as we get more and more data coming across our desktops is that at some point, we're going to get saturated. We need something else!
From the patient side, how could better-designed gadgets and programs potentially improve your own life?
If we judge design by how easy it is to use (level of complexity), functionality (does it interface well with current diabetes technology?), and reliability (accuracy, precision, does it break down often, is there good customer service?), then design is critically important, and a well-designed innovation would potentially have a significant impact.
We definitely agree that complex data logging alone is not the Holy Grail...
We doctors already are inundated with data that's presented in ways to try to "one-up" the competition but most still miss the mark. What's missing is information from the patient that allows much wiser interpretation.
For example, teleheath communication is being used for congestive heart patients, so cardiologists are getting loads of data too. There's a tremendous potential for overwhelming the system. It's all good stuff, but who's gonna look at it?
With diabetes you have so many data points. If you download a whole month of data from an insulin pump, it's overwhelming. If you then throw in patient-entered information like 'I forgot a shot,' 'I exercised for 2 hours,' — it's just so much!
Also, patients don't want to take the extra time to enter detailed diary-type information.
If data overload is the problem, then what characteristics do you think would make a "killer app" for diabetes care?
I like to think about different types of variation in terms of common cause — common things we do that can cause common variables.
"Diabetes Personal Calculator" - anyone tried it?
If someone can think about a way to create statistical analysis that allows the interpreter to determine if BG variation is due to "common cause variation" — (carb counting errors, changes in physical activity, stress, sleep variation, etc.) versus "special cause variation" (insulin was exposed to excesses of temperature, incorrect code on meter, expired strips, illness, vacation, etc.), that would be incredibly helpful.
To date, 'smart meters' and pumps still are not used adequately by enough patients to allow these day-to-day variations to be categorized in ways useful for better data interpretation.
So... an app that was really easy to use that allowed both glucose/ insulin/ medication/ carb intake data entry along with fun ways to prompt for things that might fall under either 'common cause' or 'special cause' variation which then would analyze and interpret the data based on these variations would be phenomenal!
It could look for patterns that would prompt questions like "did you forget your glipizide/Novolog?" or "check to make sure strip codes and expiration dates are OK?" or "you may be consuming too many carbs at meals," etc.
Is there any cool technology that you're using in your practice?
I'm awaiting arrival of my new iPad 2 — though that's not about the exam room chuckles But I was at a meeting at Brigham Women's Hospital where they're using it with patients while they're in the waiting room while waiting for a physician to come in. These are COPD patients, and they answer a questionnaire on the iPad that helps the doctor decide whether they need a certain kind of test. That saves a lot of time...
There are gonna be all kinds of ways to incorporate this technology into the clinic. In a few years, it'll be common practice for patients to use smart tablets of some sort during their wait time. This will be a very helpful way for clinicians to do initial screening.
But you know, the medical world is often one of the stragglers in terms of technology.
How do you define "success" for yourself and other patients living with diabetes?
Good clinical control (good, safe A1c plus low variability) plus a happy patient who feels they are in control (the diabetes doesn't control them) without breaking the patient's or the medical group's budget.
We're thrilled to have you as one of our this year. Again as both healthcare professional and patient yourself, what would you most like to see materialize out of this contest?
I think something that challenges the industry to change a paradigm would be great.
Thank you, Dan. We hope you know that doctors like you are game-changers too!
