chronic illness is such a joke I should be able to just pick a fucking medication and take it
I have been trying to get on mestinon for a year and a half to help with my me/cfs and pots and suspected gastroparesis. my cardiologist said I had to see a neurologist and he wasn't comfortable prescribing it
finally I see the neurologist today and she's like 'oh cardiologists usually prescribe this' and I'm like dear god please help me and fortunately she did but like
wtf
I hate it here

Just thought I’d put this out there on the internet in case any ME/CFSers had questions, what with Reddit going up in flames. I’m very fortunate to have been able to see Dr. David Systrom at Brigham & Women’s Hospital in Boston. I had the ICPET (invasive cardiopulmonary exercise test) done and was found to have both preload failure (a form of dysautonomia) and reduced systemic oxygen extraction (suspected to be due to mitochondrial dysfunction). If nothing else, it was an incredibly validating experience to finally get some abnormal test results. I’ve been on Mestinon for about two years now and it’s been somewhat helpful (not a miracle cure but definitely worth staying on). Feel free to ask me any questions at all about my experience with this doctor, having an ICPET, Mestinon or whatever else.

Personal post alert!
I am so fucking tired of the fact that any attempt to treat one of my chronic medical conditions makes the others worse. It’s like the worst game of whack-a-mole ever, because no matter what happens, not only does the mole pop up again, but I keep getting hit with a hammer!
For instance, my doctor wants me to increase my mestinon dose to treat POTS and help control my heart rate, improve brain fog and muscle activation, and reduce fatigue. It makes sense! It’s a miracle med for so many people!
But for me, I can only increase my dose by a 1/4 tablet at a time (anything more and I’m having utterly agonizing stomach cramps and dry heaving. Ugh), and even that is flaring my MCAS and hEDS. It is definitely increasing muscle activation, because it is causing all the muscles in my body to cramp, so everything hurts, but worse than that, they are cramping so forcefully that they are pulling bones and joints so hard they’re out of position, so now several vertebrae, ribs, fingers, carpal bones, and one shoulder and knee are partially dislocated, and holy shit does that hurt!
It also is currently increasing the brain fog and fatigue, but I’m also in enough pain that I can’t actually fall asleep if I try to nap. This is saying a lot, because I have narcolepsy, too, so I’m usually fighting to stay awake.
It does not help that my brain decided now would be a GREAT time to give me back trauma memories I had blocked out (gee, thanks, dissociative barriers. 🙃 Ya know, my writing meta about trauma was NOT ACTUALLY AN INVITATION to remind me how bad experiencing it is), so every day is an “adventure” because I don’t know if it’s going to be a “compartmentalize the shit out of everything and write 4k words” day or a “have all the feelings and everything is terrible” day, and the oscillations between the two and everything that goes with it is exhausting.
All this to say: uggggggghhhhhh.

Does anyone with ME/CFS, Fibro or POTS have experience taking Mestinon for fatigue specifically?  I only heard about this today after reading an article about poor oxygen extraction in ME/CFS. Pyridostigmine
Brand Names: Mestinon, Time span, Regenol
What is pyridostigmine?
Pyridostigmine affects chemicals in the body that are involved in the communication between nerve impulses and muscle movement.
Pyridostigmine is used to treat the symptoms of myasthenia gravis. It is also used in military personnel who have been exposed to nerve gas.
Pyridostigmine may also be used for purposes not listed in this medication guide.
Important Information:
You should not use pyridostigmine if you you have a bladder or bowel obstruction.
What happens if I overdose?
Seek emergency medical attention or call the Poison Help line at 1-800-222-1222.
Overdose symptoms may include nausea, vomiting, diarrhea, stomach cramps, sweating, blurred vision, drooling, and weak or shallow breathing.
Worsening muscle weakness, or no change in your myasthenia gravis symptoms, may also be signs of overdose.
What should I avoid while taking pyridostigmine?
This medication may cause blurred vision or impair your thinking or reactions. Be careful if you drive or do anything that requires you to be alert and able to see clearly.
Drinking alcohol can increase certain side effects of pyridostigmine.
Pyridostigmine side effects
Get emergency medical help if you have any of these signs of an allergic reaction:hives; difficulty breathing; swelling of your face, lips, tongue, or throat.
Stop using pyridostigmine and call your doctor at once if you have any of these serious side effects:
extreme muscle weakness;
loss of movement in any part of your body;
weak or shallow breathing;
slurred speech, vision problems; or
worsening or no improvement in your symptoms of myasthenia gravis.
Common side effects may include:
stomach pain, nausea, vomiting, diarrhea;
muscle cramps, twitching;
sweating, increased salivation;
cough with mucus;
rash; or
blurred vision.

Personal Notes: I get what I call "surges". It's a series of all over body muscle spasms. It's incredibly uncomfortable but had only happened when I take more mestinon than my body can handle at one time.

Day Two of Northera
I don't want to jinx anything but I was able to shower and clean the kitchen, take the garbage out and cook a quick easy meal. I felt tired but not exhausted. I was able to do each task without thinking about my breathing or heart rate. Of course I went overboard because I'm used to taking advantage of higher energy days. I had a small crash after my shower and felt it in my muscles. But not as much in my chest like I would normally feel. I go back on Mestinon tomorrow to try to help my muscles do what they're supposed to do I guess. Fingers crossed I don't get side effects again with this one. So far no noticeable side effects from Northera, knock on wood.

Myasthenia Gravis
Classic myasthenia gravis is an autoimmune condition in which the body mistakenly forms autoantibodies against postsynaptic acetylcholine receptors (AChR, 80-90%) or to muscle-specific receptor tyrosine kinase (MuSK, 10-20%). The antibodies block the receptors and stimulate cell-mediated destruction of the neurons. Proto-typical patient populations are young women (20-40 y/o) and older men (50-80 y/o). The primary symptom is weakness, beginning with small muscles, like the eyes and eyelids; then facial muscles for chewing or talking; later large muscles for posture and voluntary movements; and finally the diaphragm for breathing. 65% of patients with anti-AChR Abs have thymic hyperplasia, 10% have a thymoma, and many of the rest have some other autoimmune disorder. There are also congenital and ocular forms of MG. Diagnose with serum AChR antibody levels +/- EMG; treat with an acetylcholinesterase inhibitor like pyridostigmine (Mestinon) +/- immunosuppresants.
Pro-Tip: Patients with anti-MuSK Abs often present with weakness in neck extension, especially in the evening.

NIF is a score used to determine likelihood of a myasthenia gravis pt going into respiratory failure. Dr. Omar was telling me about it yesterday. He said the more negative the score is, the better. We have a pt with MG who has PNA. She was never treated for MG, so we gave her pyridostigmine (mestinon). Her NIF is -50, which is good. You want the score to be less than 20. If it is, it means it's unlikely that the pt will go into respiratory failure. It's basically a bedside PFT.
