The fiery depths
I recall the last time I yelled at a patient.
She was in labor, waiting to be admitted at the ER. It was 2:00 in the morning, in the bleak atmosphere, my eyes kept on shutting - my body was weak, my consciousness drifting away every 30 seconds. I wanted to sleep, like the resident who was only a few inches away from me. I wanted a little bit of rest, for the day had been long and exhausting.
I had to stay up and examine her since it was part of my job. 
She looked decent and was probably in her late 30s. I was expecting that nothing would go wrong.
But at the beginning of the interview, she acted like she was deaf. I had to repeat every question twice or thrice before she answered. The questions were simple and I could not understand why her hearing was poor. 
I was trying my best to stay awake, to attend to her, but I got so irritated during the interview. Every time I repeated each question, I felt like a balloon continuously filling up with water, with not much space left. Unsurprisingly that balloon burst. 
I ticked, I cracked, I burst. I yelled at her, asking if she was deaf. Both of us felt every fury in every word I laid down. I saw the terror in her eyes as she saw me transform from an innocent little kitten into a raging lioness, ready for an ambush. I couldn’t stop myself asking her why I had to repeat every question. 
Then the resident woke up.
All the rage began to shatter. He took over the interview as I waited for his admitting orders.
As I made an intravenous access and took blood samples, I couldn’t bear the shame and guilt I had in me. As the nurse took her to the delivery room, I did not even utter an apology.
After that duty, all I thought of was the backlash I gave to that patient. I only felt guilt, because I was not able to dig up a little compassion beneath the drowsiness. I lost myself just because of lost sleep.
In retrospect, I still have a lot of memories where I get furious with patients. I’m not quite sure if it’s the system, or the culture the led me here.
All I know is that I pray every night for kindness and compassion, which lacks all throughout the healthcare system in this country.
I try, I strive, in the hopes of not letting rage happen again.

History.
When I was in high school, history was my favorite subject.
Whether it was Philippine, Asian or World history, I read, studied and digested them like a baby eagerly feeding milk from her mother’s bossom. I loved learning about how Rizal was jailed for treason and sedition, how Pol Pot initiated a massacre in his regime and how the roman empire fell because of their own greed.
I loved every bit of it, how history turns, comes around and sometimes repeats itself.
But not as much as I first started doing clinical histories.
For the lay person, a clinical history in a nutshell is a patient’s database, rolled into a systematic, cohesive write up. It shows why the patient was admitted, the events that transpired before admission, previous illnesses, surgeries, medications taken, illnesses that run in the family, illnesses during childhood, and so on. There’s so much more about it but it gets too technical to explain.
History (in general) and a patient’s clinical history are two different things. Learning about history in high school was easy – you only needed to know the things that transpired, the things they did wrong and those they did right. The material is given right in front of us, all you need to know is there and all you need to do is read. But not for clinical histories - if you don’t know the patient had it, it’s because you didn’t ask it. Making clinical histories demand that we medical students become historians. Its not just about gulping the information as it was in high school history, because in this profession, you have to be the one probing, getting the smallest and distinct details, diving into the ocean of words the patient said and recovering the pearls – the essential and vital information you need so you can diagnose his/ her case. I thought I could do it, swim and dive to retrieve the pearls, but for most of it, I only sank into the dark and bottom corners of the ocean.
I would get lost and submit to awkward silence. Even if I finished asking questions, I would realize that I had not asked the more important ones, and this meant I had to go back and ask the patient. And. I. Hated. Not. Being. Efficient. I hated going back to the patient and bombarding him or her with my questions. It killed my nerves. It felt like I came too short, and that my effort was not even close to being enough.
I hated doing histories, because there would be always something missing from what I have collected. What I hated asking the most was their menstrual cycle interval, because those patients wouldn’t understand what I was trying to ask and hence not answer me right.                          
A little sooner, I realized that there was a way to becoming a good historian, and it meant being a good and effective communicator. Communicating effectively and confidently was key. I had to learn how to ask things clearly and give cues so patients can give the right answers. In asking when their menarche was, I had to ask whether they were in high school or in elementary when it happened. And just by cuing that, they would remember when it happened, and all I had to do was compute for their age at that time. And for the menstrual cycle, I started asking if their menses were regular, if it was monthly or not, I would show them a calendar and asked them to point the date – and so taking the interval came so relatively easy.
All I learned was that you can’t just take all that’s in front of you because you need more than that in making a 5 star history. This isn’t high school anymore, this is my dream becoming reality. You have to give a little more and even dive into the vast ocean, and beyond, if needed be. All it takes is determination and persistence.
