Gave myself almost-pneumonia and my couch looks like a damn stock photo.

Since the whole time I’ve been like, “Am I living one of my shitty fanfictions? Coz this sucks.” Let me tell y’all a story.

Cold weather sucks as a severe asthmatic. I moved to the South so I didn’t have to deal with the frigid winters of upstate NY. I’m basically on and off sick until Spring comes (and then there’s allergy season but I digress). I think it’s helped some, but my lungs are just fucked up ok?

Anyways I went to a concert last Saturday and it was freezing. Then I went to the zoo on Sunday with a friend and it was also cold and swarming with kids who don’t know how to cover their mouths when they cough. It was a great weekend but by Tuesday I was sick - great. I had some warning bc my friend I went to the zoo with said they got sick yesterday. But it just seemed like a minor cold and I’ve been through this a million times, I truly did not think it was gonna get too much worse. My asthma was mostly under control and I rested a lot all week.

Thursday I’m more tired, but I start nebulizer treatments and even skip ice skating class and reschedule it for Saturday bc hey, I’m responsible. But Friday I start to feel worse. Like to the point where everybody at work is like wtf go home and one of them told me she’s gonna get me holy water. But it’s okay, it’s still been SO much worse and I’m really fine.

Saturday morning I wake up and I feel like I’m cured. So I go to ice skating class. And maybe I take a little walk in the rain. Bad fuckin idea. By the end of the day I’m having full blown asthma attacks one after the other and sweating like crazy. My abdomen is aching from coughing so much that it hurts to sit up. But I really don’t want to go to the ER. Not again. So I message my pulmonologist and hope I can just say never mind I’m good now by the time he answers on Monday.

That brings us to today, Sunday. I woke up at 6am after only 4 hours of sleep because I can’t stop choking. I’m sneezing and coughing up fluorescent green stuff, my throat tastes like blood and I have a fever. I really, really didn’t want to go to the doctor but it’s time. I drag my sorry ass to urgent care where the entire hour I sit in waiting, everybody who walks by gives me a ‘goddamn’ look because I’m coughing loud enough to alert the entire damn office. I’m so embarrassed bc what if they think I’m being dramatic and wasting time - again? I awkwardly explain my situation and the doctor sends me for CXR. When it comes back he says “Well, you don’t have pneumonia yet but see alllll this stuff here? That’s inflammation. I’m gonna prescribe antibiotics and (way heavier) steroids and you might have bronchitis already but your asthma is so bad that it’s indistinguishable by now. Also with your lungs you probably won’t be able to tell you have pneumonia until it’s pretty bad.”

So anyways, that’s my week. At least I got a lot of writing done for Whumptober - didn’t have to dig very deep to find enough misery to go around to all my fav OCs lol.

Psittacosis

Let's open with a case report, like we're on an episode of house.

Case Report

35 yo otherwise well, suddenly presents with 2/52 of high fevers and a headache (usually this means > 39)

a/w chills and rigours, responsive to medication/presumably panadol and intermittent (would resolve then come back)

no respiratory symptoms

She had neutrophilia and intrestingly, a CRP of merely 30.

CXR revealed nonspecific consolidation in 2 lobes, they followed this up with a CT revealing pretty impressive ground glass opacities (or GGOs)

She was empirically treated on IV tazocin only (I'm used to atypical coverage empirically started if there's even a whiff of resp, which she may not have had symptoms but her CXR confirms this)

eventually she was on referred to the authors, who felt her CT findings with consistent with psittacosis and treated her with doxycycline which resolved her symptoms in 48 hrs

on further history, it was revealed that she had parrots at home, one had died 2 days preceding her symptoms and she was sleeping next to its body at night (crazy)

What is it:

psittacosis is a zoonoses (transmitted by animals, animals = reservoirs), in this case, transmitted by birds. Orthinoses if birds in general, but psittacosis if referred to macaws, parrots etc. YOu can also catch it from chickens and turkeys.

Some what related is Bird fancier's lungs. Which just sounds fancy.. I'm sure it's just an old term.

Bird fancier's lung refers to a hypersensitivty pneumonitis (ILD) caused by bird exposure. DIfferent disease process, but birds is the come denominator. INhaled bird particles

Psittacosis specifically refers to the infective disease process caused by a bacteria. It was 'identified" or reported in the 1870s, when a cluster of 7 swiss patients developed the same symptoms and found to have possessed tropical birds.

Similarly, in the 1930s there was an outbreak in the US with a mortality of up to 20% (80% in pregnant women), also attributed to parrots from South America.

Eventually, with further scientific development, the causative pathogen was identified as chlamydia psittaci, an atypical intracellular organism.

Psittacosis is a significant differential to consider in community acquired pneumonia as it has a high mortality if left untreated. But it is rare, and causes about 1% of cases in the US. Part of this is due to improved hygiene practices and strict importation guidelines of tropical birds.

It's spread through the inhalation of dust with either dried faeces or respiratory secretions from infected birds.

Clinical features

Variable! but the key thing on history is birds

incubation time can be anywhere from 2 days to 20

Flu-like (fevers/chills/myalgias/arthralgias/malaise/headache)

high fevers is key

respiratory symptoms - does not always present as per the case report, and can be mild on spectrum (dry cough) to more severe

if systemic, can also get photophobia, deafness and epistaxis

Rare (particularly where doxycycline or azith are prescribed at a low threshold): hepatosplenomegaly (look out for LFTs), GI symptoms (remember CAP can present with diarrhoea, nausea/vomiting --> always do a CXR)

even rarer: endocarditis or myocarditis, encephalitis or hepatitis (usually the complications of untreated disease)

Increased risk groups:

pet shop owners

bird owners

farmers

zoo, lab workers where birds are kept, vets, avian quarantine station workers

poultry handlers/workers

So ask if they live or work with birds, or had recent exposure.

INvestigations

serology is gold standard - so looking for antibodies in blood tests

it's intracellular - so hard to culture if even possible on standard blood cultures

elevated ESR/CRP may see LFT derangement and creatinine rise in systemic illness

CXR- usually lower lobe changes, if CT is done, you can get pulmonary infiltrates with GGOs

Treatment:

usual culprits for atypical coverage: azithromycin 3 days or doxycycline 100 mg BD for 14/7

Differentials

always broad if systemic features only (also consider IE and other causes of sepsis)

with resp symptoms - legionella, Q fever, mycoplasma, tularaemia (except for tularaemia, the rest are also covered by doxycycline)

In clinical practice, I'm so used to just having atypicals on board for any cases of atypical pneumonia. I really take it for granted. But will consider this differential more myself in cases of PUO - but I feel like there should be at least CXR findings regardless.

Anyway, prognosis is very good so long as it is treated.

Asumo que te refieres al análisis político que sigue el Comité por la Reconstitución/Línea Proletaria

Sobre el contenido político, me parece innecesario intentar crear una corriente "nueva" dentro del MCE o MCI a partir del desmantelamiento de la URSS. Por más devastador que fuese para el comunismo en Europa y el mundo, nuestra posición en el segundo escalón de la jerarquía imperialista estadounidense y la manera en la que se manifiesta el capitalismo sigue siendo parecida.

Seguimos en la etapa imperialista del capitalismo según la describió Lenin, sólo que más avanzada, y no me parece que sigue un análisis materialista de la historia decir que porque la clase obrera internacional haya sufrido una derrota haya que cambiar radicalmente el concepto de Partido Comunista (hasta el punto de pretender reconstituir). Dudo que ningún comunista protagonista del principio de lo que los recons* llaman el primer ciclo revolucionario pensase que el MCI sólo iba a conseguir victorias de ahí en adelante.

Además es hasta solipsista tener esta perspectiva tan cerrada de que la URSS = MCI. Sigue habiendo en todo el mundo estados proletarios, cada uno con sus fallos igual que tuvo la URSS durante los tiempos de Lenin y Stalin. Aparte de eso, el MC europeo está poco a poco superando la dominación del eurocomunismo y de la derrota del proyecto de la URSS, y están surgiendo en una buena parte de los países de Europa partidos marxistas-leninistas que tampoco ven necesario rehacer su línea política enteramente alrededor de la reconstitución.

No digo que no haya que reconsiderar el como se plantea el Partido Comunista de los tiempos de la URSS. Pero esto ya fue hace 33 años, y como buenos marxistas, hemos sido capaces de hacer tanto autocrítica como crítica a los errores que llevó al triunfo de la contra-revolución, sin que haga falta crear una nueva teoría para describir la marcha de la historia. No es más complicado que decir que la burguesía internacional le ganó el pulso al proletariado soviético, pero no se ha dado el caso en el resto del mundo.

*Digo recons para abreviar un poco, no como apodo despectivo aunque haya gente que lo use así

Sobre el CxR/Línea Proletaria, te puedo decir poco porque hacen poco. Creo que nunca les he visto en la vida real, y si les he visto se han fundido en mi memoria con el resto de partidos sin praxis que van a las manis a vender periódicos. Esto no es necesariamente una crítica a sus posiciones, pero sí lo sería si en vez de reconsiderar sus posiciones cuando no consiguen ni un ápice de praxis, se empeñan en tener la posición más correcta y avanzada, y esperan a que la clase obrera por si sola se de cuenta espontáneamente.

El rol de un partido de vanguardia no es actuar como si la revolución ya estuviese a la vuelta de la esquina en un ansia casi aceleracionista. Dado que el Partido sólo es capaz de dirigir a las masas una vez se encuentre entremezclado en el seno de la mayoría obrera, hasta que se da ese momento, siempre debe mantenerse un paso por delante de la conciencia de las masas. Ni debe quedarse estancado en la opinión mayoritaria, ni debe avanzar su praxis tan lejos como para, hablando en plata, parecer unos frikis que se creen que van a formar una célula armada en la Sierra de Guadarrama.

Espero haberme expresado bien y no parecer excesivamente hostil a los partidarios de la reconstitución, no es mi intención, pero a veces no me apetece hablar como un artículo académico.
