Why I  left Catholicism

I used to go to Catholic Church until I read the Bible and found out that nobody prayed to Mary, she was a sinner, she wasn't called holy or the Queen of Heaven, she didn't die a virgin, purgatory and lent are unbiblical, Popes sin too, We can confess our sins directly to God, we should only call God in Heaven Father, Jesus built His church on what Peter said about Jesus and not on Peter, scripture says Jesus Christ is the Rock not Peter, all Christians are saints because of Jesus, angels aren't saints, we're only supposed to pray to God not Mary since she isn't a comediatrix, Jesus is the only mediator between God and men, Christians are the church.

I'm starting a nephrology rotation today. Nephrology is not my strong suit, which is why I chose this rotation. I want to review renal anatomy and physiology and common renal diseases and their management. Today, I have a pt with most likely renal cell carcinoma. He has hypercalcemia (to 14.5), most likely 2/2 malignancy. CT abd/pelv showed a R renal mass with lymphadenopathy and mets with lytic bone lesions from T12 to L2.

PTH: parathyroid hormone; FHH: familial hypocalciuric hypercalcemia; PTHrp: parathyroid hormone-related peptide; SPEP: serum protein electrophoresis; UPEP: urine protein electrophoresis.

* Some medications associated with hypercalcemia include thiazide diuretics, lithium, teriparatide, abaloparatide, excessive vitamin A, and excessive theophylline. If possible, any medication or supplement that may be causing hypercalcemia should be discontinued.

¶ Serum PTH typically ranging from 35 to 65 pg/mL in an assay whose normal range is 10 to 60 pg/mL.

§ Additional work-up is warranted to identify malignancy.¥ Serum 25-hydroxyvitamin D must be markedly elevated before hypercalcemia develops. Although the serum concentration of 25-hydroxyvitamin D at which hypercalcemia typically occurs is undefined, many experts define vitamin D intoxication as a value >150 ng/mL (374 nmol/L).

So basically, you're getting PTH to determine whether the hypercalcemia could be do to increased PTH. The pt is on HCTZ, which can also increase your calcium levels. Having elevated vitamin D can cause hypercalcemia. Or if you have a paraneoplastic syndrome with PTHrp, then that can cause hypercalcemia.

Overview of approach – The diagnostic approach to hypercalcemia typically involves clinical evaluation and laboratory testing to distinguish between primary hyperparathyroidism and malignancy, which together account for greater than 90 percent of cases. The remaining 10 percent of patients with hypercalcemia may have one of many causes (table 1) that must be systematically considered and evaluated (algorithm 1). (See 'Introduction' above.)

●Confirm hypercalcemia – The first step in the evaluation of a patient with hypercalcemia is to verify with repeat measurement (total calcium corrected for albumin or ionized calcium) that there is a true increase in the serum calcium concentration. If available, previous values for serum calcium should also be reviewed.

●Determine the etiology

•Clinical assessment – Clinical evaluation, including duration of hypercalcemia, presence or absence of symptoms, family history, and medications, may help determine the etiology of hypercalcemia.

•Measure PTH – Measurement of intact parathyroid hormone (PTH) is important to distinguish PTH-mediated from non-PTH-mediated causes of hypercalcemia.

-Elevated PTH – A frankly elevated PTH concentration in the setting of hypercalcemia is highly likely the result of primary hyperparathyroidism (figure 1 and algorithm 1).

-Mid- to upper-normal or minimally elevated PTH – A PTH value that is minimally elevated or in the upper half of the normal range in the setting of hypercalcemia is likely the result of primary hyperparathyroidism. However, in this circumstance, the diagnosis of familial hypocalciuric hypercalcemia also should be considered.

-Low normal or low PTH – PTH concentrations below 20 pg/mL in the setting of hypercalcemia are usually not consistent with primary hyperparathyroidism and indicate the need for evaluation for other causes of hypercalcemia (table 1 and algorithm 1). This evaluation should include measurement of vitamin D metabolites and PTH-related protein (PTHrP) if no obvious malignancy is apparent. \

If the diagnosis is still not clear, other tests should be considered, including thyroid-stimulating hormone (TSH), serum protein electrophoresis (SPEP), urinary protein electrophoresis (UPEP), and vitamin A.
