Summary: Jo Karev is 41 weeks pregnant with her last baby. There is no way the baby is going to send her into labor the day before her C-section. But It wouldn't be a Karev baby if they came into the world without a little bit of drama.
Or the one where Jo is in labor, but denies every minute of it and baby Karev makes an unexpected entrance.

AN: I have been writing this for almost a year and I'm so excited to share this with you guys. I haven’t had the time or energy to write as I moved three weeks ago and I just got the last of my stuff unpacked. I also got a new job with a promotion so I’ve been working a lot more! But finishing this story feels like the closing of a book, and the start of a new sequel, it’s all been very bittersweet.
It's also the closing of the chapter for Jo and Alex. as they have their last baby and start their life as a family of three. But like anything the Karevs try to do, it doesn't exactly go as planned. 😉
For reference, Luna is 2 years old and Helena is 21 months old.

Jo Karev was not in labor.
“Yes you are,” Alex insisted as he argued with her.
They were both standing in the kitchen on a Friday afternoon. The August sun illuminated the room although they escaped the heat thanks to their hardworking air conditioner. The summer heat outside made everything seem a little bit lazier. It had been a quiet morning as their daughters had played inside. Jo had gone into the kitchen to start lunch and Alex had followed her to do it for her as he often did these days. She let him because after a while her body hurt too much for her to stay standing. However before she could even ask the girls what they wanted to eat, Alex had told her they needed to go to the hospital because she was in labor.
“No I'm not,” Jo argued back as she turned around and glared at him. “They're just Braxton Hicks contractions and they're not even consistent.”
She stood up a little straighter, even though at 41 weeks she was constantly in pain. She could barely walk with her huge bump and the baby in her pelvis. Jo was absolutely done being pregnant and had been since she hit 35 weeks. No one, not even Carina thought she would be pregnant this long. Especially, given that Helena had come a week early.
They had discussed inducing her when she made it to 40 weeks and given her heart condition they had a scheduled C-section for tomorrow. Jo was ready to have this baby and actually looking forward to her C-section, rather than having to push a giant Karev sized baby out of her. Given that this baby was currently in the 94th percentile, a vaginal delivery would not be easy.
“You just don't want to be admitted to the hospital,” Alex said as he crossed his arms and glared at her.
She couldn't argue with that one as she looked away from him.
“Jo for your sake and for the baby's sake we are going to the hospital now!” Alex said, putting his proverbial foot down.
However, Jo still argued with him. “I’m the OB resident. I’m the one who's pregnant. I know when I'm in labor and I'm not!”
“Says the woman who slept through her labor last time around,” Alex said, as he deadpanned and raised an eyebrow at her.
“Well I'm not asleep now,” Jo said, gesturing to her body and the fact that she was standing upright in the kitchen.
She didn't understand why Alex wanted to go to the hospital so soon. By the time they got to the hospital her contractions would still be several minutes apart and she would get sent back home or worse booked in overnight for observation. She was going to say as much, but then another Braxton Hicks contraction hit. Jo gripped the countertops as she gritted her teeth, trying her best not to let Alex know that she was in so much pain. But of course, he knew and checked his watch.
“They're coming at 15 minute intervals and have been for the past hour. Carina said that for the sake of your heart, you have to come in as soon as we even think that you're in labor.” Alex said, coming up to her and wrapped his arms around her waist.
She let herself be vulnerable in this moment and relaxed into his embrace. He held her belly, taking away the weight of their baby for a moment and giving her a moment of peace after the contraction ended. He leaned and kissed her cheek, his lips lingering on her skin.
“Jo, please, let's just go and get you checked out. I love you and I’m worried about you.”
She felt tears fill her eyes, she was emotionally and physically exhausted as the contractions had already taken a lot out of her. “I just don't want to be stuck in the hospital until this baby is born. I want to be at home with you and the girls.”
“I know,” Alex whispered softly, kissing her cheek again. “But let's be safe. The most important thing right now is that we go in and check your heart and check the baby. Who knows maybe she'll finally come on her own.”
“Or he,” Jo gently corrected him with a smile as she wiped away her tears. “We still don't know the gender.”
“Well, I think it's a girl,” Alex said as he rubbed her belly with a little smile. “Isn't that right, sweet girl?”
Jo put her hands over his on the baby. They didn’t kick, but Jo felt them move under her hand and she knew Alex felt it too. There wasn't much room for them anymore and they felt every little movement.
“Okay,” Jo finally agreed and she could tell Alex was smiling as he kissed her cheek, but they remained in the same position for a little while.
Jo closed her eyes, however, her calm moment was over too quickly as another contraction came ten minutes later. She let out a moan this time and leaned into Alex further as he whispered sweet nothing into her ear. He held her as the contraction finally passed and Jo let out a breath.
“Come on,” Alex said, finally letting her go although she still held onto the kitchen counter. “Let's go to the hospital.”
However, Luna appeared in the kitchen having wandered in from the living room, and signed to her. ‘I want lunch Mama.’
“Okay baby, Daddy's going to make you something for lunch,” Jo said, looking over at Alex who sighed at the delay, but went to the fridge.
‘No, I want Mama lunch,’ Luna insisted, referring to the mac and cheese that Jo would make for them.
It was mac and cheese and the cauliflower and broccoli she's secretly put into the cheese sauce so they would at least eat some vegetables. The girls called it Mama lunch and they only liked it when Jo made it. Whenever Alex tried to make it for the girls they always complained and said that it didn't taste right.
‘I know that you want Mama lunch,’ Alex signed as he knelt down next to Luna. ‘But Mommy has to go upstairs so Daddy has to make you lunch, but I'll make it just like Mommy does.’
‘No, mama lunch,’ Luna signed, letting out a whine as she kicked her feet and threw her arms around. Luna continued to sign Mama and cheese which was the sign she used for Mama lunch.
“Well, I'm sorry baby, but that's just how it is,” Alex said, getting up and ignoring Luna's tantrum as she carried on.
Jo sighed as she looked down at her toddler. She knew Luna was just hungry and tired as it was almost nap time, but she didn’t have the energy to deal with a tantrum right now. She looked over at Alex as he caught her eyes.
“I got it, Jo. Why don't you just go upstairs and get your bag together?” Alex said as he got the mac and cheese box from the cupboard.
“Once I go upstairs, I’m not coming back down until we leave,” Jo said, turning around to go upstairs. “Can you bring me a serving to eat before we go to the hospital?”
“Yeah, I’ll call Mer and she’ll be over in a few minutes,” Alex called up to her as Jo just waved back. “And put on the heart monitor!”
“Yes dear,” Jo called back as she walked away.
“I mean it Jo!” Alex yelled back at her.
As Jo went upstairs Alex got Luna settled with a string cheese stick. She now sat on the bar stool and patiently waited for him to make their lunch. She wouldn't let him cook mac and cheese but they had agreed on peanut butter sandwiches cut into dinosaurs, As he looked for the cookie cutters Alex pulled out his phone and called Meredith.

Jo slowly walked upstairs as she thought back to that morning, trying to figure out how many hours she had been in labor. She had woken up that morning around 7:00 in pain but that wasn't a contraction. It was just because the baby was sitting on her bladder and she had to pee so badly it hurt.
Jo quickly threw off the covers and rushed to the bathroom. She didn’t bother to be quiet as Alex was used to her morning pee rush. He always slept through everything. To her surprise, Jo had actually gotten a good sleep that night and only got up to use the restroom once.
After she used the restroom, Jo decided to be up for the rest of the morning. Lord knows the girls would be awake soon enough and she decided to enjoy the morning peace to herself. She went downstairs, got breakfast and coffee, before sitting down and turning on the TV, and pulling out her phone.
She scrolled through social media as she waited for her waffles in the toaster oven then slathered them in syrup and a little bit of peanut butter for the sake of her health. Curling up on the couch, she enjoyed her breakfast as she turned the TV on low, so as not to wake anybody up.
After she finished her waffles, Jo went back up to the nursery, something inside her just told her the nursery needed to be ready. She was in a crazy nesting mood these days and everything had to be organized, put away, and cleaned. Something Alex had dutifully helped her with. Although the nursery was perfect, they had completed it weeks ago. Still, Jo double-checked everything.
The diapers were in the diaper bin next to the changing table, wipes and everything were set up and the chair next to her nursing station was ready for her. She decided to check on Alex and the girls. It wasn't unusual for them to sleep in, and when she checked the baby monitor camera both of the girls were still asleep.
Luna was curled up on her bed while Helena was spread out on her bed. When she went back into the room, Alex was sleeping too. He had his arms outstretched into her empty space and although he probably hadn't woken up and seen that she was missing yet. She decided to enjoy the luxury of a shower to herself and went to the bathroom.
She washed her face, before hopping into the shower. It was so nice to have a shower to herself these days. If it wasn't Alex hopping in with her, it was the girls and they never ended up clean; she and Alex enjoyed some intimacy together, and with the girls, they just ended up splashing and playing.
Jo found herself rubbing off the remnants of a crayon drawing on the white tile as she let the warm water roll over her. Then she washed her hair, before she stared at herself in the reflection of the glass, putting a hand around her belly and holding her baby as they gave her a series of kicks. Jo found herself admiring her body and her belly more so these days as her due date came, and went.
They had agreed that this would be their last baby, her last pregnancy before they had conceived. Jo wanted to savor it as much as she wanted it to be over. She was huge at this point and her belly stuck out like she had a beach ball under her shirt. Her baby was about ready to burst out of her. Literally, as her skin stretched and she was constantly putting lotion in her belly to help as her skin stretched and ached. She traced the dark line that trailed down her belly with her finger, before holding her belly with both hands.
There wasn’t much room left in her uterus for them, however, when she took a breath she could feel the fullness of her ribs and knew that the baby had dropped lower. She could feel their back pressing out and their little feet just kicked above her belly button. They must have dropped into her pelvis when she was sleeping. Which was a surprise, and a bit of a displeasure as her lovely baby’s huge Karev head was now pushing down into her pelvis as they prepared for labor. If she pressed where she knew their feet were they would kick her hand and did so now as she smiled.
“Hey you, how are you doing today? Do you have any plans to come out or are you just going to chill until I evict you?”
The baby responded with a light kick and Jo smiled.
“Yeah, I wouldn't want to come out either. Especially as I know you hear your sisters screaming and your Dad being grumpy and me complaining at the time. I don't mean to complain, but being pregnant is hard. Especially after everything we've been through with my heart.”
The baby kicked her hands again as if to agree.
“Yeah, but I promise you, that you are so loved and so wanted. We all can't wait till you're here. Your dad and sisters kiss you in my belly all the time, but I can't kiss you yet, but I promise I'm gonna kiss you the first chance I get.”
Jo kissed her hand and placed it on her belly. Hopefully, she could kiss them soon. Then she closed her eyes and turned her head back under the water. Then she heard the door open and looked over as Luna came in.
‘Mama?’ Luna signed as she came in.
‘Good morning Luna, come here,’ Jo waved her over and Luna smiled as she ran over, pressing herself up against the glass of the shower.
Jo leaned down and cleared the fog from the glass. Luna pressed her lips against it and kissed the glass. Jo laughed as she opened the door and greeted her daughter.
‘I love you, little moon,’ Jo said as she wiped the fog off the glass before kissing Luna’s cheek.
‘Good morning.’ Luna signed as she yawned.
‘Are you hungry?’ Jo signed to her, but Luna shook her head.
‘Can I get in the water?’ Luna signed to her as she reached for the zipper on her pajamas, telling Jo her intentions and not asking for permission.
“Alright, baby,’ Jo said as she carefully squatted down to help Luna unzip her Bluey pajamas.
Jo grabbed the shower head and sat down on the wooden bench. She brought it down to hold it at Luna’s shoulders so she could stand under the water. Luna giggled as she danced and played before she climbed up next to Jo for a quick snuggle. Jo snuggled her baby close and relished having Luna in her arms. They sat together under the water as Luna told her about her dream and Jo watched her sign with earnest.
Soon enough, Luna got out the toys and grabbed a boat. She started to push it under the water as she made the little boat go up the wall and moved it around. Jo figured it would be a good time to wash Luna’s hair and grabbed the shampoo. After they were both clean, she let Luna play as the warm water as the steam soothed her muscles. She rubbed her belly, but the baby was quiet and seemed as content as she was.
‘Mama, I hungry,’ Luna signed to her as she dropped her boat and came over to her. ‘Daddy make pancakes?’
‘Yeah baby, let’s go wake him up.’ Jo signed with a smile as she shut off the water.
She grabbed her towel and wrapped Luna in her little ducky towel. She had Luna sit on her toddler toilet as they were trying to potty train her. She was showing all the signs of being ready to potty train, but they only had a few successes so far.
After they dried off, they went back to the bedroom. Alex was lying on his side, turned into the bed, and curled around Helena. Helena must have also woken up, but instead of coming into the shower with them, she climbed into bed with Alex. The girls definitely had their preferred parents, although Jo knew Helena loved her as Luna loved Alex too.
Luna ran around to Alex and climbed up on top of him, patting him and trying to get him up as she chose to talk. ‘Dada!’
‘Good morning, little moon,’ Alex groaned as he woke up and wrapped his arms around Luna as Jo grabbed some clothes.
‘Daddy, pancakes?’ Luna signed to him with a smile.
‘Yes, I’ll make pancakes,’ Alex signed as he sat up. ‘But you need a diaper and some clothes first.’
‘Okay, I get clothes,’ Luna signed before she ran off to her room.
“Your clothes can be optional though,” Alex said to her as she looked up to see a familiar smirk on his face.
Jo laughed as she picked out her favorite sundress. “I love that you still find me attractive, let's wait till nap time.”
After she got dressed, Jo did give him a good morning kiss, although she pulled back wrinkling her nose at his morning breath. She should have gotten used to it by this point, but he had a particularly stinky breath. He laughed as he covered his mouth and Jo shook her head.
“I didn't hear you get up this morning,” Alex said, as he sat up and looked up at her with a lazy but happy smile.
“Yeah, I wanted some alone time,” Jo said as she smoothed the fabric that bunched around her belly. “I had breakfast and everything.”
“Oh, so I suppose you don't want pancakes then.”
“No, I definitely want some.”
Helena finally woke up as she rolled over and sat up. She rubbed her eyes and crawled over to them. Alex pulled her into his lap as she laid her head on his shoulder. “Hey baby girl, do you want pancakes?”
“Yeah, pancake,” Helena said around her binky as she was suddenly more awake and crawled off the bed.
“Alright let's go make some pancakes,” Alex said as he grabbed Jo around her waist and kissed her cheek as she smiled.
The rest of the morning was lazy and quiet, the girls ate and watched TV as she and Alex relaxed. It was set to be a quiet summer day, until now.

In the Middle of the Night

Summary: Alex comes home grief stricken in the middle of the night and wants to hold his wife and daughters as tightly as he can.

It was late when Alex got home. The only one to greet him at the door was Reeses. The pup was a shadow in the dark house, but his eyes glimmered with excitement. Recess’ collar jingled as he wiggled his but. Alex dropped his bags with a sigh, but smiled at the warm greeting. He kneel down and buried his hands in Reeses’ ears, giving him a good heads scratch.
“Hey buddy, I’m happy to see you too,” Alex said, letting himself smile as Reeses panted happily. “I'm guessing our girls are asleep, yeah?”
Reeses didn't respond but when Alex gave him one last pet, he quickly ran back upstairs to the girls. Alex hesitated to follow him as he picked up his bag again and put it on the seat in the mudroom. He could unload it tomorrow as he had the day off. He did grab his phone though and put it on the bench as he changed. He stripped off his clothes, like they had done since covid and changed into the fresh pair of pajamas Jo had left in the dryer for him. He pulled on the shirt and breathed in the little bit of her scent that had mixed in with it. Yet, it was a pale comparison to the real thing.
Alex put his phone in his pocket as he went to the kitchen. There wasn't much left of dinner and he had eaten at the hospital. Still he still grabbed a few bites of the chicken Jo had made. Then he went in search of his girls as he walked upstairs. Alex needed them tonight.
He stopped by the nursery first as he heard their sound machine going and the star lights twinkling on the ceiling. Both of the girls were in Luna's bed. They always ended up sharing the same bed. He didn't know why they had separate beds on different sides of the room, but they did occasionally nap separately. Alex fell to his knees in front of them and carefully wrapped his arms around them. He needed to hold them in his arms, but he tried not to disturb their sleep. He kissed both of their heads and closed his eyes as he held them.
They were alive and breathing and they were happy and healthy. But the sound of the crying children still rang in his ears.
He lingered in the nursery holding them, watching them sleep, and thanking god that they were alive and that they were his to come home too. He held them both and traced the slope of Luna’s nose and Helean’s large forehead. They were so little, still had their baby features. He wanted them to stay like this forever.
He looked up when he heard Reeses’ whine at the door. He gave the girls one last little squeeze, kissing both of their foreheads before tucking the blanket around them. They will be safe sleeping here tonight. He needed his wife now.
Alex sneaked out of the nursery and gave the girls one last look before he closed the door. Reeses whined at the door to their bedroom as Alex quietly opened the food to the master bedroom. Recess trotted in and jumped up on the bed, turning around a few times before he settled next to her. Jo was asleep on his side of the bed, curled up, but relaxed as she snored.
He instantly smiled as he saw her and let out a sigh of relief. Then he crawled up into the bed and carefully tried to hold her as he wrapped his arms around her. He wanted to hold her; he needed to. Alex was desperate to have her in his arms again and to know that she was his. That his wife was alive and well.
Then he heard the screams of a mother who will never hold her child again and he thought about Jo.
He hoped he would never hear her make that sound. He would do anything to make sure of that. Jo woke up as he squeezed her, perhaps too tightly, she stopped snoring and looked up at him.
“Alex what happened?” Jo whispered as she turned in his arms and he pulled her into his chest, but she wrapped her arms around him as well and let him hold her as tightly as he needed to.
“Who did you lose?” Jo asked, although she doesn't need to know the answer.
Alex closed his eyes, trying not to picture it in his mind. When he opened them, he saw his wife in front of him. He traces the details on her cheek, her nose and her lips, pink and full. He signed as he heard the echo of the screaming mother.
“A 16-month-old baby. It was a car crash. They came in and we tried everything, but, but… The mother was in the room with us. She wouldn't let go of her baby. I could hear her…” Alex trailed off, his voice breaking a little bit as he shared the grief with her.
“Helena is 16 months old,” Jo whispered, pulling him in again and letting him hug her.
“I just kept thinking about her and Luna, then before we could do anything, the mom collapsed and she died too. She had a pulmonary embolism, but it seems like her heart just gave out, as if losing her baby was just….”
“I'm here,” Jo said, pulling back, holding his face in both of her hands as she looked into his eyes. “I'm here, I'm alive, I'm safe, and so are the girls.”
“I know,” Alex whispered, believing it, but his heart still broke for his patient.
He pulled her into his arms again, he just needed to hold her. They laid back down in the bed, but he sat up, with his back against the headboard and pulled her into his arms. Jo laid her head on his chest and stared up at him for a while. She traced gentle patterns in his chest and it helped him focus on her. But she grew tired and closed her eyes again as she yawned.
“Go back to sleep, Jo.”
“What about you? You should get some rest too.”
“I don't think I can right now.”
Jo just nodded, pulling him in to kiss her for the first time as he lingers on her warm lips. He wouldn't sleep for a while, he couldn't until the memories had faded away. Jo fell asleep easily, especially in his arms. She drifted off with a little smile as he held her. Alex stared at his wife, looking at all of the little details of her face. The baby hairs that fell on her pink cheeks and her eyelashes that fluttered as she dreamed. Her lips that open and shut with each snore, and her large forehead that Helena had inherited.
A few minutes later a little voice calls to him. “Daddy?”
Alex hadn’t heard them come in, as he was so focused on Jo. But Luna stood next to him at their bedside, clutching her little white bear close to her chest. Helena was next to her, half asleep but holding her sister's hand as she sucked on her pacifier, she was probably there for emotional support.
“What's wrong baby?” Alex asked he reached out to her with his free hand he's laying down next to Jo now and he backed them into the pile.
“I wanna hold you,” Luna said, as she crawled up on the bed, pulling Helena with her.
“Okay, baby come here,” ALex said as he moved to lay down with Jo next to him.
Helena crawled in next to Jo and tucked herself against her mama. She’s out like a light the second she laid down and Jo sleeps through the whole thing. Luna crawls over Jo and he pickles her up and holds her against his chest as Luna hugs him.
“I'm right here little moon, daddy's got you,” Alex promised her as he gave her a little squeeze and kissed her forehead, reassuring her that she was safe in his arms.
Alex holds his family tight. He's got one arm around Jo, his hand holding Helena's hand, with his other arm around Luna. Even Reeses joins in and moves to curl up on his legs. His girls sleep peacefully with Alex holding them. He was reminded of when he held the girls when they were babies, they loved contact naps and he would hold them all night.
He stared at his girls, until the Sun started to come up and its golden rays came in through the window. Alex closed his eyes as sleep finally started to overtake him. When he does, he sees his wife and daughter’s faces in his mind as he falls asleep and Alex knows he'll be okay.
