Self-Doubt and Insecurity

Her love for Ichigo is evident in her actions, such as her unwavering support for him during battles and her willingness to sacrifice herself for his sake.
She might've feared that confessing her feelings to Ichigo could jeopardize their friendship or lead to rejection. This fear of rejection could stem from her past experiences with loss and abandonment, causing her to hesitate to take the risk of confessing her feelings openly.
