Tony Stark, the mafia boss, but right now his title is "horrible patient". The boss is recovering from a gunshot wound, and over the next few weeks, Peter will become closer and closer with the mafia family. Will he stay, or will the family's methods scare him away?

Silence
Kamar Taj.
Tony stared at the building in front of him. It looked entirely innocuous. Not like the place someone might run to if they were searching for miracle healing. But Tony had traced Stephen’s steps. He had found Johnathon Pangborn and his miracle healing. Had followed Stephen’s steps to Nepal. Had wandered the streets of Kathmandu. Had asked stranger after stranger until one had pointed him here.
Perhaps this wasn’t Kamar Taj at all. Perhaps this was nothing.
Maybe it was, though. Maybe Stephen lingered somewhere within, still as far out of reach as he’d been when Tony had walked into their apartment to find Stephen gone without so much as a note.
God, silence spoke volumes.
Tony stared at the door. Grief twisted through him as he realized that he wasn’t going to knock. That he had never been going to.
Say what people would about him, but he could take a hint.
Silence spoke volumes.
He chose to ignore it, sometimes. But he could still read a room with the best of them.
Stephen had left. Stephen had chosen this unknown miracle over Tony and their life. Stephen hadn’t said a word.
The silence spoke volumes.
A woman watched him from across the street.
Tony met her gaze, read in her eyes the weight of someone who saw too much. Was she from Kamar Taj, he wondered. Waiting for him to choose to knock or to walk away. Could she guess what had brought him here?
Would she let him in, if he asked?
For a long moment they just stared at each other.
He looked away first, back at the door that hid Stephen behind it. He could knock. Could try to find Stephen. Could try to get answers. Could try to heal this chasm that had opened between them.
But god, the silence spoke volumes.
Because Stephen didn’t want to be found. Stephen didn’t want to let Tony in. Stephen didn’t want to heal the gaping wound between them. Stephen didn’t want… didn’t want Tony.
Stephen had chosen.
Tony… Tony loved Stephen, loved him with a devotion that ached. He’d always feared that Stephen didn’t love him back the same way.
It turned out Tony had been right.
He stared at the door for one moment longer, just hoping. But there was nothing. 
Grief was rope around his neck, tightening and stealing his ability to breathe. He closed his eyes, blocked out the sight of the door that taunted him with the possibility that Stephen stood just on the other side.
It didn’t matter either way, though. Because he and Stephen…
Stephen had chosen.
Tony didn’t think he’d ever stood a chance at being the one Stephen chose.
He turned away. 
He never saw the door to Kamar Taj open.
The silence spoke volumes.

The picture is a work in-progress but I think it's cute so far. I'm just posting a bunch of work on here to get my stuff out there. So, don't mind me. Lol.

my imagination tells me two options:
They fought against the enemy, Tony almost died and Stephen rushed to him, kissing the one he almost lost...
and angst, in which Tony and Stephen fight against each other, desperately, cruelly, but when Stark falls from a height to the ground, Strange rushes to him, suddenly realizing that he does not want him to die and he needs him alive.

Only When I Sleep (the full fic!)
TW: Gun violence, Idiots In Love
“Did either of you put poison in my food?”
“What? Of course not!” Tony cast Stephen a doubtful look. “I didn’t.”
“Don’t be an idiot, Stark,” Stephen growled. He turned his attention back to Loki. “More importantly, why do you think someone’s poisoned you?”
“It tastes a bit strange…”
Tony grabbed a clean fork from the cutlery drawer. Before Stephen could stop him, he scooped a mound of cauliflower rice and shovelled it into his mouth.
“Tony!” Stephen yelled.
Tony chewed thoughtfully. “Tastes fine to me.”
“Just give it a minute or two, it will come,” Loki said ominously.
“Are you crazy? Do you have a death wish or something?” Stephen berated.
“If it was really poisoned, there really isn’t any point in living anymore, is there?” Tony lamented. “A life without Loki is a tragedy too terrible to contemplate.”
“Tony, you idiot.” Loki’s eyes shone. “I love you.”
“What about me?” Stephen asked, almost fearfully. “Do you love me too?”
“Out of necessity, I suppose I must,” Loki sniffed. “You know CPR.”

"I don't think Loki loves me very much."
"Don't be ridiculous," Tony said mildly. "Of course he does."
"He loves the things that I can do, sure," Stephen said glumly. "Like making the best eggs Benedict on this side of Manhattan. Or turning cheap wine into the best vintage. Or getting him authentic momo dumplings from Nepal - "
Tony sneaked an amused glance at his morose lover over his Starkpad. "I never realised that your achievements were all food-related."
"Can you be serious for once? I'm in the middle of a crisis here!" Stephen glowered. "And that cauliflower rice recipe had five stars on Good Eats!"
Tony sighed. "Look, Doc. Loki's not a big fan of our planet, as you very well know. The fact that he's staying here, with us? He must love you a little."
Stephen snorted. "That's not saying much."
"What's that supposed to mean?"
"Well, he's obviously staying for you," Stephen pointed out. "I'm…baggage."
"Baggage," Tony said flatly. He snapped his fingers. "Hey. Levi. Get over here."
The Cloak of Levitation wafted over from where it had been hovering by the windows for the past few hours people-watching.
Tony pointed at Stephen. "Can you knock some sense into your Master here for me?"
The Cloak gave Stephen a few sympathetic pats on the cheek, before returning to its station and favourite pastime.
"See?" Stephen cocked his head unhappily. "Even Levi agrees with me."
"Have you ever heard of 'opposites attract'? Loki's a closed book, and you…" Tony waved a hand, making vague gestures that alluded to something Stephen could only guess the meaning of. "You gotta open up! Tell him you love him in public! Shout it from the rooftop!"
Stephen blinked. "But we live on the hundredth and thirtieth floor. No one's gonna hear it."
Tony rolled his eyes. "That is not the point, you dumbass."
"Then what is your point?" Stephen asked, exasperated.
"Put aside your ego and show him some love! And I don't mean the tough kind."
"But I only know just the one kind," Stephen grumbled, blushing a little.
Tony stared. "You're hopeless."
"I knooow…" Stephen moaned, listlessly dropping his chin onto the table. "I'm screwed."
"Hmm, what to do, what to do…" Tony drummed his fingers on his desk. "I've got it. Remember that scene in The Sixth Sense? When the 'I See Dead People' kid told Bruce Willis how to get through to his wife?"
"What about it?"
"Say it when he's asleep," Tony said excitedly. "Tell him you love him, and to please love you back."
"But that's ridi - " Stephen paused. "Actually, that's kind of brilliant."
Tony gave a modest shrug. "Well, I am known for my genius."
Stephen snorted. "Thanks, Mr. M. Night Shyamalan."

"That was a stupid plan," Stephen growled as he emerged from their bedroom the next morning.
Get out of my head, a half-asleep Loki had hollered at the top of his lungs in the middle of the night, waking the entire household in the process. Stephen, being the closest to him in terms of physical distance, had received the brunt of Loki's wrath, which explained his brand-new shiner.
"Serves you right for being a creep," a fully-awake Loki glowered. "And you!" Tony cowered under the intensity of his glare, "Enable him further and we shall see how you fare."
With that parting warning, Loki stormed away, presumably to make himself a hangry sandwich before they were due to join the Avengers in battle in a few minutes, something Tony had neglected to mention to either of his husbands.
"So…planting subliminal messages is a no," Tony said sheepishly. "We should have thought about that, huh?"
Stephen heaved a sigh of despair, and hung his head. "I'm screwed."

"We're screwed," Stephen announced an hour later; the enemy had utilised an impressive new weapon, a magic dampener that had basically rendered him powerless. "Hand-to-hand combat is not my forte!"
"Hang on," Tony grunted through the communicator. "I'm coming!"
Stephen frantically looked around for his other husband, and his blood ran cold.
A sniper on the roof had Loki in his sight, right in the line of fire, magicless and as helpless as a kitten.
And Stephen ran.
"Loki!" he shouted, "Look out!"

Stephen awakened to a startling brightness and his whole body hurting with an unidentifiable pain. "Urgh."
"You fool."
Stephen saw the tears in Tony's eyes and his heart began to pound. "Tony?"
"You brave, crazy fool."
"What's wrong? Is Loki okay?" He asked anxiously. "Is that why you're crying? Did he get shot?"
"No, but you did. You pushed Loki out of the way, and got hit instead."
"What?" Then Stephen remembered the barrage of bullets hitting his chest. "How am I not dead?"
"You were bleeding to death on the damn battlefield, right in front of my eyes," Tony said, his voice stricken.
"Tony…" Stephen reached for Tony's hand. It was shaking terribly. "Tony, I'm alright."
"I nearly lost you. I nearly lost you both."
Stephen stilled. "Loki?"
"He's resting." Tony nodded at the other bed across the room. "He nearly drained every drop of magic but it worked. He got you back."
"Help me up," Stephen mumbled. "I need to tell him something."
"Can't this wait? He's still sleeping, and you should be sleeping too - "
"No. It has to be now."
With herculean effort, Stephen dragged himself to Loki's bed. He climbed in beside his slumbering husband.
One look at Loki's exhausted face and the memories came rushing back: the pain, the geyser of blood spurting from his torso -
Damn you, Stephen!
- the sensation of Loki's healing magic pouring into him, dousing the agony in his chest like ice water.
But most of all, he remembered Loki's desperate whispers, I love you. I love you. I love you.
"I love you too, Loki," Stephen whispered in Loki's ear, hugging him tight.
Loki sighed in his sleep. A tell-tale smile began to tug at the corners of his lips. His hands palmed the silk sheets as if looking for something.
"You too, Tony," Stephen ordered. "Get in here."
Grabbing one of Loki's flailing hands, Stephen watched as Tony clambered up onto the bed and grabbed the other.
As they cuddled each other,
"Should we just order in breakfast?"
"For the rest of our lives, yeah."

Ironstrange identity reveal? I was reading Spy x Family when I thought of this, so that's the background/setting I was thinking of initially, but it can definitely be a superhero thing too. Or whatever other idea pops into your head. I'm always excited to see what you come up with!
I read the top of the wikipedia entry for Spy x Family, but that setup is not working for me here, so instead we’re going with some classic “Iron Man is Tony Stark’s bodyguard” shenanigans. 😀
Uh, this got… more than a little out of hand. This has many themes in common with other drabbles, but now it’s 1360 words long. Enjoy?

Tony stares stubbornly at Steve, who is rubbing his eyes with the kind of exhausted exasperation that has become his hallmark around Iron Man.
“Iron Man, you were unconscious,” Steve says. “After being electrocuted. You need a real medical check.”
“The armor has medical sensors,” Tony insists. “It says I’m fine. I can answer questions for a neurological exam. That’ll have to be good enough.”
Steve gets that stubborn look on his face. The one that no one wins against. Fuck. “You’re benched until you get a real medical check.”
“How about if I get checked out in my civilian identity?” Tony suggests.
“We have no way of verifying that you’ve done that,” Steve says. “And while I hate to say you’d lie, I can’t dismiss the possibility after this conversation. I understand your hesitation about revealing your identity, but your life is at stake.”
“It’s really not,” Tony mutters. He’s fine. But he can tell that Steve isn’t going to budge on this, and he wants to be off the bench sooner rather than later. “Fine. Call Strange, then.”
Steve’s eyebrows go up. “Doctor Strange? He’s not a practicing physician.”
“He’s kept his certifications, though.”
“And he hates you.”
Stephen hates Iron Man. He’s quite fond of Tony Stark, though. They’re… friends. He’s asked Tony on half a dozen dates, which Tony has been forced to turn down because he refuses to lie about something as important as Iron Man to someone he’s dating. It’s made for a lonely couple of years. At least if he has to reveal his identity to someone, he might get a date out of it. If Stephen forgives him for lying. If he can get past Tony Stark being an Avenger in general (apparently they make a lot of work for the sorcerers; something about the barriers between dimensions) and Iron Man in specific, who he’s always hated the most. 
“Then you’ll know he’s being honest when he clears me,” is all Tony says aloud.
Steve still looks baffled, but he makes the call.
Tony’s expecting Stephen to be angry when he arrives. He may keep his medical certifications up to date, but he doesn’t work as a doctor and he’s sure as hell not on call for the Avengers. But when he arrives he’s crisply professional, if frosty. He’s even wearing a lab coat over street clothes. He locks the exam room door behind him, which would be odd except that he follows it up with, “Armor off.” Tony hesitates. Stephen’s expression tightens. “Captain Rogers indicated you were willing to be examined,” he says sharply.
“This is kind of a big deal,” Tony snaps. “There is literally no one living that knows who I am.”
Stephen blinks. “Surely Tony knows.”
Tony sighs. “Yeah. About that.” He issues the command, and the helmet retracts. 
Stephen stares. And stares. And there it is. There’s the anger. “You utter fucking idiot!” He shouts. “What the hell are you doing risking your life in that goddamned tin can?! Don’t you know what a catastrophic loss it would be if you died out there?”
Okay, Stephen is yelling, but it seems like… good yelling? “I’m saving people,” Tony argues.
“You save plenty of people as Tony Stark!” Thank God the soundproofing in this place is spectacular; Stephen doesn’t lower his voice one bit. “So why do you insist on ruining your own life with this, this,” he waves his hand inarticulately at the armor still covering Tony to the neck, “bullshit.”
Okay, that’s enough. “Iron Man is not ruining my life! It’s the best thing I’ve ever done!”
Stephen's expression shifts from furious to incredulous. “The best thing?” he demands. “The best thing? What about the 100 million dollar donation to spinal cord research? The Foundation that helps people rebuild after things like alien invasions when their insurance won’t? What about the shield you invented that makes it possible for people with sensitive implants to get an MRI safely, or the modular smartphone that doesn’t need to be replaced every two years? What about the scholarships you’ve endowed?”
“Stephen—”
“Are those not big enough in scale?” Stephen demands. “How about the clean energy technology that looks like it might stop global warming in its tracks? Is that enough? Is one planet not enough? Do we need to talk about the international collaboration that you started to combine Earth’s resources with alien technology to turn us into an interstellar civilization before another interstellar civilization can dismiss us as barbarians—”
“I’m not the one who decided none of that counted!” Tony shouted over Stephen. It stops the tirade, at least. Tony blows out a hard breath. “The guy who donates the 100 million dollars isn’t the hero,” he goes on. “That’s the guy who makes the discovery. The Foundation isn’t celebrated for the people it helps, it’s critiqued for the ones it doesn’t. The MRI shield, the smartphone… people like them, but they’re advances that ‘were always coming’. Or maybe ‘were overdue’. That doesn’t mean they aren’t worth doing. Of course they’re worth doing.” Tony retracts a gauntlet and rubs a hand over his face. God, he’s tired. “I just… I wanted to be the hero for once,” he admits. It sounds so fucking selfish. “Tony Stark could never do enough. It always had to be about the money for them, even when it really wasn’t about the money for me. But Iron Man… No one wonders what his ulterior motive is when he does something good. They just cheer.”
Stephen sighs and hitches himself up onto the medical bed next to Tony. “Do you know why I hated Iron Man so much?”
Tony grimaces. “I always figured he was just too… blunt instrument for you.”
Stephen snorts. “Hulk and Cap are far more blunt,” he says. “Iron Man at least has precision weapons to go with the punches. No. I hated Iron Man because when the suit failed somehow Tony got the blame, but when it worked, Iron Man got the credit. Iron Man made everything you just explained worse, not better.”
There’s not really a counter argument for that. It’s true. Except, “Nothing was going to make that better,” Tony says. “Nothing could. Not until I’m dead, anyway.” He half expects Stephen to go right back to haranguing him for risking his life. 
“Speaking of which, I’m meant to be making sure you’re not dying now,” Stephen says. He stands and moves to face Tony. “Come on. Get the rest of the armor off.”
Tony obliges, and they proceed with the rest of the exam in silence. 
Near the end, Tony looks over Stephen’s shoulder rather than meet his eyes and says, quietly, “I’m not going to stop.”
“My initial reaction aside,” Stephen answers, just as soft, “I never thought you would. I know as well as anyone how this life becomes a part of you.” When he finishes the exam, he steps back. “You’re fine.” 
Tony nods and puts the armor back on. It’s never been so quiet between them before. His chest aches.
When he’s got everything but the helmet back on, Stephen stops him. “Is this why you always said no when I asked you out?”
“Yeah,” Tony says. “Didn’t seem right to lie like that to my partner.”
“Right.” Stephen nods firmly. “So will you go out with me now?”
Tony perks up. “You still want to?”
Stephen gives him a look. “You have not had a personality transplant,” he says dryly. “So yes, I still want to.”
Do not push your luck, Tony tells himself. He says it anyway, “Thought the lying might be a bigger deal.”
“Everyone keeps secrets,” Stephen says. “But the secrets that a friend will accept are different from the secrets that a partner will accept. You’ve already demonstrated that you understand that.” Tony grins and Stephen shoots him a look. “We will, however, be having words about you assuming I’d react like anyone else in your life.”
Tony swallows a laugh. “Of course,” he says. “I should have known you’d be exceptional.” 
“And don’t forget it,” Stephen says, a smirk tugging at the edge of his mouth.

I still love the fact that the entire Ironstrange community decided that Stephen fell in love with Tony after viewing 14000605 possible timelines bc he watched him die for everyone on Titan, watched him sacrifice himself for people he just met and essentially for the entire universe, watched him fall over and over and over again just to stand up and keep fighting because he had to. He’s seen every decision Tony could ever make, has seen every sacrifice-play he’s made, every victory and defeat, and decided ‘this one.’
Like, Stephen’s seen this man at his worst and at his best, probably knows him better than anyone else ever could, and he fell so in love with him. I will go down with this ship.

Just a Kick
“Well,” Tony said, trying not to let his amusement get the best of him. “Say what you will about Peter, but subtle he is not. I think he wants us to date.”
Stephen glanced at him, arching an eyebrow. “What gave it away?” He asked, tone dry. “The mistletoe a few weeks ago that was completely out of season? The three different staged dinners that, admittedly, took us two dinners too long to figure out? The time they locked us in a closet after having stolen my sling ring? The fact that they’ve now tried to shove us into a single room with only one bed despite the fact that I could literally portal us to our own rooms for the night without anyone the wiser?”
“We could do that,” Tony agreed, even though Peter had made them promise not to abuse portals. Apparently it was part of the ‘vacation experience’ and it was cheating if they just went home every night. Tony didn’t get it, but he’d gone along with it this far. He let out a little hum as he considered the situation. “Or, I suppose, we could just go along with it.”
“You kick,” Stephen muttered under his breath. “It is incredibly annoying.”
Tony had gotten used to Stephen knowing random things about him—about them, in the hypothetical situation that they happened—that Stephen should, by all rights, not have any way to know.

Tony didn’t know how Stephen had done it, wasn’t sure how Stephen was still sane.
Wasn’t sure how, after all that Stephen had to have seen, Stephen for some reason loved him. There was no way that Tony deserved that.
Personally, Tony blamed Stockholm Syndrome. Stephen had clearly been forced into Tony’s proximity for so long that he’d been tricked into it. And yes, he knew it didn’t work that way, but Tony thought it got the point across remarkably.
But however many timelines it had taken Stephen to fall in love with Tony… It had only taken this one for Tony to fall in love with Stephen.
He just wasn’t sure how to actually do anything about it. He wasn’t sure that Stephen would believe him if he did, for one. And, perhaps more importantly, a part of him worried that his love could never measure up to Stephen’s own.
He would try. Hell, he’d try. He’d try to do right by Stephen. But the fear lurked. What if Tony wasn’t enough?
Such thoughts clearly didn’t plague Peter who clearly thought that he and Stephen were perfect for each other. 
“Well, wouldn’t want to kick you,” Tony said easily, trying to hide his gut reaction. “Just open me up a portal to my place. We can deal with Peter tomorrow.”
The words were supposed to come out casual, but something must have slipped because Stephen eyed him curiously.
“Do you want to share a bed?” he asked, sounding uncertain. Uncertain didn’t suit Stephen.
Tony opened his mouth, then closed it. He wasn’t sure how to answer that question. The truth was that, yes, he sort of did. But he also wasn’t sure what that would do to their relationship. Would it progress things faster and further than Tony was ready for?
Would he be giving Stephen promises that he couldn’t possibly live up to?
He knew he wasn’t good enough for Stephen. Did he want to give Stephen the false hope that Tony might someday be?
Stephen’s brow furrowed and Tony realized he’d been silent too long.
“Yes,” he admitted. He ran a hand through his hair before throwing caution to the wind. “I do want to share a bed with you. I want to do a lot of things with you, actually. The sort of things that Peter wants us to do. Like hold hands. And kiss. And sleep together, both platonically and not. And you know… uh, fall in love with you.”
Stephen’s mouth dropped open just slightly. Clearly he hadn’t expected for Tony to go all out like that. Well, Tony had always lived to surprise.
“Except,” Tony continued. “It can’t be perfect, because apparently I kick in bed, and maybe that’s a sign of other things. That maybe I’ll metaphorically ‘kick’ if I try to love you.”
“You…” Stephen shook his head. “You sound ridiculous, you know that, right? Are you really suggesting that because you kick in your sleep you’re incapable of loving me?”
“I’m just saying that maybe it’s a sign.” He looked away. He sounded like an idiot and he knew it. But the point stood. “You love me.” Stephen didn’t deny it. They were far past that. “And I just… I don’t know if anything I give you can ever match that.”
Stephen huffed out a breath, shaking his head. He waved his hands and then suddenly he was in his pajamas instead of his many layers of sorcerer garb. Tony felt his mouth go dry. Stephen looked… well, he always looked good, but he looked soft and approachable in the soft, worn white t-shirt and comfortable looking lounge pants. “You’re an idiot. Now get into your pajamas and get in bed.”
“Did you hear anything I just said?”
“Yes,” Stephen said. “You admitted you love me. You added a whole bunch of nonsensical commentary about somehow not being enough for me that I’m choosing to ignore until we’re both less tired. But the essence is that you love me. Since that’s true, I don’t see why we’re still dithering about this.”
Tony stared at him, searching for the right words.
Somehow he still surprised himself. “Alright.”
If Stephen was going to make it that simple… Well, Tony could accept it. For now, at least, until they had that discussion they needed to have.
Stephen waited until Tony was comfortable in the bed before wrapping an arm around his waist and pulling him close. “I love you, Tony,” he murmured, breath warm against Tony’s skin. “Even when you kick.”
