true sadness is sasuke saying "it's okay, as long as we can still spend some time together" in response to itachi’s "we'll be busy with our lives from now on, you with your academy and i with missions" and then sasuke never getting to spend time with his brother ever again.

and itachi saying "maybe next time" to sasuke’s request to train him on their last day together, knowing full well there would never be a next time ever again and sasuke would hate him for the rest of their lives and he'd never get to be the brother he truly wanted to be.

this is why itachi’s final words to him on his first death hit so hard with all those flashbacks.
