Sorry I've been MIA. Life has had some pretty good highs and some really horrible lows recently. Christmas was great, but a few days later my mom went back into the hospital. After spending a day in the ER she was transferred back down to Philadelphia. She has a bad exacerbation of her CHF and it's not looking great. It's already gotten to the point where being the primary caregiver for both mom and dad has gotten to be too much, but now there's no way I'd be able to take care of her post-discharge, whenever that may be. Thankfully I have an amazing brother and SIL who are here for me and we are all going to do this together. My SIL drove me and dad down to Philly today to visit with mom. It took about an hour to get there. It was so nice to be able to see mom, not on facetime, but in person, and be able to hold her hand, talk to her, give her a hug and a kiss. But it was so fucking hard to see her like that. I don't know how long she will be in the hospital, but it's going to be a long stay. In the meantime, once the social workers reach out this week, we will all have to work together to figure out what type of facility is best suited for mom's needs, is close to home, and can safely provide all the care she needs. It kills me knowing that mom won't come back home. Just sitting here writing this, sitting in the living room where I usually am watching TV with mom at this hour, being here alone is hard. I'm crying just thinking about it. I hate this. I hate all of this. Coincidentally enough, the weekly email from church last week included a flyer about an anticipatory grief support group that another place nearby will be hosting. I signed up for it, its virtual, for 8 weeks, and starts February 1. I'm glad, because I don't know how to do this. I'm so scared.

21 days of DAPT, and then clopidogrel alone is ok if mild stroke. Alteplase if 5.5 hours after symptoms. Thrombectomy within 6 hours; but some pts can benefit from thrombectomy within 24 hours of symptoms onset.
Jardiance prevents hospitalization in pts with HFpEF.
Lower to BP no more than 25% in the first hour for pt's with hypertensive emergency.
From Up To Date:
Acute management – Optimal therapy, including the choice of agent and the blood pressure goal, varies according to the specific hypertensive emergency. It is generally unwise to lower the blood pressure too quickly or too much, as ischemic damage can occur in vascular beds that have reset their autoregulatory threshold to the higher level of blood pressure. For most hypertensive emergencies, mean arterial pressure should be reduced by approximately 10 to 20 percent in the first hour and then gradually during the next 23 hours so that the final pressure is reduced by approximately 25 percent compared with baseline. [So it is 25% in 24 hours].
[According to the rationale for the question, it said to lower the BP 25% in 1 hour, then to 160/100 mmHg.}
The major exceptions to modest and gradual blood pressure lowering over the first 24 hours are:
•Acute phase of an ischemic stroke – The blood pressure is usually not lowered unless it is ≥185/110 mmHg in patients who are candidates for reperfusion therapy (table 2) or ≥220/120 mmHg in patients who are not candidates for reperfusion therapy.
•Acute aortic dissection – The systolic blood pressure is rapidly lowered to a target of 100 to 120 mmHg (to be attained in 20 minutes).
•Spontaneous hemorrhagic stroke – The systolic blood pressure can be rapidly reduced if no contraindications exist.
Stop the ACEI 36 hours before starting Entresto in pts you are switching from ACEI to Entresto for HFrEF.
Non-DHP CCBs (verapamil and diltiazem) are not good for pts with HFrEF. The pt already has a poor EF and these meds slow the HR.

Hello everyone I haven't posted in a very long time. Today is giving Tuesday and I'd like to embrace and encourage  giving.  Id like to honor a family who has been struggling for sometime. She is a single mother of 2 but has taken in her sisters 3 children as well and raising the for years. Her father was recently diagnosed with calciphylaxsis.  He's had his pinky amputated and possibly his hand will be next along with other major medical issues. She has become his care taker. She homeschools the kids and has a part time job as a care taker. She does this all without a car.

Personal: I am drowning at night, which is why I’m unfortunately very awake lately during the witching hours.
Anyone have any cheap tips for lifting the bed or like, getting your upper body up? I tried pillows, but I just kick em around.
I tried sleeping in a recliner and it’s soooo uncomfortable.
Or I’ll just be awake, I guess. Channel my inner soulless Sam? Or maybe my inner-Dean-being-drowned-by-Michael? 😩 Or maybe just write silly beachfic?

Signs to monitor with CHF: JVD, pitting edema, crackles, urine output, weight
Every 1 increase in bicarb equals an increase of CO2 by 10. Pts who are heavier need more prednisone. We have a pt with BMI >50, and she needs 60 mg prednisone for COPD exacerbation.
