Is it just me, or is Othello the only character (besides Grell herself) to not misgender Grell? I don't have all the official translations with him, so I can't confirm, but I don't think I've seen Othello use pronouns for her at all?
Approaching gender identity within the series
As far as I can recall, Othello hasn't (yet) referred to Grelle in third person, so I have no idea. Othello is often talking to Grelle, but I haven't seen him talking about Grelle.
Ronald usually refers to Grelle as his superior or mentor.
William typically refers to Grelle as some creature or troublesome/terrible employee.
Sadly, we can't rely on licensed translations to get genders correct, anyway. It's a licensed translation that has Snake calling Doll "he/him" more than once, even though Doll calls herself a girl much earlier. Though, that might be on purpose. Snake might actually be doing that... or his snakes might be doing that... because Susan has to remind Snake that Doll is a female and has female issues to deal with. Considering how Doll dresses on stage vs how she appears off stage, perhaps Snake and his snakes know her better than we do. Perhaps Doll appreciates Snake and his Snakes saying "he/him"... when no one else seems to realize Doll's preference? Maybe Snake and his snakes are the only ones that Doll has confided in? Doll hasn't made it clear to us or to our earl... but possibly made it clear to them. 🤔
Unless a character specifically says what their own or another character's sex or gender is, we might not know, because gender pronouns like that aren't used much in Japanese, right? People often display their own gender in how they speak. But when talking to or about others, even honorifics are mostly gender neutral.
Back to Grelle... it seems to be that Othello is supportive, Ronald is respectful, and William is derisive. That's about Grelle in general, though, not specifically about gender identity.
