H5N1: What to know before fear spreads
What is H5N1?
H5N1 is a 1996 strain of the Spanish or Avian Flu first detected in Chinese birds before spreading globally across various avian species. H5N1 is similar to H1N1, but spreads slower and has a much higher mortality rate.
H5N1 may also be referred to as Influenza A. The American Association of Bovine Practitioners has seen fit to rename H5N1 to Bovine Influenza A Virus, or BIAV, and are encouraging others to use the same terminology.
I would not be surprised if the colloquial name among the public becomes Bovine Flu or American Flu in the coming months, and may be referred to as the Chinese Flu by the same folks who took the spark of the SARS-CoV-2 (COVID-19) pandemic as an excuse to be publicly racist to East Asian people without social repercussions.
BIAV is a virus, meaning that it is a (probably) non-living packet of self-replicating infectious material with a high rate of mutation. BIAV is structured similarly to SARS-CoV-2, having a packet of infectious material encased in a spherical shell with a corona, or crown, of proteins that can latch to living cells to inject RNA.

What is the history of BIAV?
In 1996 and 1997, an outbreak of BIAV occurred among poultry and infected 18 people in Hong Kong, 6 of which died. This seemingly isolated incident then infected ~860 people with a >50% death rate.
At the time, BIAV was known as Highly Pathogenic Avian Influenza, or HPAI, and killed nearly 100% of chickens within a 48 hour period.
From 2003 to 2005, continual outbreaks occurred in China and other East Asian countries, before spreading to Cambodia, the Netherlands, Thailand, and Vietnam.
From 2014 to 2016, it began being detected in American fowl, as well as mutating the H5N6 (lethal in birds, no human to human transmission) and H5N8 (largely spread through turkeys, ducks had immunity) viruses.
BIAV has since evolved into a clade known as 2.3.4.4b, and was first detected in 2021 in wild American birds. This then caused outbreaks in 2022 among wild and domesticated birds (such as chickens) alike, but was largely being overshadowed by the pressing SARS-CoV-2 pandemic at the time.
From 2022 to 2023, it was observed to be spreading among various mammals, including humans. Now, in 2024, we're having the most concerning rapid outbreak of BIAV since 2003.
BIAV is known to spread from mammal to mammal, particularly between cows and humans. BIAV may also be spread from cow to cow (highly likely, but not confirmed - this is likely the reason the virus has spread to Idaho from Texan cattle), and is known to be lethal to domestic cats and birds within 48 hours.
How does BIAV spread?
BIAV spreads through fomites - direct contact with infected animals or infected surfaces and then touching parts of your face or other orifices - as well as through airborne particulates, which may be inhaled and enter the sinuses and lungs.
BIAV is known to spread through:
Asymptomatic Ducks, geese, swans, various shorebirds
Symptomatic, may be lethal Foxes, bears, seals, sea lions, polar bears, domestic cats, dogs, minks, goats, cows, (potentially human to human, but unconfirmed - there have only been 8 potential human to human cases in 2024).
How can I protect against BIAV?
As BIAV is a type of Influenza A, existing protocols should do fine.
Current recommendations are to wash your hands vigorously after interacting with birds (I would also recommend doing this with mammals), avoid touching your face or other open orifices, and wear N95 masks.
Avoid sick or dead animals entirely - I would also recommend reporting them to your local Animal Control or veterinary centre and warning them about the infection risk. People who work with animals are recommended to also wear full PPE such as N95 masks, eye protection, gloves, and partake in vigorous hand washing.
If you suspect you've caught BIAV, seek medical attention immediately. Existing medications such as oseltamivir phosphate, zanamivir, peramivir, and baloxavir marboxil can reduce BIAV's ability to replicate.
Standard flu shots will not protect against BIAV. Remember - symptoms of BIAV may not manifest for between 2 to 8 days, and potentially infected people should be monitored for at least 10 days.
How far has BIAV spread?
BIAV is currently a global virus, though the current infection location of note is the United States.

Should I be afraid?
You needn't be afraid, just prepared. BIAV has a concerningly high lethality, but this ironically culls its spread somewhat.
In the event human to human transmission of BIAV is confirmed, this will likely mainly affect marginalized communities, poor people, and homeless people, who are likely to have less access to medical care, and a higher likelihood of working in jobs that require frequent close human contact, such as fast food or retail jobs.
Given the response to SARS-CoV-2, corporations - and probably the government - may shove a proper response under the rug and refuse to participate in a full quarantine, which may leave people forced to go to work in dangerous conditions.
If this does spread into an epidemic or pandemic, given our extensive knowledge about Influenza, and the US having a backup vaccine for a prior strain of H5N1, a vaccine should be able to be developed relatively quickly and would hopefully be deployed freely without charge - we won't have to worry about a situation like The Stand.
Wash your hands, keep clean, avoid large social gatherings where possible, wear an N95 mask if you can afford them (Remember: Cloth masks are the least protective, but are better than nothing. If you can't afford N95 masks, I recommend wearing a well-fitted cloth mask with a disposable face mask over it to prevent pneumonia from moisture buildup in the disposable mask), support the disabled, poor, and homeless, and stay educated.

Moderna will keep its COVID vaccine on the market at no cost to consumers, even after the federal government stops paying for it, the company announced Wednesday.
"Everyone in the United States will have access to Moderna's COVID-19 vaccine regardless of their ability to pay," the company said in a statement.
Last month, the vaccine maker was slammed for reportedly considering a dramatic price increase for the shot, which it had developed with the help of the federal government.
The proposal was also bad timing: The Biden administration was moving toward ending its designation of a public health emergency on May 11, which meant that federal funding for vaccines would soon dry up and uninsured Americans would have to pay out of pocket for their boosters.
Among the critics of Moderna's reported consideration of a price increase -- from about $26 a shot to as much as $130 -- was Sen. Bernie Sanders, who has long advocated for government-funded health care and alleged the move would result in deaths.
"How many of these Americans will die from COVID 19 as a result of limited access to these lifesaving vaccines?" Sanders, I-Vt., wrote in a January letter to Moderna.
"While nobody can predict the exact figure, the number could well be in the thousands. In the midst of a deadly pandemic, restricting access to this much needed vaccine is unconscionable," he added.
Now, Moderna will be the sole manufacturer of COVID vaccines offering its shot for free to the uninsured. Under federal regulation, insurance companies are already required to foot the bill for COVID vaccines.
"Moderna remains committed to ensuring that people in the United States will have access to our COVID-19 vaccines regardless of ability to pay," the company wrote in its statement.
"Moderna's COVID-19 vaccines will continue to be available at no cost for insured people whether they receive them at their doctors' offices or local pharmacies. For uninsured or underinsured people, Moderna's patient assistance program will provide COVID-19 vaccines at no cost" after the public health emergency expires.
To date, the federal government paid for all COVID vaccines for Americans, whether they were insured or not using emergency money passed by Congress. But President Joe Biden says he plans to let the nationwide public health emergency expire May 11.
Once that happens, federal support ends for many of the programs put in place to help uninsured Americans, including expanded Medicaid, testing and treatments.
Last month, the World Health Organization said COVID-19 remains a public health emergency worldwide, but that the pandemic was at a "transition point."
WHO Director-General Dr. Tedros Adhanom Ghebreyesus said the "global response remains hobbled because in too many countries, these powerful, life-saving tools are still not getting to the populations that need them most – especially older people and health workers."

Why UK doctors strike and stress
Junior doctors working in England’s National Health Service started a six-day strike over wages on Wednesday, a desperate move in a demoralised NHS suffering from huge workloads, staff shortages and lack of investment, according to Politico.
Healthcare workers are demanding pay rises since 2022 to keep up with inflation, which is also part of a wider sense of dissatisfaction among staff who feel overworked and undervalued, especially following the COVID-19 pandemic.
UK healthcare faces challenges on all fronts. Firstly, there are fewer doctors per capita than any other country in Europe. Secondly, healthcare providers and think tanks have systematically reported staff shortages and expressed alarm over the impact on patient care and workforce.
Moreover, underinvestment led to the NHS being ill-prepared for emergencies and staggered in the aftermath of the pandemic. This had a negative impact on staff morale.

As frustrated demonstrators take to the streets across China to protest the government's draconian COVID-19 restrictions -- prompting rare civil unrest and clashes between the public and officials -- the Biden White House is choosing its words carefully and deliberately.
Top US officials who have been closely monitoring the unrest in China have made two things clear in the past few days: that the Biden administration supports any people's right to peacefully protest and that it simply does not see China's so-called zero-COVID policy as a sound approach.
But administration officials have been careful not to step beyond the contours of those public comments, carefully stepping around broader questions about the US's assessment of the situation or its potential future role in supporting the Chinese people's cries for more freedom.
That sensitivity and caution is in no small part a reflection of the deeply complicated -- and tenuous -- place that US-Chinese relations currently stand. The two countries' relationship had reached its lowest point in decades this year, with tensions fueled by the standoff over Taiwan, economic disputes and other areas of disagreements.
A senior US official emphasized to CNN that the White House is being careful not to overstate the nature of the protests, noting that while there have been some calls for Xi Jinping to step down, as of now, most of the protests in the country of over one billion people seem small, localized and aimed more at the narrow goals of ending the COVID lockdowns and securing better working conditions than a loftier push for democracy.
"We have to be very careful of not creating a distorted reality," the official said.
A historic bilateral meeting between President Joe Biden and Xi earlier this month on the sidelines of the G20 summit in Bali, Indonesia -- the first in-person meeting between the two men since Biden took office -- was aimed at marking a starting point of sorts in trying to strengthen those strained relations.
And while that meeting, which spanned more than three hours, hardly resolved some of the longest standing issues between the two superpowers, Biden and top officials made clear afterward that there was general agreement on the importance of keeping lines of communications open between Washington and Beijing -- and finding area of common ground.
"It was not an easy meeting," another senior US official told CNN, "but they both understood that they represent countries with very different views of the world, which need to manage competition responsibly." Other officials said the US and China relationship at least now had a "floor," with both countries in relative agreement on issues like climate change and the danger of Russia using a nuclear weapon.
This week, the White House has insisted that the administration intends to continue building on that progress.
"There's been no change to our desire to continue to see these channels of communication stay open, and we were heartened coming away from the G20 that both leaders were able to agree on getting some of these working-level discussions back open," National Security Council spokesperson John Kirby said in the White House briefing room on Monday.
The first senior US official said that there have been "zero concessions" to China since the Biden-Xi meeting took place, but acknowledged that both countries have backed into their respective corners when it comes to openly hostile rhetoric — a deliberate approach to avoid a continued deterioration in relations.
A separate senior administration official said, however, that, "To us, this is not about the US-China relationship. This is about the fundamental right of peaceful protests and the ability of the protestors to speak for themselves. We are watching this closely."
Asked by one reporter whether the protests in China may mark a moment for the US to "more forcefully" advocate for freedom and democracy -- particularly given how forcefully Biden has spoken out about the important distinction between autocracy and democracy both at home and abroad -- Kirby responded: "This is, I think, a moment to reassert what we believe in when it comes to free assembly and peaceful protest."
"And we've done that and will continue to do that," Kirby continued, "whether it's people protesting in Iran or China or anywhere else around the world."
What Kirby didn't mention was that Biden himself has weighed in much more forcefully on the unfolding protests in Iran -- even saying at a campaign event earlier this month: "We're going to free Iran. They're going to free themselves pretty soon."
Officials argued to CNN, however, that in the White House's view, the protests in Iran and in China are not comparable, at least for now. In Iran, the regime is routinely killing protesters, who themselves are fighting for a largely unified goal: an end to the Islamic Republic, following the deaths of women in the custody of Iran's so-called "morality police." Still, White House officials do view the protests in China as remarkable because they are evidence that Beijing's total information blackout is cracking, a senior official said.
Asked to elaborate on how the US is weighing the protests in China given the context of the tenuous Washington-Beijing relationship -- and what role the US believes it might play as people in China call out for more freedom -- one senior administration official said it would be best, for now, to leave the answer to that question of: "We are watching this closely."
DIFFICULTY ASSESSING SCOPE OF PROTESTS
While Biden administration officials have been closely monitoring the situation on the ground for several days, one US official acknowledged it was still difficult to assess the scope and scale of what was taking place on the ground. The early stage nature of the protests, along with the inherent unpredictability of such events, has played a role into the nature of the response, the official said.
But so, too, has the concern that, shared by several top senior Biden national security team officials, that appearing to lean toward the protesters would actually have the effect of undercutting what's taking place. The concern that Chinese officials would use any US response as an effort to paint the protests as a malign foreign effort to undermine China has been a closely considered factor, a senior administration official said.
White House officials have kept explicit and intentional distance from the more fiery calls from protesters related to Xi and the communist party. There is no plan to shift that position, part of an implicit acknowledgment of a reality that carries bilateral and geopolitical consequences of enormous significance.
Still, the White House has continued to monitor events closely -- and Biden has been kept regularly updated -- as US officials attempt to assess how the protests will progress and, perhaps more importantly, how Chinese officials will respond should the effort grow in size and intensity.
US officials have declined to weigh in on what actions, if any, would be taken if a particularly harsh crackdown comes in the days or weeks ahead. "We're not doing hypotheticals right now," one official said. "We're watching closely and just don't have anything more than that."

Bulgaria faces pandemic-related lawsuits
Three lawsuits were filed against Bulgaria over the failure of a vaccination campaign and the authorities’ response during the pandemic that led the country to have the highest COVID-19 death rate in Europe, Euractiv reported.
The complainants seek state responsibility for the failure to vaccinate people most vulnerable to COVID-19. One claim was submitted to the national courts and the second was filed with the European Committee of Social Rights of the Council of Europe. The third concerns the decision to close schools when free access to vaccines was available.
The reason for the lawsuits against the country is that thousands of Bulgarians with comorbidities – cardiovascular, diabetic, chronic respiratory or nephrological diseases or cancer – as well as people over 60 could have avoided serious illness and death if they had been vaccinated in time.
Although the lawsuits are directed at the actions and inactions of the previous administration, the current one would have to be held liable.

Meanwhile, war-related disruptions to energy and food markets are receding, it said.
“At the same time, the massive and coordinated tightening of monetary policy by most central banks should start bearing fruit, with inflation returning to targets,” according to the IMF.
