Post-Top Surgery Weight Gain: Will My Breasts Grow Back? (Nope!)

I've had a couple people message me asking about this privately, and it was something I had a fear of when I was getting ready for top surgery, so I wanted to address it in a public post:

After top surgery, your breasts will NOT grow back, even if you are not on testosterone. No, not even if you gain weight.

The chest is, to borrow Shrek's wisdom, like an onion, and has many layers. The ones top surgery is concerned with are the chest wall (muscle), the breast tissue, fat, and skin, in that order.

Your chest wall is a bunch of muscle that protects your organs. This will not be removed or touched. Your breast tissue sits on top of that, and this is what will be removed during surgery, leaving just enough tissue to make your chest proportional to your body. Very few people have totally flat chests. On top of this is body fat, much of which will also be removed or reshaped during surgery. Then there is your skin, and your nipples. Typically nipples (including areola, the area around your nips) will be taken off, trimmed down to a smaller 'masculine' shape, then grafted back on. Or you don't have to get your nipples back at all! I didn't, so it's up to you.

To reiterate: your breast tissue will NEVER regrow. Your nipples and areola will NEVER regrow, regardless of if you are on T or not.

However, FAT may fluctuate in this area. If you lose weight, whether on T or not, you will probably lose fat here and may have loose skin, which many surgeons can address in revisions. If you gain a substantial amount of weight and are not on T, you may gain more fat here- take a look at your close female relatives. When they gain weight, do they gain it in their breasts and side-boob area? When they lose weight, do they go down a cup size?

If you are on T, testosterone will change the way your body distributes the weight it gains. Look at your close male relatives. I'd be willing to bet money that when they gain weight, they gain it predominantly in their stomach. When you are on T, your body will probably begin to redistribute the way it gains fat to a more 'typically male' pattern, like the stomach, as opposed to hips, thighs, and chest.

Something to keep in mind, especially for fat people but really for anyone getting top surgery: YOUR ANGLE IS DIFFERENT! Look down at your chest right now, pre op. You can see aalllll the way down your cleavage. But if you asked someone standing in front of you if they could see all your cleavage, they could not. Even if you were naked, you seeing your own chest from a top-down angle makes things look further out/rounder/bigger than how other people see you.

Please keep this in mind when you look at your surgery results- the fat that was left behind and reshaped on me looks bigger/rounder to me, looking down, than to other people looking at me. And this is only in resting position! Move your arms around (once it's safe dont do this freshly out of surgery please dear god), raise them over your head, put them out to your sides and T pose. Look at the way the fat moves under your skin and around your body; how it looks 'flatter' in some positions and 'rounder' in others. This is totally normal! This is what skin and fat are supposed to do, and do on everyone who has skin and fat! These are NOT breasts, and they will NEVER be breasts again.

I hope this helps some peoples fears be assuaged. I know it can feel silly to ask if your breasts will grow back, but it is a valid fear, especially for plus size people, and it is also a fear I hope is now at rest. Your breasts will never come back. You're gonna look and feel great.

tl;dr- no, your boobs will never come back after top surgery, regardless of whether you are on T or not. you might have leftover fat here or gain fat here, but this is a universally human experience, and happens to cis men just as much as any other person. also, your angle viewing your own chest from the top-down is a misleading optical illusion that is not accurate to what other people see looking at you front on.

just over a week post top surgery and man…it’s been one of the most Up and Down weeks of my life and i’m just about coming up from a HUGE depressive hormonal dip. the pain is slowly going away and even though i have some fluid build up which is annoying it still looks really good and as far as i can tell it’s all healing well! recovery has sucked a lot but now i’m feeling better and a bit less sore i’m so so so happy i did this and my body feels so much more My Own even with the pain and achy-ness

i’ve still got a bit of my surgery costs to pay off (LINK HERE) so if everyone could share as much as possible and donate if they can i’d be incredibly grateful!

not to stop being fun and cool and chill on main but i’ve been super dysphoric about my chest lately and it’s really starting to get to me

I was talking with my mom and she said they'd probably make me lose weight to go on T. Is that something that happens? I haven't started transitioning yet and I'd like to asap. I'll lose it if I have to, I've just never heard of that before and would appreciate other fat transmascs chiming in. Thanks!!

Doctors will often recommend you lose weight before you do anything at all. You could say you're nauseous and they'd tell you to lose weight before they'd give you an antacid *is bitter*

But in all seriousness, no, you probably won't have to lose weight before starting T. Some doctors may explain to you that it will be easier for your body to redistribute fat to a more typically "male" pattern if there is less fat to move, but this isn't a requirement (legally or medically).

Also they may suggest you lose weight in preparation for top surgery, but the key thing here isn't necessarily losing weight, it's building muscle definition in the chest wall. You can work out your chest and have hella muscle and still be fat, as the fat sits on top of the muscle.

Plenty, plenty of fat dudes are on T and have been/stayed fat the whole time, so don't worry about it! As long as you don't have any contraindications like chronic high blood pressure, you'll probably be allowed to start T after just a couple appointments and tests.

I'm going to reiterate what I said in the tags on that one post so more people that need to see it:

IF YOU ARE A TRANS FEMME PERSON ON HRT AND ARE UNSATISFIED WITH YOUR GROWTH AND FAT REDISTRIBUTION, PLEASE CONSIDER TRYING EATING MORE.

I know too many rail things trans women who complain that they aren't getting what they want and are too quickly jumping to surgical remedies due to it. I'm not saying top surgery shouldn't be an option or that we need to put more barriers, but I am saying it is an incredibly costly surgery that could have impacts down the road that you should consider your options before doing so.

Your body can't redistribute fat it does not have.

Genetics has a role to play, absolutely - I'm lucky that I have the kind of genetics I do - but I promise y'all it can work. After my doctor convinced me that putting on a little weight was okay and not the end of the world and would help with my results, I went up a full cup size and then some, and I have hips and an ass.

Some of that is adding on progesterone, but one of the things progesterone does is cause you to retain more fat, but that fat goes to the places you want it!

Like, I get it, everyone woman struggles with our weight and the constant inundation of 00 models and DD breasts, but that's not reality. Those women are on hyper specifics diets that are basically a full time job to achieve that look (plus some help from post production). It's not feasible to look like that.

Don't be afraid of weight gain. It's okay. You'll be okay. I was deathly afraid of it, being skinny was honestly part of my identity, I could wear children's extra large until I was 27, but even if it's not for your own health because despite popular belief skinnier is not necessarily healthier, just consider it.

#So my dogs fatty tumor is coming back faster then we hoped #We had it removed at the start of 21 but they had to leave some to not damage his muscles #It's only started regrowing in the last few months and it's already almost half the size it was before #Normally I wouldn't worry about it but this one is on the back of his paw #And we're all worried that if it gets to big it could break through his skin #Plus it started really affecting how he could walk #And on top of all that I don't know if he can go through another surgery because he's 10 now #He's doing mostly good for his age but I don't know if I trust putting him under for hours again
