Thinking about the possible aftermath between Tomura and Izuku after the war, and how Inko might feel about it
Like... You tried to kill my son multiple times. You held him hostage once. You tried to kill everyone. The attack from your villain group is the reason he isolated himself from the people who love him. I can't understand you. I don't think I want to understand you. How could you do all of this? Why did my son want to save you?
Why, in the end, did it work?
You're in this room with these heroes and I don't know anything about you but still I never imagined someone like you could look so small. I don't know what they plan to do with you. I don't know what to do either. I used to think you should leave us alone, locked up and shut away. But the way my son looks at you, the way he hangs onto the hope for your future, the way he looks even more changed after coming out of that final battle, I wonder if he wants there to be another way. I wonder if locking you away is the right thing
I offered cookies to my son. I only offered them to you because I felt obligated and nervous about what you would do if left out. I can't forget the look on your face when you took one and bit into it. You thanked me, quietly, and it was strange to me that a villain like you could behave in such a way
You look small again. You keep to yourself despite how rude you can be. You keep bringing out that video game device when you're bored and take up space anywhere you can, on the floor or on the couch, in people's way. There's paperwork to do, a pile they want you to fill out before going forward, and you're groaning like it's homework. You behave more like a teenager than a villain. You're more like a kid, sometimes
I made cookies again. This time I offer one to you because I want to. You take one and thank me before you even take a bite. It's like you already know they'll be good

Midoriya is hit by a quirk and ends up in a parallel universe where he has been dead for about five years. In this universe, All Might was declared legally dead three years ago, All For One is a gray guy and his mother is a super villain, who is hunting him down because she thinks he's just some sick bastard using the face of her dead child. To stay alive and under the radar, he teams up with this version of Dad for One and Small Might, who is not dead yet, but almost.

Decay Belonged to Izuku
Here is my two cents.
All for One being Hisashi Midoriya would not be enough to break Izuku nor Tenko. Tenko doesn't feel like he was replaced, like how Touya feels. He feels used. Like a puppet. Izuku has more father figures than he knows what to do with. I like Dad for One, don't get me wrong.
But it's not powerful enough.
The only thing that would hit every box of everything the story has been about is if All for One and Dr. Garaki gave Tenko Izuku's original quirk. If Izuku was meant to decay.
"Face it, Deku, you'll never have a quirk stronger than mine."
"Even you could become a hero."
"Perfect quirk for a villain. Even I started to believe it."
"These things aren't gifts, they are curses."
"I can take and give quirks, but I can't just throw them away!"
The absolute worse thing to Horikoshi isn't merely good or bad things. If it was, All Might would be dead. But he's alive because the worst thing in the world is for him to watch his legacy burn and be helpless. If it was, Endeavor would be in jail. But he's free, and expected to re-explain and re-affirm to every citizen and journalist who asks him that this is all his fault. The absolute worse things to Horikoshi are the bittersweet things you are expected to be grateful for, even as they eat away at your will to live. The worst things are the awful truths you must endure. Congrats Hawks, your childhood hero is worse than your parents. Congrats Overhaul, you accidentally put your father into a permanent coma....
So wouldn't the worse thing Izuku could experience from "Hisashi Midoriya" be him expecting gratitude for protecting Inko from the ticking time-bomb that is Decay? Wouldn't the worse thing be the guilt Izuku would have for everything Tenko had gone through? Not simply because "I know the guy that did it to you."
But "he did it to you to spare me."

Someone is going to explain to me why Inko was doing the Upset Woman pose BEFORE Izuku was declared quirkless. And then the next thing is her bland "shocked" performance with the nonsensical test results and exposition provided by All for One's personal doctor... then she's crying and apologizing to Izuku...

funniest possible outcome for the "who is izuku's dad?" question is if afo very dramatically reveals that he is izuku's father a la darth vader on the battlefield -- "that's right izuku.... i am your father!" -- and everyone stops in shock but izuku just looks confused and is like "what? no you're not. my dad is midoriya hisashi" and afo is like "you foolish boy, midoriya hisashi is my alias! i am your father!" and izuku just goes, "what? no you're not. papa works in hokkaido. that's why he's not home often. look, we went to the musutafu aquarium 3 months ago." and he pulls out his phone and starts flipping through his gallery and just shows afo countless photos of inko and hisashi holding hands, holding baby izuku, izuku sitting on hisashi's shoulders. cue the league in back going, "well this is awkward"

Summary: you are the sibling of Izuku and your upset that he isn't taking care of himself.

You were the child of Inko Midoriya and sibling of Izuku Midoriya. You would think being the younger sibling of Izuku you too would want to be a hero, but oddly enough you never really liked the idea of being a hero. Something just felt wrong with the hero system. You were just a year younger then Izuku but you were stronger and faster, mostly because you were smaller and more flexible. You would always do your best to protect Izuku from bullys but they always find a way past you, especially Bakugo. Unlike your dear bother you got a quirk, mostly your mother's quirk, unnamed object attraction. Yours is much stronger then your mother and you can sometimes even use it as telekinesis.
While you were in middle school you grew to highly dislike heroes and mainly because of Bakugo, you couldn't believe they would consider making him a hero. He was no hero. you deeply cared for Izuku and wished everyone would just leave him alone, he was much to kind for this world, he was always a hero to you. But sadly as kind and as loved as you were, you couldn't stand by and watch heros do as they pleased. So when Izuku left for UA, you started to work secretly as a villain, you felt bad for tricking/using Izuku to get information from UA and other heroes. You were know as (V/N). You hunt down heroes that take advantage of their status and do bad things behide closed doors. You were originally just a vigilante but you accidentally killed a hero who was using his popularity to get his hands on younger girls. You went a bit far and ended up killing him, after that you were seen as a villain. You were happy that decided to wear a mask so that you can live a normal life with you mom and brother.
'I'm not bad, I just want to protect my mom and brother' is what you always say to yourself when you sneak out in the dead of night to hunt down a hero.
You care deeply about your mom and brother, so when you found out Izuku wasn't eating enough because he'd rather trian, you were mad. That was a big no no for you, so you decided to take action. Late at night you broke in to the dorms where Izuku was now staying after that villain attack that nearly gave you a heart attack from pure worry. You had a bag of food with you, filled with packed lunch and sweets.
The following day
Izuku woke up scratching his arms, ready to go for a morning run. He stood up and went to get clothes to get changed but when he opened his drawer he found a lunch box with note saying "EAT NOW" Izuku was confused but the food looked really good so he ate it, reminded him of home. But that wasn't the only time this happened, when he got to class, he found a packet of sweets in his bag that he doesn't remember buying, and this happened throughout the day where he would find food randomly, in his bag, his drawer, his shoes, his hero suit and even his socks, and this happened again the next day and the next day and the next day. It all eventually stopped when he started having a proper balanced meal time again. But he still couldn't figure out who it was but decided whoever it was was just looking out for him, he even joked about how he had a personal guardian... well he wasn't wrong.
You watched happily through the security cameras you hacked as Izuky at breakfast with his friends before going on a run. Feeling like a proud sibling.

You know what, I’m gonna do something fun. You remember that theory I had about AFO stealing the Shimuras quirks and those quirks & corresponding vestiges being in Tomura? Well I’m thinking I’m gonna take a crack at guessing what those quirks could be (/making up whatever sounds cool & might work because there’s not much to build off of), under the assumption that Tomura will get and use them like Deku and the OFA vestige quirks. Because I’ve grown really attached to this theory*.
Kotaro Shimura:

Starting off is everyone’s...at least 2nd least hated abusive parental figure of MHA and Nana’s son; Kotaro.
My guess for him is one I had since his last appearance alive, when a pair of hedge trimmers seemed like they flew to his hand to help him strike his son. Not that that’s how it happened in the anime, but I’m still attached to the idea regardless. I think he had a form of Psychokinesis, similar to Inko Midoriya (if you’d like, feel free to insert an Inko Shimura theory here). It seems a natural child quirk to Float, and would be useful to Tomura.
A telepathy quirk would, for example, allow Tomura to fight at range in a way he (and most of his allies) otherwise can’t; which opponents have tried to use against him in the past.
Chizuo:

Now, because of that fact that quirks build of their parent quirks, we’re gonna be a bit unconventional and do the grandparents next and move down the family tree; starting with the father-in-law because his cooking gives him one (1) actual character trait to base some assumptions off of. So I’m only mostly making something up from thin air.
Of course it probably wouldn’t be anything too impressive, being a civilian, so I’m gonna go with something really basic but potentially useful. Namely, a heightened sense of taste and smell that allowed him to become a fairly decent cook in life; mostly for his family.
Still as I said, basic can be useful, and a heightened sense of small could provide Shigaraki numerous advantages like it does Gigantomachia; such as tracking opponents or sniffing out traps.
Mako:

The grandmother, and sadly the one who we know the least about and has no parents I can speculate on; meaning I’m just making something up from scratch. If there’s anything to build on, it’s that she was a caring person, and I kinda get the impression she was the bread-winner of the pair back in her day. Hmm...
How’s about a Magic Hand quirk. Like the DnD cantrip. The ability to conjure some energy hands for when you need an extra pair or some extra reach. ...It’s the best I got.
Now Magic Hand might not seem too useful for Tomura; what with them likely to not be able to kill people or steal quirks like his normal hands, and also he can conjure all the hands he wants now. But idk, I think a pair of ghostly grabbers could prove useful.
Nao Shimura:

The penultimate family memeber; it’s time for Tenko’s kinder parent, Nao Shimura. Sadly, she is also someone without much of a developed personality outside her familial role to take power ideas from; but we did just came up with ideas for her parents to combine & build off of. Better than nothing.
Magic Hand & Enhanced Taste shall combine to form Body Double. The idea is that she conjures up an assistant made from a mist she exhales to, like her mom’s quirk, act as an extra pair of hands. Except it’s better because it’s a whole body. Still relatively short range though, if just to give room for her daughter.
This would be useful to Tomura as a distraction and something to take a hit for him when he’s in danger. Because it feels fitting for his mother’s quirk to be the one to best protect his life. (especially since I keep theorizing he’ll give Regen to Dabi.)
Hana Shimura:

Last but only technically least, one of the first Shimuras we ever saw, and the one I assume has the best quirk after Decay in the Shimura family since she had ambitions of being a hero; it’s Hana’s turn.
Factoring that with her free spirit, and combining Psychokinesis with Body Double, I’m guessing she had an Astral Projection quirk. The ability to create a solid double of herself which can be remote controlled. Unlike her mother’s quirk, which was more like an extra set of limbs, Astral Projection allows the user to see and hear through it and use it from any distance; and had Hana grown proficient with it, she even would have been able to change it’s shape in a variety of useful ways.
When combined with the AFO quirk in Tomura’s hands though, it allows for an even greater application. As by allowing a quirk vestige to inhabit the Astral Projection; Tomura could effectively bring back to life anyone who’s quirk was stolen, including anyone in his family. With enough control, perhaps his entire family! Now the catch is that any Projection may not be able to use their own quirk, as that would still be in Tomura’s body. But since the form could shape shift, the vestige may be able to replicate their quirk by simply shaping their body the way it should do with their quirk.
Such an awesome ability would surely become one of Tomura’s headliner powers, thus making the duo they always wanted to be...probably minus the ‘hero’ part, depending on when all this happens. (And honestly I don’t think he could ever be a hero with how the word has been soured for him.) But the important part is, they’d be working together. And with the rest of the family to boot.

Au where Inko Midoriya helps out people down on their luck - some of which eventually become villains - and is considered a ‘DO NOT HARM’ individual because she is seen as the civilian mom of the villains.
A bunch of villains attack class 1A while Inko Midoriya is there and when she sees them she’s just like “I’m not mad, just disappointed.” and all of them swear off villainy.
Could potentially be paired with a Dad For One AU or Vigilante Inko AU

Inko as a villain who falls in love with All For One. The pair of them becoming a terrifying villain duo and starting a life together then eventually have Izuku.
Inko stops being an active villain, choosing to instead focus on raising her son.
All For One, also known as Hisashi Midoriya to their civilian friends, accepts this decision and chooses to pay for his wife and son to live a comfortable and peaceful life.
And then fourteen years later All Might gives Izuku One For All.
