ear’s guide to writing stab wounds
disclaimer!!!: this isn’t to be used as actual medical advice there isn’t enough information at hand to properly treat someone, this is just for writing.
hemostatic (blood clotting) control is the number one priority. minor bleeding can be controlled with direct pressure to the wound. moderate bleeding may require a compression bandage as well as direct pressure. severe penetrating wounds or a nicked artery means wound packing will be necessary as well as direct pressure.
types of stab wounds:
- blunt stab wound means whatever object caused the trauma wasn’t sharp or wasn’t moving fast enough so the skin tears.
- penetrating stab wounds go deep into the skin and into the muscle.
- superficial stab wounds don’t go too far under the skin and look worse than they actually are.
steps to treatment:
1. if the object is still inside the person’s body do not remove it unless it’s to the groin, neck, or axillae (armpit) and the bleeding is hard to control.
2. remove person’s clothes to check for any other wounds and keep the area clear.
3. keep an eye on blood pressure and airway.
4. the wound type and location changes how the rest of treatment will follow.
location:
head: direct pressure is mainstay. head wounds also bleed more than any other part of the body. has the highest mortality rate.
face: severe wounds to the face means the patient has to be seated forward to keep blood out of the airway.
neck: direct pressure is mainstay. if the airway can be secured and is absolutely necessary, wound packing can be applied.
arms: depending on the severity, any of the three treatments can be used.
legs: depending on the severity, any of the three treatments can be used.
abdomen: damage to organs is highly likely. direct pressure should be applied first while surveying if the object was long enough to damage an organ. if so, wound packing may be necessary.
chest: if the wound is deep enough it can cause open pneumothorax (‘sucking’ chest wound) a seal needs to be placed over the wound to keep air from getting inside. if this isn’t done in time the affected lung will collapse.
back: can typically be treated with only direct pressure. wound packing is rarely necessary.
neck, chest, abdomen, and pelvis wounds should never be packed unless absolutely necessary.
treatment types:
direct pressure: key to any wound. can be done with whatever is available even if that means the medic needs to use their own body weight.
tourniquets: applied to the limbs. typically not applied for more than thirty minutes. in some cases, they can be left on for hours, keeping the phrase “life over limb” in mind. complications with tourniquets like nerve damage or ischemia (no blood circulation) are rare. don’t apply over a joint and apply above the wound.
wound packing: done with standard gauze and or hemostatic dressing
wound packing steps:
1. control the bleeding with pressure. use anything available even if it means t shirts or a knee.
2. place a gloved finger inside the wound too apply initial pressure. this will hurt like a bitch. also gives you an idea of what direction the blood is coming from so gauze can be used more accurately.
3. begin packing the wound with gauze. keep pressure on the wound with finger while wrapping gauze around another finger and pushing it in the wound.
4. keep packing the wound until no more gauze can fit in, and then keep direct pressure on for at least three minutes.
5. after the three minutes, use something like a bandage wrap to keep the gauze secure inside the wound.
6. splinting the area to keep it immobilized may be vital to keep the hemorrhage from restarting
7. if bleeding continues medic has to decide if they need to take out gauze and reapply with new gauze or apply more direct pressure. this is usually done by how long it takes to get to further treatment. the longer the wait the more of an incentive it becomes to repack the wound. if it’s just down the road then apply pressure.
most likely complications:
hypoxia, shock, and hypothermia are complications that need to be watched for and treated immediately if they occur.
hypoxia:
occurs when a region of the body doesn’t have enough oxygen in the tissue. can lead to organ damage, brain and heart damage being the most dangerous.
symptoms include: tachycardia (rapid heart rate), difficulty breathing, confusion, shortness of breath, anxiety, headache, and restlessness.
severe symptoms include: bradycardia (slow heart rate), extreme restlessness, and cyanosis (blue or purple tint to skin).
treatment: oxygen
shock:
life threatening condition where the body doesn’t have enough blood volume to circulate through itself. if it goes on for long enough, organ damage and death may occur.
symptoms: rapid, slow, or absent pulse, heart palpitations, rapid shallow breathing, lightheadedness, cold clammy skin, dilated pupils, chest pain, nausea, unfocused eyes, confusion, anxiety, and loss of consciousness.
treatment: if they’re not breathing, cpr is required. if they are breathing, lay on back and raise feet a foot off the ground to keep blood in the vital organs.
blood transfusion and fluids once in a hospital setting.
hypothermia: occurs when the body is losing heat quicker than it can produce. the more blood that’s lost the more likely hypothermia is to occur.
symptoms: differ based on severity
hypothermia:
in mild hypothermia: shivering, exhaustion, clumsiness, sleepiness, weak pulse, tachycardia (rapid heart rate), tachypnea (rapid breathing), pale skin, confusion, and trouble speaking.
in moderate hypothermia: bradycardia (slow heart rate), bradypnea (slow breathing), slurred speech, decline in mental function, shivering slows down, hallucinations, cyanosis (blue or purple tint to skin), muscle stiffness, dilated pupils, irregular heart rate, hypotension (decreased blood pressure), and loss of consciousness.
in severe hypothermia: shivering stops, hypotension (low blood pressure), absence of reflexes, compete muscle stiffness, fluid builds up in lungs, loss of voluntary motion, cardiac arrest (heart stops beating), coma, and death.
treatment: covering with a blanket, hat, and jacket, adding external heat like a hot pack, and if severe and in a hospital setting, warm fluids via iv, warm oxygen, and or a machine to warm the blood in the body.

You were bleeding. Heavily.
The weapon was still lodged within your chest. 6 inches deep it sat, dislodging your ribs which you were sure were shattered, and tickling away at your lung. You hadn’t seen it coming, hidden behind the back of the man you had been tracking. It was too late by the time you had realised he had it; he got you good before sprinting away.
You had tried to get out of this life, leaving those you loved behind. You couldn’t deal with constantly living a lie. So you left. Packed your bags one night, upped and left. They had tried relentlessly to get ahold of you; Missed calls, unanswered texts and even trying to track you on the street cameras, but you knew how to cover your tracks. Being trained as a vigilante does that to a person. You changed your hair and your name, changed your clothes and your hobbies until even you began to not recognise yourself. It hurt to heave your family that way, but you thought it was the only solution. You had seen the texts flooding in. At first, they were concerned, then they turned to anger and morphed into pain and fear once again.
As you staggered backwards, gripping onto your chest, you knew that you shouldn’t have followed him. You knew that you shouldn’t have, that you should have just turned and ran the other way like any normal person should have, but you couldn’t. You missed them. You missed Tim’s meek smile and Damian’s sarcastic remarks, Jason’s wit and Dick’s comfort. You missed collapsing in the study after a long patrol to curl up and watch a movie. Doing this would have brought you one painstaking step closer to them again. So, it was one more criminal.
The blood pooled around you as you cried out, sinking to the floor; a dark river of scarlet oozing from between your fingers. Coughing harshly you tried to apply as much pressure to the wound as your weakening body would let you. The fit only jostled the wound more, eliciting a cry of pain which caused you to fist your shirt.
That was when you suddenly remembered the phone in your jean pocket. You knew that you should call for help, but when you agonisingly slipped it out of your pocket, the number your bloodstained fingers dialled wasn’t the typical number someone would call if they had a fatal stab wound.

Tim froze when he saw the name flash on the screen; a name that he knew at one glance. Your name. He hadn’t stopped searching for you since you left them high and dry, albeit there was no sign of you. You had vanished without a trace. His late nights turned even later as he scoured the internet and hacked into the cameras trying desperately to catch a glimpse of your face. He was often accompanied by Jason, who spent his time calling your number or leaving you strands of unopened texts. He hadn’t taken it so well. His nights were full of relentless torment in the form of unsaid thoughts of you. So, when he strolled in groggily to take his usual seat beside his brother but saw your name on the screen, his glass shattered on the floor.
“Is this real?” He asked, ignoring the glass and crossing the room in two strides to reach the computer. Some part of him believed that this was some sick joke, but then the ringing stopped. And started up again moments later.
“Answer it.” Jason told him, fumbling with the hem of his shirt.
When Tim pressed the answer button, they were hit with the sound of ragged breathing.
“Y/N?” He furrowed his brow, unable to hide the worry laid thick in his voice. A million thoughts raced through your mind at once.
There was silence on the other end of the line, before your voice pierced through the emptiness. “Jason?”
“Y/N? Where are you? What’s going on?”
“I…”
You were cut off by a coughing fit which caused blood to spill from the corner of your mouth. Both of the boy’s eyes widened at your wine of pain.
“I… I’m sorry.”
“What?” Jason was frantic now, moving around the cave to gather his weapons. “What’s going on?”
Your breath shook as you struggled against the pain. “I didn’t see it in time…”
“See what?”
“I’m sorry I left.” You dismissed his question to continue your ramble. You had to get the words out. You had to tell them. “I had to get out…I had to-”
Tim repeated his brother’s question, but once again it didn’t register in your mind.
“I didn’t want to leave. But it was the only way and I miss you. I miss you all and I need you to know that I love you all so, so much-”
“Y/N!” Jason cut off your rambling and you went quiet on the other end of the line. “What didn’t you see?”
“The knife.”
Jason’s blood ran cold. Suddenly your state made sense. The ragged breathing and the cries of discomfort. “Tim. get a tracking signal.”
“On it.” His fingers were already trailing the keys, working away to find your location.”
“Where are you?” Jason was tugging on his mask.
There was no reply. Just pained wheezing. Then the line went dead.

During your time as a vigilante, you had often thought long and hard about how you would die. And you supposed this wasn’t far from it. You had always imagined you would go down in a fight. Shot down in action while saving the city, or whilst sacrificing yourself to save one of the boys. In a way, this wasn’t too dissimilar. You did go down trying for the greater good. Trying. But you hadn’t imagined that you would be alone. You supposed it was better for everyone this way. The one thing you definitely hadn’t accounted for was the cold. It dug down deep into your bones, stabbing away like a thousand tiny needles. It made an odd contrast to the burning of your chest. Though slowly, you started to feel nothing at all as you began to drift into a numb haze.
Gazing up at the sky, spots danced between your vision, blending in with the stars that twinkled above you. Smiling, you watched them dance around until one feeling began to blend into the other, until there was no feeling left but the darkness of the sky above you.
At first, you didn’t realise that there was a face besides you, until you were brought back to reality by the stabbing pain radiating through your gut. You screamed, writhing as the knife was eased from your chest. For some reason, he wasn’t wearing his mask, but from the way that his hair was jostled, you could tell that it had not long been torn from his face. You could see the outline of his face in the light.
“Jay.” You whispered.
You could see his lips moving, but you couldn’t understand what he was saying. He looked panicked, speaking to someone that you weren’t able to see. You squirmed, trying to see who was near you, only to be held down by Jason.
Pressure to the gaping wound made you cry out. Then Jason’s lips were moving faster, and everything seemed to hit you all at once. Jason was calling your name, tapping your face gently. You could hear the murmur of Tim who talked to himself as he worked.
Your head rolled on a loose neck as Jason tilted it towards him.
“Stay with me, y/n. Please.”
Everything hurt, and your mind throbbed with your fast heart rate. It was hard to focus on anything with your doubling vision making everything swim around you.
You whined as you were shifted on to your side so that the younger of the two could begin to secure the bandages around you. You tried to focus on Jay, who had rested your head in his lap and was combing your hair through his hands, but you couldn’t. It was just too hard.
Your body began to slow down. Your eyes dropped and head lolled in his lap.
“No. No no no.” Jason was pinching you now trying to get a reaction from you, but your eyes only fluttered as you struggled to keep a grip on consciousness. “You have to stay awake.” He pleaded.
“Hurts…” you forced out, face contorting in pain as Tim continued to work and apply pressure to the laceration.
“We know. We know we’re sorry.”
“I’m nearly done. You’re doing so well.”
It felt like you were trapped in a never ending, agony filled cycle by the time that Tim finally finished bandaging you up and rolled you back into your side.
By this point, your eyes were spotting and your mind was swimming. You couldn’t even comprehend the pain. Your eyes fluttered, barely open a crack as you felt your mind wander, soon the faces around you blurred into the rest of the world and for one moment it was just you, consumed by the pain. And it wasn’t long after that until there was nothing. Just darkness.

hc that for their fifth anniversary Neil gives Andrew a joke knife, one of those with the retractable blade.
at first Andrew was like :/ but then he discovered the power he had in his hands.
no one knew of the existence of this knife, so Andrew, with Neil's acting skills, gave more than one Fox a cardiac arrest, like Matt, Allison, Dan, Kevin and obviously Wymack pretending to stab Neil .
Aaron and Nicky found out the hard way that the knife was a fake: Andrew was bored, Neil was gone, and Nicky was being annoying.
the fake knife was physically the same as another one he had, so the rest of the Foxes had no way to distinguish between the real one and the fake one.
Neil loves to steal it and pretend to dramatically stab his friends. (very dramatically) and more than once he has pretended to bury it in his heart after some "hurtful" word from Andrew.

What is the truth to those scenes where people will be fighting and not realize they’ve been stabbed, then look down at see the knife (or other sharp object) and then collapse? Can that happen? Is adrenaline that powerful? If someone stabs a knife into someone’s stomach/intestine area, surely they’d feel SOMETHING at the impact, right?
It's extremely possible for a character to be stabbed and not notice immediately, especially if their adrenaline is already pumping from fighting. Lots of people report just feeling like they were punched in the moment. Of course, the second that adrenaline wears off it's big ouchie time.
NOTE: obviously these are not scientific journals, so take everything with a hearty grain of salt.
There's a lot of answers in here saying similar things, along with the aftermath of realizing they're been stabbed.

how would you go about treating a sword through the abdomen?
Heyyy, I am so sorry for the long response time! I would not be surprised if you've forgotten about sending this in the first place.
To answer your question it's important to know if the sword is a through and through or not. Because you said "through" i'm going to treat it as such.
Then it's important to know if 911 is available. Because most sword wounds happened pre-emergency services, i'm going to write it like you'd 100% treat it yourself. Otherwise step 1 would be: call 911.
IF ANYTHING LIKE THIS HAPPENS IRL, ALWAYS CALL THE EMERGENCY NUMBER AND DO AS THEY SAY. THIS IS FOR FICTIONAL USE ONLY.
Now for the treatment of a stab wound to the gut:
First and foremost, DO NOT PULL OUT THE SWORD until you're ready to deal with the bleeding.
Step 1: Make the stabbed person lay down. Because the stab wound is probably front to back, make them lay on their side. This reduces the risk of falling after the adrenaline stops. Which could cause more damage, think head trauma, or disturbing the sword making the wound worse.
Step 2: Remove the clothing around the wound. This gives better access to assess the damage done and get a clear working area.
Step 3: Put on gloves. Easy to forget, but it lessens the chance of infection in the wound. If you can't at least wash them as thoroughly as possible.
Step 4: Prevent bleeding. Severe blood loss will cause shock or worse death. So any blood that can stay inside, should stay inside. If the sword hit a (major) artery the person could die within a minute when the sword is pulled out. WHICH IS WHY YOU LEAVE IT IN FOR AS LONG AS POSSIBLE.
Arterial Bleeding: The blood will be SPURTING out. It will be a bright red color.
Venous Bleeding: The blood will be oozing out. The blood will be a darker color.
Capillary Bleeding: The blood will slowly come out. It will eventually stop on its own.
To prevent bleeding, apply pressure. You can still apply pressure when the sword is still through the body. Just put pressure around the blade from both sides. and since the knife went through. pressure on 4 sides basically. Be careful not to move the blade too much as you do this.
Chest wounds should be sealed with a credit card/plastic bag/duct tape etc, that sticks on 3 sides, leaving it so air can go out but it cant go in. This helps prevent a collapsed lung.
Step 5: Remove the sword. Get ready to put real pressure on it as soon as the blade is pulled out, because it will probably start gushing. If its not that bad, you can wait until it stops. If not, the wound will need to be sealed/stitched. Now the abdomen has a lot of nasty things called organs in it. Lets hope we missed all of those, because if not, intestine contents can do horrible things to the rest of your body.
Close the Wound Under These Circumstances:
The wound is large and refuses to stop bleeding
The wound penetrated through the entire skin (you’d be able to see underlying tissues)
The wound has been open for less than 6 hours
The wound is over a joint or moving part of the body and won’t close by itself
The wound is gaping open and won’t be able to close without your intervention
You close a wound only when it's dry, preferably with butterfly bandages or adhesive tape, (sutures only by profesionals or in dire circumstances). Or if you want to go for dramatic, cauterization as absolute last resort.
Open or closed, it's now time to cover the wound. You should first use any form of antibacterial treatment, preferably conventional medicine, but if you don't have that, use honey. Its antibacterial too. Then you can cover the wound with a bandage, make sure to change it every so often for a clean one. Every 12 to 24 hours.
Open wounds should be covered by wet dressing. Closed with dry. Make sure to pack around the puncture wound. If you can't find sterile bandages you can boil rags before using them.

Villains don't play fair
CW: blood, stab wound, sadistic antagonist
"You know, I expected a bit more of a fight, kid. Gotta say, I'm a little disappointed. however, I think I know what your problem is." Villain stalked forward and gazed down at Hero's pale and blood-soaked form, who had managed to prop themselves up against the alley wall.
Villain crouched, wrist resting on their knee while the other cupped Hero's bruised cheek. Hero glared through the tears pooling in their eyes and Villain clicked their tongue, eyes trailing over Hero's face covered with a sheen of cold sweat.
"Your problem is, Hero, that you play fair."
Villain stood, towering over Hero for a moment before landing a strong kick to the hilt of the shining dagger still shoved in Hero's stomach. Hero brought shaking hands to the wound, slumping to the ground with an agonizing cry.
"But villains don't play fair, now do they?"

thinking of this oddly specific whump trope about an orator / political figure whumpee angering a crowd through their words or speech & cue the chaos. team tries desperately to get whumpee out of the mess, pushing and pulling against the masses, shoving and slapping away hands reaching out for whumpee but amongst all this whumpee feels a sudden sharp pain in their abdomen and there lies that knife and the blood dripping down slowly and the rabid eyes of the assaulter meet whumpee's pained ones as they stare at the blood gushing out of them or on the contrary they feel a muzzle of a gun press against their abdomen as it goes off with the silencer intact, all while collapsing in the arms of the assassin in agony

Their back and head connecting with the mattress jostling their wound, eliciting a thud and a loud groan from Whumpee. “Aahh”, they hiss as their legs involuntarily stretch out, hands searching for something to hold onto.

Pain knifes through you. You grit your teeth against it and pull your overcoat closer to hide the wound. Your steps are slowing, your legs getting weaker, but you can’t stop, not when the others need you—
A spasming cough makes you double over. Blood speckles your hand. That’s not good. That’s pretty bad, in fact. But you walk on, suffering in silence as snow falls lightly through the dark.
A little stab wound won’t phase you.
The cold seeps into your bones, and you shiver. Another slice of pain under your ribs. But they need you, so you keep going.
